My Mom was treated very poorly in this facility. A nurse wiped my mom's feces on her nightgown and on the pillow next to her head (I have photos of this, which I sent to facility administrators, who never addressed it in any substantial way). My mom had stage 4 lung cancer and had been prescribed 10 mg of methadone every 6 hours by her palliative care doctor in October 2023, but when my mom was admitted to Haven in January 2024, they cut her meds to 5 mg with no explanation and we had to beg and wait for her to even get that dose. She was in agony for four days until I had her palliative care doctor contact the facility so she could get the dosage she'd been prescibed for months prior to coming to that place. The entire facility smelled like urine and my mother's room was not cleaned a single time the entire 10 days she was there. Her patient buzzer often went unanswered for hours. I would NOT recommend this facility to anyone.

my mom was there a year ago and it was awesome. unfortunately she is back from a fall. the place smells like pee! people are very rude!!! food is the worst!!! as the p.o.a. of my mom i informed the facility to tell me of any change of medication before it happens and that has not happened!!!! no one called me to inform me of the fall!!!! very upset and will be reporting this!!!!!

This facility has caring staff, both nursing, housekeeping, and food management, however, they have no time management, are disorganized, and often forget what they were supposed to do. They also employ more agency folks (temporary nursing) than they seem to have staff. That means there are a lot of medication mistakes (ones that could kill a patient), and lots of misinterpretation of patient orders and times. Not sure if that is because they use military time or something else and these agency folks are unfamiliar with what they are doing. They are ill equipped for any dietary needs so if you are gluten intolerant, are a cardiac patient, a diabetic, etc., too bad. They cannot accommodate you immediately, and so you either starve or have your friend bring you a protein powder to make your own protein drink. The head of the kitchen tried but couldn't seem to order food for me for about 1 week. I left 2 days later. Getting fresh water daily is up to you. if you can't get out of your room, too bad. If you want a shower good luck. 2 times a week, unless you are pushy, that is. They need a new administrator and new staffing head nurses because both shifts are clueless to what is going on and the administrator is clearly not equipped to organize nor comprehend staffing needs and routine tasks that should be mandatory. My recommendation is find somewhere else to put your loved one. Since there has been a Covid outbreak, there are no activities. Everything is cancelled (if ever there were activities.

Run, don't walk! In my opinion, inhumane! My 86-year old mother is under the care of Haven recovering from a broken back. They do not give her meds as prescribed. When she asks for pain meds, she often hears, "I'll look into that and get back to you," and then never hears back. Even after 3 attempts over a several hour period, she did not get her pain meds until I called. She's in excruciating pain and was left to sit in a chair for an hour and half begging for someone to put her back in her bed. They did not listen to her, and again, did not do anything until I called. Last Sunday late morning/early afternoon, my mother called me crying because she was in so much pain-- she had not received any pain meds since Saturday evening. The nurse I spoke with confirmed that was true. She's supposed to get her pain meds every 6 hours, so that means she should have received her pain meds 3 times during that period. I spoke with the Director who insisted his staff follows protocol even though the nurse I spoke with confirmed that my mother had not received any pain meds over an 18-hour period. I have to ask, is Haven there to help people or torture them? We will be getting my mother out of there ASAP!

Please do not send your loved one to this facility. If you go to medicare.gov and type in this facility, you can see they have been cited for harm or potential harm to a patient or patients recently noted by the red hand symbol. This post was written on 12/30/2020. I placed my mother at this facility and it is not a decent place at all. On 12/9/20, I called my mother at 6:46 pm. She is bed bound and needed to have her diaper changed. At 6:48, I heard a woman say she would change my mother but needed help. I stayed on the phone with my mother. No one came back. At 7:46 pm , I called the facility and spoke with a nurse named Kim who said she would get help for my mother. I called my mother back. At 8:11pm, no one had came in to change my mother. I called the facility again and finally at 8:15 pm she was changed. My mother sat in her own feces for 1 hour and 29 minutes. If I hadn’t called the staff twice, how long would it have been? My mother was left without her hearing aids for three days. When I finally got the Director of Nursing (Shantelle) on the phone, she told me she dropped my mother’s hearing aids on the floor because she had put them in her pocket without the case on Friday night and wasn’t sure if they were working. Instead of checking them out by placing them in my mother’s ears, she locked them in her office. No one has keys to her office and we had to wait till Monday morning for her to get them back. I can not see how the Director of Nursing seems to think it is acceptable to leave a patient without a way to hear. My mother needs help eating and no one helps her. I have called and I am told they will send someone to help her. I then call my mother and wait on the phone with her. No one comes. I am pulling her out of this facility asap. As a registered nurse myself, I am embarrassed that people in my profession are caring or rather not caring for patients in this nursing facility. Please do not send your loved one here. With the lack of care, they could end up with a pressure ulcer or a much worse outcome.

My father was only here a few hours before we knew he had to be moved. His care team was mostly unresponsive and uncaring. He did think the therapy team was good. I checked on him early the first morning to find him very upset and disheveled. He was short of breath and waited 20 minutes for someone to check it. A patient fell in a room next to him, his air conditioning didn’t work and it was extremely loud day and night. I gave two stars for the therapy team and the fact that it MAY be okay as a nursing home but not a rehab facility.
