I am a friend or relative of a current/past resident

No facility is perfect, but I am very happy with the Fountains overall. I am an RN so I am very aware of how patient care should be provided. My mother has been a resident for rehab for two admissions. The rooms are clean, and more spacious that our local hospital. The surroundings are pretty, with gardens, fountains and places to enjoy the sunshine with family. The staff are friendly, caring, and gentle with the residents. Everyone seems happy and eager to help. There are activities everyday. Most are provided by the staff activities department, but there are also people from the community that come in and provide music, karaoke, and best of all a wonderful weekly church service. Other activities are bingo, card games, group stretch and exercise, crafts, nail care and current events discussions. There is a beauty parlor with hair cuts, perms and styling available at very reasonable cost. The therapy department is very good. They encourage the residents to do the prescribed therapy with care and safety. I am very impressed with the food. Every meal that I have seen has been well prepared, pleasantly presented, in more than sufficient quantity. There is a good variety in the menu, so you are not bored eating the same thing every day. Mom is on a regular diet, I am not sure if this applies to all the special diets. Overall we are both very happy and I feel blessed to have such a wonderful facility to help to care for my mom.

The room and most of the staff took wonderful care of the residents. HOWEVER something needs to be done to improve the food. I filled out my likes and dislikes to NO avail. Kitchen staff would say the dietician was here and talked to you and clearly that wasn't true.

My friend is a resident at The Fountains Yuba. I saw all levels there, they have the physical therapy, and it was a nice, a newer building. The care was pretty good. They didn't have chairs that were comfortable for the patients to sit in. There's no place like home, where you can have a comfortable chair. I visited my friend a year ago. It was a nice big room, and the bathroom was for only 2 people. There were few things that weren't perfect, but they rate closer to a 5. I wasn't the patient, but I visited there twice a day, so I got to observed quite a few things. They had a gym. They had a little coffee places where you could have your coffee. Covid wasn't a problem at that time, so you have a free run at the whole building, and that was nice. They have a garden, where you can go outside. Excellent on cleanliness. The room was very nice, they've furnished chairs, and they had air circulation. It was nice.

The grounds and facilities are well kept. The nursing staff was always helpful. Therapy did a grest job......The food is terrible and the altertinive chooses are not much better. I met the Dr. on the first day I arrived with his bad adititued and saw him one more time on the day I left, same bad adititued.. It seems that overall quality of caare increases when visitors check in. I would NOT reccomand this place for my friends or love ones.

My dad was in The Fountains Yuba City. He was agitated, but they took good care of him. My dad started feeling better, but he just didn't like the food at all. The care is awesome and they have some good people there. It's a skilled nursing community, it's all it is, for therapy. The staff was really good, from the nurses, to the CAN's, meds assistance, and everybody. He was diabetic, and they accommodated him. He did therapy, speech therapy, they helped him to the bathroom, and what they could do basically. I liked the staff very much. They're really good about that.

I visited this facility

The Fountains is a full nursing care community and I like their program. If you get discharged from the hospital right after an operation for example, they send you to The Fountains, and they get you back into your own room if you're already in a senior community. I think it's a pretty good deal, and it looked better than other nursing places I've seen. It appears to be a nice and clean community, it's not cluttered up, and there was no odor or anything. The therapy facilities looked about average. When you go in, you walk through a common gate where on the other side of The Fountains is another senior community.

MY SISTER, AGE 65, IS CURRENTLY AT THE FOUNTAINS AND I FIND IT TO BE CLEAN AND THE STAFF TO BE OK. THE STAFF SEEMS WILLING TO HELP AND SEEMS TO BE GIVING HER THE RIGHT MEDS; DOCTORS CHECK ON THE RESIDENTS...ALTHOUGH I HAVE NOT SEEN A DOCTOR THERE YET, BUT I USUALLY DON'T GET THERE UNTIL 10 A.,M. THE PHYSICAL THERAPIST SEEMS OK. WILL CONTINUE TO REPORT

I was at The Fountains for a week for rehabilitation following surgery. The whole staff was kind, professional and caring. The facility was clean and comfortable and the meals were good. I would recommend The Fountains to anyone who needs physical therapy and recovery care.

My stepfather was at the facility the rooms were very clean and spacious he liked all of the staff members sometimes a little slow on getting his meds but otherwise he was happy with the fountain

I am a friend or relative of a current/past resident

This place need to be shut down . My husband mom suffered a broken nose and a severe head injury when they claim she fell ,while they weren't watching her . And a lady sharing a room with her begging for 30 min please nurse I need to potty while the continued ignoring her . I finally said she needs help they said we know but she has scheduled bathroom time and she would have to hold it that it wasn't her time yet to go. Please don't take your loved ones to this place.

I am a friend or relative of a resident

My mom was in The Fountains and just got out a few days ago. She was getting therapy and getting rehabilitated until she can go back home and live on her own again. She's been there four times since she's had four major surgeries. I've visited the facility many times. It's an excellent facility. The inside was always kept clean and just very nicely decorated. You can take your family members outdoors, there's sunshine and it's just beautiful. She's staying in a shared room and it's pretty spacious. It was really modern and excellent. The facility's been there a few years but it's well kept and well maintained. Everything in there is modern and updated. It's really a nice place.

The facilities and grounds of the home are quite nice. They could clean the bathrooms more often, but other than that the staff is wonderful and great with my grandma.

I am a friend or relative of a resident

My dad is in The Fountains and currently staying there for more than 3 weeks now. He had a series of falls and he was in the hospital for a week. Then he was transferred to the nursing home. The place was actually nice and they have very nice facilities. The staff were friendly and professional. They are very caring. It's been a hard situation for us, but we are fairly satisfied with their service.

I visited this facility

My grandfather stayed at this facility following a hospital visit for some serious age-related health problems. He was on life support in the hospital. After he was realeased from the hospital, it was reccomended that he stay at this facility before returning home. The staff was wonderful and very nice with amazing bedside manner. They took the time to get to know every patient, their needs, as well as their families/friends. They were also very accommodating. The rooms are very spacious; however, the patients do share with one other person. My grandfather happened to know a fellow patient and the staff made it to where they could share their room. I feel as if the company and conversation that this provided lifted my grandfather's spirits leading to a faster recovery. The staff is not pushy, but does encourage the patients to participate in activities that will lead to their recovery. They have facilities - indoor and outdoor - for them to get up and around and practice maneuvering again. My grandfather also complimented the food often joking that it was much better than the hospital. I enjoyed visiting my grandfather very much during his 3-week stay at The Fountains!
