I am/was a resident of this facility

The staff here are really nice. I liked tauna, Jody, sam and madea. Rosy the house keeper and Eric the wound nurse were really good and nice. This was the second time doing rehab here and a few people remembered me. I love the activities staff krystal, jasmine and rachal. I had three roommates while I was there and the first two were bad but the last one was cool because we were the same age and went to the same high school

Citadel Post Acute did exceptional care. They were outstanding. The staff members were pretty good. Sometimes there was a lack of communication, but otherwise, they were good. My husband's room was comfortable, adequate, and clean. It was good. The food was excellent as well. It's a beautiful facility. The way it was kept and decorated was very nice.

If we could give zero stars, we would. Where to begin... - Checked my mom in to recoup from a hospital stay. Immediately upon entering their facility, we smelled a strong urine odor. If you smell that, turn around and walk out. We had no choice as there were no other beds available at various rehabs. - Staff could care less about concerns we had. Paint was chipping off the wall in her room, call button pushed numerous times over her 4 day stay, with no one responding, food was lackluster, activities/therapy were a joke. The list goes on and on. - Staff lost her clothes and could not find them. After 5, (yup... FIVE) voice messages, admin called back. The clothes were apparently in a plastic bag, in an office there all along. - We finally transferred her to another 5 star facility. If you want your loved one taken care of, AVOID this place like the plague.

We chose this facility for my elderly mother who needed a few weeks of mobility therapy. The hospital said that this is a 5-star facility, so we were excited at first...until my mother was checked in. Long story short, the nursing staff are all rude and incompetent, except for Sam. He is the only nurse that will give you a clear answer. The other nurses will answer your questions with "I don't know" or "so-and-so handles that". And that's IF they even acknowledge you! My mother would pull the emergency cord and the staff wouldn't come for at least 30 minutes. Her roommate vomited on the floor and the staff didn't come to help her clean up until 90 minutes later, bagged up her clothes and left it in the bathroom for 2 days before throwing her belongings away instead of washing them like they're supposed to. DON'T SEND YOUR LOVED ONE HERE UNLESS YOU HATE THEIR GUTS AND WANT THEM TO SUFFER.

Citadel is one of the good ones! The staff genuinely care about the patients and the therapy staff were amazing! We would recommend Citadel to anyone!

I have a family Friend who was placed here and was apprehensive at first - based on the reviews I saw. I was pleasantly surprised it was nothing like the reviews said. My family Friend has been at other facilities that were terrifying with poor staffing and inedible food. Citadel has far exceeded our expectations with a clean facility, really caring staff and case managers who are eager to help us find placement so we don’t have to go through this again. Food is good and overall we’re really happy

This place was horrible! I stayed there 4-5 days before my wife saved me. The first night my wife had to come by at about 11:00 pm to give me my meds because they still weren't available for me. The whole place went into COVID lockdown. My roommate tested positive for COVID. Sam is the greatest guy working there. There was no PT because we couldn't leave our room. Food was horrible and small portions. I pushed the help button several times and never did get help. They just ignored it. I am being totally honest!

Took hours to get your medicine,never answered the help button, room very small, food was awful and not nutritious, given a shower like you were an animal, was given medication that was not prescribed, security was very poor , most of staff was very unprofessional, never want to go back and definitely would not recommend to anyone

My father was in a very weakened state and sent here from the hopsital to gain strength. In the 2 weeks he was at Citadel, he was never bathed. He was to weak to sit up by himself of even feed himself, but every morning he was placed in a wheelchair and expected to fend for himself. He needed help with toileting. I don't believe much was ever done to help him. He left the Citadel and returned to the hospital because of huge sores on his rear from sitting in filth, and sore all over his legs from not having them elevated as recommended by the doctor. Never once were we advised about his condition or his treatment plan. Never once was he given a bath. If you have any compassion whatsoever for your loved one, do not place them here.

This place is absolutely awful. My dad was here for 14 days and on his 14 day he was positive for covid. Nobody returns your calls. I have called dozens of times in the 2 weeks and if the front desk even answers they say they will leave a not. I have guardianship and not once did they call me to discuss his care plan. They put him in a broken bed even though he is a high fall risk. They only gave him one shower even though I constantly left messages. His room phone was broken so it was extremely hard to reach him. None of the amenities offered were actually available. One of the times he fell they sent him to the hospital for x-rays and I found out because the hospital called. When I called citadel they said he was in his room and everything was fine. They spent 14 days neglecting my dad, not returning my calls and today they sent him to another facility with out my permission because they gave him covid.

If I could give a negative review, I would. This is a horrible facility and I would never recommend it to anyone, especially an elderly person who is confused, ill and in need of quality care. Let me start off by saying I understand policies have changed some due to COVID; however, I do not believe not involving the family in treatment plans of a patient is an acceptable practice in any situation. I could write a very lengthy review detailing every aspect of my dad’s care and the contact the family had with this facility. I’ve decided to be as brief as I can. The visiting policy is terrible. You must continue to leave messages with the activity person in order to schedule a visit for the next day. Visits were held either in a visiting area in the building for 20 minutes and only if there was space. Otherwise, you were allowed to visit in an outside patio area in June in the Arizona heat. This does not seem like a very good option for a 90 year old man who is sick. In order to talk to staff, you must continue to leave messages, hoping someone will call you back. When staff did return phone calls, they were open to providing updates. But not including us in the treatment plan. The only staff member in this facility who treated my dad and the family with respect and kindness was the Speech Therapist. Once she was able to contact us, she continued to update us. The receptionist at this facility is the rudest, most unprofessional person I have ever had to deal with. She lacks any kind of compassion or empathy for a family going through a loved one’s health crisis. I’ll end by saying, after eight days of endlessly trying, I was finally able to take my mom to his room to visit him. Seeing him for the first time in his room, I knew he needed to get out of there. I was finally able to transfer my dad out of this facility. However, it was not to another rehab or skilled nursing facility, it was to a hospice inpatient facility, where he passed away four days later. Even if his passing was inevitable, not allowing his wife of over 40 years, which he continually asked for, to be with him during that week he was in their facility was cruel and unbelievably insensitive. And I’m sure after reading other reviews about this facility, there will be a reply from them with excuses. However, this was our experience with this facility.

