I am a friend or relative of a current/past resident

Administrator does not return calls and has no clue on how to run this facility. I never received any phone calls on how my father was doing or what their care plan was for him. Basically there was no communication what so ever. Food was . He stopped eating because he had not idea what he was eating. He lost so much weight the 2 weeks he was there before we had him discharged. When he was released the nurse ran over my dad's catheter tube and wound vac tube with the wheel chair. He shoved 2 boxes of medical supplies at me with no instructions on how to use them. We had to ask for my dads personal items. The social worker was suppose to call in his medications and that was not done in advance. He laid in pain for a couple of nights only to be given melatonin. Which in actuality it was his catheter which we found out once he got taken to the emergency room. They are under staffed and staff is not equip with the training they need to help the patients that are in this facility.

Do not place your loved one in this facility My mother arrived in the facility and was transported from a hospital. We were there 1.5hrs before anyone came to the room- no one acknowledge she had arrived. A receptionist came by with a gift bag but that was it. I located the nurse to discuss her medications and I was told by the nurse her medication had been submitted to the pharmacy and he would have them. She received one of her medications the first night and did not receive her medications the next day. An Admin specialist called me to assure she had received them BUT she had not. The nurse verified that she did not get the meds. I called and asked why and spoke with a woman she stated there was "glitch"! I asked to speak with the Dr. or have him/her call me. I never received a call or other. My mother asked several times for assistance as she is incontinent- she had to lay that way for over an hour. We had a horrible experience and had to move the family member after 2 days. Staff is nice and caring however, the facility is very understaffed and staff may not be adequately trained. When the call bell is engaged it will take a minimum of one hour for anyone to check on the resident/patient. They will die before anyone checks on them. Don't expect to have any questions answered.

My husband arrived in the facility. He was transported from a hospital. I called the nurse to discuss with her about his medications and I was told by the nurse his medication had been submitted to the pharmacy and he would have them. Well that was not the case he did not receive his mediation all night until the next day. I called and asked why and spoke with a woman she stated they were headed into a meeting and they would get back to me in an hour or so. She did call back sooner and told me that his medication had arrived. On 04/14/2021 he asked several times for assistance to the bathroom with one nurse coming in several times. Most often he said they appeared to be under staff. When they weren't they would completely ignore and not address his needs. He was to have physical therapy daily. No therapy on 04/16/2021- 04/18. Left to sit in a chair. He was not given his insulin daily or other medications. He received his insulin on 4/16 thats when he realized that he had not been taken it. He was constant pain in the evening nurses wouldn't bring in medication.The did not take his sugar daily only once while he was there. He never saw the doctor. On 4/19/2021 I was really concerned about his well being and welfare I contacted the main number that would connect to the corporate office and told them the situation. With that being said my husband called me later and asked if I had called someone because they were attentive to his needs for a short duration. He was on oxygen and they allowed the tank water to be empty. After not checking it. This was even after I had called. He was told that they would return to clean his room and physical therapy and they never returned. Financial personnel came in to speak with him about what benefits he received and was insistent on him having his benefits to come to the facility. He said that he had to tell her not to put pressure on him because he was not interested. He was so disturbed by this behavior and the other nurses that he started to become uneasy about speaking to me on the phone. We spoke daily 2-3 or more times a day. I had been concerned since 04/14/2021. He started sayin that he could not speak. At this point I felt like he life was in danger. I had not known he wasn't getting his medication especially his insulin nor his sugar not being taken. I concluded and he agreed that I would pick him up on 04/20/2021. I called and made them aware and asked when could they have him ready to be picked up. I was told that was up to me. I told her I could be there in an hour. I told her I would arrive at 9:30. I received another phone call asking was I picking him up for the day I told her that he would not return. She asked why and I told her the quality of care. She said okay they would get him ready. He was given no medication from the evening of 04/19/2021. When I arrived at 9:30 they said they would bring him out. He came up out sweating and flushed. He said they had removed his oxygen an hour or so before and he was having great difficulty breathing. I rushed him to the Hospital. I fear that had I not picked him up I cant say what would have happened.

I am/was a resident of this facility

I was recently admitted here after having surgery and was required to be on a wound vac. When I got to the facility it took hours for anybody to come introduce their self or check in on me after being transferred from the stretcher To my new room. I waited and waited and waited for the nurse to come in to reconnect my wound VAC. I requested my pain medication as I was in terrible pain and in tears. I was told I couldn’t have any. The next day my blood was drawn And again I kept asking for my pain meds because of the pain I was having. I was still told no I couldn’t have any by the nurse on duty. I experienced minimal help from the staff and ringing “the Call bell” For what seemed 45minutes to an hour at a time before anyone came to assist. Later this evening I was sent out to the hospital for my wound and was told that it appeared to be very infected. I was crying Hysterically and so scared I was going to die here! The dr came in and gave the nurse an order to give me one pain medication to hold me until I arrived to the hospital. I begged for more because so many doses were missed and I was in such agony! I WILL NOT BE RETURNING TO THIS FACILITY! No staffing, no help and the staff doesn’t care!

I visited this facility

Consulate is very clean. The staff is very professional and so willing to help the patients with any problem. They’re excellent that way. The rooms are immaculate, beautiful, and brand new. I can’t say enough about that part of the facility. It’s very nice. The dining room is excellent; it’s very bright, sunny, and open. It’s beautiful.

My father recieved very loving care here. He was well cared for after having him in many bad places. The caregivers were kind and loving.
