I am a friend or relative of a current/past resident

My mom is at Sunnyvale Post Acute Center. The people were very kind, and my mom was welcomed very warmly. The facility is like a one-story multi-wing building, and there's a big courtyard in the center where people can go and get some sunshine. There's a fountain in it. The rooms are nice, but there are no showers in the bathrooms. The social worker is very kind. The room is more like a hospital. They have a workout area and a little library. I wish the residents had a little bit more outdoor space.

Sunnyvale Health Care Center was fine. They had good people that were very reactive to our needs. It was clean and friendly.

I am a friend or relative of a resident

Family member fell at night when unsupervised, and was readmitted to hospital with head injuries. Family member was found with no fall monitor on many visits. Family member was left in wheelchair for hours with no stirrups and no circulation in feet. Clothing was lost by laundry. Family member lost 25 lbs. in a month and a half from poor diet. We had to bring outside food to maintain weight. Rehab staff made a sincere effort to help, but after-hours supervision of this facility needs improvement.

I am a friend or relative of resident

I'll give the CNA's and PT's five stars for their gentle, loving care towards my Mom, but I give one star to two of the incompotent RN's one by the name of Jazziel and the other Mars. Jazziel was accusatory and overly suspicious. She wanted to know "what was going on here" while I along with my brother and sister were giving my Mom a kiss goodbye when we we're leaving after our visit. I said to Jazziel, "we're just saying goodbye to our Mom." Jazziel insisted my Mom take Vicodin (which my Mom didn't even need) My Mom did develope two very serious bowel obstructions because of the Vicodin. I told Jazziel what I read about Vicodin causing severe constipation amongst other things, and all she could say to me was, "I don't know what you've been reading." I told Jazziel to contact my mother's assigned doctor and have her discontinue the Vicodin; Jazziel never did, because she was after my Mom the next day to take the damn drug. My Mom refused the Vicodin and my Mom told me that Jazziel was saying that, "the doctor say's you have to take it." Thank goodness my Mom refused. The bowel obstructions my Mom suffered caused her abdomen to swell severely. My poor mother was in excruciating pain and all Mars, the other RN, would do is administer enemas and suppositories! This only complicated things worse! One evening my Mom was crying in so much pain from the bowel obstruction (this was the third day of the obstruction) and I told Mars that my Mom needs to go to the ER. All Mars could say was, "oh she'll be ok." Thank God my Mom survived this horror. Later I told my Mom's regular doctor what happened and he said that my Mom most certainly should've been transported to the ER and stat! Another inferior aspect of SVHCC is the fact that they allow smoking on their campus. My Mom's room was right next to the outdoor smoker's court. Sometimes when I visited my Mom her window would be open and cigarette smoke would be wafting in! This is a serious health threat! How is this even legal? SVHCC is also an old facility that with lots of uncarpeted flooring that echos noises day and night. How do you expect to recover from egregious sounds day and night? I got my Mom transferred out of SVHCC as soon as I could and got her into another facility closer to home which was a palace compared to SVHCC. My Mom is in her own home now, I'm her caretaker and guardian and I love her, cherish her and I'm blessed to have her with me as she continues to improve her health with diet and excercise and no toxic drugs pushed on her.

I visited this facility

my children were not allowed to visit their grandfather. So if you like that no children are allowed then this is the place for you.

I visited this facility

My grandma stays here and the staff seem attentive and on the ball. The Center is well-staffed and appear to always have someone available, if need be. They also have a nice garden area, as well as something of a physical therapy room.