I’m writing this review after I’ve had time to deal with the frustration and anger I have at the care my father received at Citadel Post Acute Care. My father had lost the use of his legs after a fall in his home and after a short stay at a local hospital we transferred him to Citadel to receive physical therapy. It was a nice place when my grandmother stayed there many years ago so it seemed only logical for my father, but as time went by we started to notice a difference. The staff seemed to lacked compassion for my father who depended on them for his nutrition, hygiene and hydration. He could never reach his water bottle, was rarely shaven or given the opportunity to brush his teeth. The container used to empty his catheter was normally left on the bathroom sink near his toiletries. When visiting it was quite common to find used medical gloves on the floor, his nurse alert button under the foot of his bed, dirty linen and clothing lying about. There were instances where we had to let staff know he defecated and needed to be cleaned. The smell permeating throughout his room making it quite obvious, but staff members normally claimed they could not smell it. My father was considered a fall risk and he was fitted with a fall alarm which was attached to the bed and his clothing. Like his alert button mentioned previously, it was just as common to find his fall alarm on the floor and disconnected. I got so tired of the excuse of being undermanned during the shift, and then observing staff comparing “selfies” at the nursing station and generally screwing around. My dad told us he could not feel his legs so the staff was told to reposition him when needed. Thinking back it amazes me that we, the family, had to tell trained staff to move him from time to time. The staff again failed to follow our wishes and basic patient care, because my Dad developed a bad bed sore. In the center of the facility there is a patio area for residence to enjoy some time outside. It was quite disturbing to see several homeless cats drinking from the residence water dispenser. I found it appalling to watch a dirty cat licking the same nozzle that residence would fill their cups from. There was always piles of cat food were in the parking lot to attract cats. I have to deal with the guilt I feel for leaving him in that facility for as long as we did. He did not progress in his therapy; in fact he is worse off today then when he was admitted. He in a much better place today and although he still can’t use his legs, he is clean, well fed and basically happy. I can only speak of the care my father received and the observations I made, I cannot speak for the other residence, but I have no reason to believe my father was singled out and treated differently.

I am thoroughly disappointed in the care my father is currently receiving at this facility. The nurses and the CNAs don't seem to care. My father will have his nurse light on for 30 minutes and no one will even come to check to see what he needs. It seems that there are only a few people that actually work at this facility. They don't really pay attention to their patients needs either. My father will sit in a dirty depends for half a day without anyone changing it. He'll also ask for water and it'll take an hour for him to get fresh water with ice. I have to say for a facility that I was told got a 4 to 5 star ratings I can honestly say it is run like it's a one star motel. I am so disappointed.

My wife was admitted to the Citadel after breaking her hip. The care was horrible: it took hours just to get a pitcher of water, pain meds were ALWAYS late, when she needed help getting to the toilet no one responded and she had to lie in her feces and urine for hours until "help" arrived. Her iPhone was stolen the first night. Patients were yelling, swearing and moaning. If you love someone DON'T send them here. I pulled my wife out after 2 nights. I wrote 2 letters of complaint and didn't even get the courtesy of a response. This is a disgraceful excuse for a care center!

I visited this facility

Citadel Care Center was very well kept, and the staff did a fine job. Most of the work was done by the caregivers; however, there was always a nurse readily available when needed. The food was OK. Because my mother was pretty much in hospice the whole time she was there, we really didn't get involved in the activities, but there were quite a few. They had a day where all families were there. The overall atmosphere was good.

I was in the facility in April and may of 2014. I was there after a knee replacement. The care was very good but I agree there is not enough staff. It is an older facility and is not very bright not enough sunshine. I am going back to another facility where I have been before. They insist I go out and eat with the other patients and it has lots of sunlight.

Nursing always understaffed Dietary has people who don't care the man in charge I believe his name is Mark is always in his office playing games on his phone or computer if you try to talk to him you feel like you are nothing but bothering him I ended up pulling my MOM out of there and one nurse thee told me how to file a complaint with the state she said maybe that will help to get the place to change she asked me not to use her name as she needs her job

My sister in law was in this care center and I liked it so much that if or when I need a care giver/provider I would choose this one. She had Alzheimers Disease and could not have gotten any better care. My husband and I were always invited to get to gethers during the holidays. The night she died we were called by her nurse on the shift and we were told we should get there as she did not think my sister in law would make it through the night. the nurse even shed a tear and told us how they would miss her. She had been in a few other places in the Northern part of the state and they always stunk. We went to see her at Citadel every week and she was there for close to 4 years and not once did we get that smell. Was always extreamly clean. When sister first arrived she was able to go to the market or outings which she dearly loved.
