I am a friend or relative of a current/past resident

DO NOT PUT YOUR LOVED ONES HERE!!!!! This home LOST my uncle who has stage 4 cancer and dementia. For five hours before notifying anyone and possibly even noticing!!! Called after midnight asking if we had him after an ambulance found him and took him to ER!!!! blantant disregard for anyone's care. Cant explain how he lost his front teeth with no SIRs filed and lost personal belongings and all they can say is he is on hospice he's not our problem. But you're sure being paid for him to be there!!

Grandma is in a rehab called New Hope Post Acute Care. The rehab people come in to get her to exercise, but she refuses to do them and if they push her at all, she slaps them. They kicked her out because she's been violent with the staff. Most of the staff was great, but the only thing I didn't like is that it was really hard to get the nurse to come to her room. Several times I had to go out into the hall and say we need their help. She didn't like their food. I tried it, and their food wasn't the best. Her room was pretty nice. It was a shared room that was more spacious than a hospital room. It's a clean facility. When the residents hit their button, they didn't come right away. Some people had to wait hours.

I am a friend or relative of a current/past resident

My dad is in New Hope Post Acute Care right now. A couple of weeks ago, he was rushed to the hospital because he was suffering from acute dehydration, and they kept bugging me about signing a DNR, but I wouldn’t do it. Everybody thought he was on his deathbed, but right now, my dad is up and walking because I insisted that they feed him certain food, and that if he was not eating, they had to tell me. I put him on Ensure and now he’s out there walking around again and nagging me about wanting to get out of there. Less than half of the staff has a degree or the license so they’re like assistants of the assistants, but they’re very good. You could tell they love their patients and they’re very comforting with them, but the higher up you go, the worse they behave. The place itself seems nice, but it needs better people.

This was the most horrible place to ever leave your loved ones !!!! Instead of helping my grandmother get better they made her worse. The food is the worse I think that that people in jail eat better then this !!! We even found rat droppings in her oatmeal!!!! We removed her from this facility right away.

My father died in this facility because of lack of care. It gets hot in Tracy. My father had Diabetes, so he needs to keep hydrated. He was in his room for 3 days without air conditioning. They said they tried to feed him, but he would not eat. They sent him to the hospital. What they found was food on his tongue, in the back of his mouth which rotted. His kidneys were shutting down. When they sent him back to the facility, he went downhill and died days later. I kept hearing from the staff, "he had so many problems, he could die of anything." What a poor statement and attitude! Is this what they feel about all their patients? There was never a incident report filed. They should be checked by the State often.

I am a friend or relative of a resident

My mom stayed at New Hope Post Acute Care because of her doctor. I could not move her in a different city because the doctor could not go outside Tracy. The staff were inefficient. Sometimes my mom would go to the bathrrom herself because she had to wait for someone to help her. There were not enough staff in the facility. The food was the worst. They served potatoes almost everyday. The room was small and seemed like they had no private section. All the closets were lined in one wall, so the residents had to cross each other when they went to the closets. It was mediocre. The only thing that impressed me was their recreation. They had bingo, games, toured around the city, went to the mall and the movies.
