DO NOT place your loved one in this facility. I was moved there from the hospital and it was 3 days later before I was evaluated or even talked to. I had a hip replacement and had to manage to do everything myself. I was in a room with another patient who was so neglected, I started taking care of her needs myself. We constantly rang to get help for a pain med and my roommate to be changed. Someone would come in and instead of helping, said they would send someone in to help but no one ever did. My roommate and her bed were soaked in urine before I finally got one of the competent staff to come in and take care of her needs. I filled out a menu daily but never received what I requested. When I finally went to the dining room to see why I didn't get what I requested, I was yelled at in front of the other residents and staff that they weren't going to accommodate me and they don't always look at the menus handed in. The entire 5 days I was there, there was feces and urine in the bathroom and the room was never properly cleaned and sanitized. I was told by a nurse to contact housekeeping. The tv stopped working and I was told that I would have to contact maintenance. I spoke with other family members of patients and staff members who also had many negative things to say about this facility. The day before I left someone came in to finally check my wound. The stay at this facility was an absolute nightmare and did more harm than good. I shared alot of my concerns with social services and the director who pretended to be concerned. I did speak to a few staff members who were competent and caring and it was expressed how unhappy they were with the way the facility was run. This is the first negative review I have ever written but after this horrid experience found it necessary to do so.

Run as far away as u can from this facility I have been here 3 days and have waked 10-12 hrs for pain meds I had hip surgery can’t get out of bed without assistance staff assistance many times over 20 mins given inconsistent stories as to why medication not been dispensed correctly as well as according to schedule major staffing issues very decentralized care no coordination between various staff members. Went so far as to call Julie admin on Friday up to midnite I was three days post op broken hip surgery at 7:4o pm 4/11 have not had pain med opioid) since 8 am today

Took my mother in law to Orange city and they have super helpful staff members, beautiful and clean facility. we had a great experience.

During Covid is the hardest time. They allow window visits but take along time to answer phone and open blinds. My uncle needs help and we were on the outside and could not help him. He needed tissues. Not sure how often they check in him. We were outside the window for 1 1/2 and no one came in. Some days are ok. People are here because they are sick and need attention. No one wants to be a burden. I have questions and no one picks up phone to answer or help out. It’s sad for patients like my uncle. He feels isolated and has bad anxiety.

My mother was first here after a fall and had hip replacement surgery in May 2020. My mom felt like she never received anything nutritious and her health was degrading and not improving. They didn't check her allergy list and served her eggs one morning for breakfast. They were giving her something daily that by mid-morning to evening, she was so out of it she couldn't think, focus or complete a sentence. We were not able to visit due to Covid19. Make sure you check their medicine list, ensure all allergies are noted if coming from the hospital. Talk to your loved ones daily and at different times of the day. Ask them what they were served for food and if they liked it. Ask them about their room... can they reach their water or any personal items or are they moved away from easy reach. Ask them how long did they wait for staff to come help them to the bathroom. If they seem out of it and can't finish their sentence, something is wrong and they may be overly medicated. I do not recommend this place.

First of all I will give the pt rehab people a 5 star. The food I give a no star. Do these people go to culinary school to learn how not to cook. Terrible. The nursing staff was sometimes good, about 1/3 of the time. A lot of the time I had to wait up to an hour to get my meds. The place was run down and dingy. Again I want to give thumbs up to the rehab people especially Irma.
