About Southwestern Illinois VNA - Swansea in Swansea, Illinois

Southwestern Illinois Visiting Nurse Association is one of the oldest freestanding not-for-profit home health care agencies in the country. Founded in 1918 to combat the deadly world-wide influenza outbreak, SIVNA is guided by a tradition of personal, clinical and technological excellence. We are dedicated to providing coordinated services, in cooperation with other organizations, to improve and maintain the personal health and safety of all members of our community.

SIVNA's Case Coordination Unit is funded by the Illinois Department on Aging, Area Agency on Aging of Southwestern Illinois, and the U.S. Administration on Aging.

AVAILABLE SERVICES (linked by our Case Coordination Unit Case Managers):

Homemaker:

Homemaker's assist with household tasks such as house cleaning, laundry, shopping and errands. Assistance is also provided for personal care such as dressing, bathing and grooming, instruction in meal preparation, medication reminders, and escorting our clients to physician appointments.

Home Delivered Meals:

Under Title III of the Older Americans Act, seniors may be assessed for individual needs such as home delivered meals. If assessed as eligible and in need of a home delivered meal, the client will be allowed to participate in this government subsidized program. The home delivered meal provider assigned to you will discuss the start of service, special circumstances that may exist such as special needs diets, and will discuss a suggested donation amount which, together with Federal and State assistance, will meet the cost of producing and delivering each meal.

Adult Day Services:

Adult Day Care supervises and cares for clients in a group setting outside the client's home. Services may include counseling, exercise therapy, as well as social activities. It provides seniors the opportunity to meet others of the same age group while enjoying fellowship. Transportation to and from the Adult Day Center is provided or arranged for when assessed as necessary.

Senior Companions:

The Senior Companion program provides visits by another senior for friendship, companionship, and social interaction

Caregiver Support Program (Respite Care):

This service allows caregivers a brief period of rest or relief from their care giving duties. Example of caregivers are: a 76 year old woman/man caring for their spouse who has suffered a stroke; a 52 year old woman/man caring for their 80 year old mother who has Alzheimer's Disease; or a 27 year old woman/man who regularly volunteers to help their frail neighbor.

Medication Management:

Southwestern Illinois Visiting Nurse Association's Case Coordination Unit employs a full time Registered Nurse who has the responsibility to assist those clients - referred to her by fellow Case Managers - who are in need of organizing their medications, have confusion regarding their personal medications, or simply need assistance in assuring themselves that they are taking the proper medication, as prescribed by their personal physician, as directed.

If you feel any of the previously listed services are appropriate for you or a loved one, please contact us. We will send an application for services form to your attention. You may also use our online form to request an application. This application must be completed and returned to Southwestern Illinois Visiting Nurse Association's Case Coordination Unit to ensure proper consideration. If necessary, a friend or family member may assist in completing the form and may sign their name on behalf of the applicant. When the application arrives at SIVNA, a Case Manager will contact the applicant and arrange a personal visit in the applicant's home to determine all specific personal needs.
