Woodbury Wellness Center, Inc., established in 1998, is a local privately-owned Long-Term Care facility centrally located between Wilmington and Jacksonville, in the small coastal community of Hampstead. We offer close proximity to medical services and other amenities that our residents may need and desire yet are nestled in a small quiet residential community that provides the feel of home.

We offer a continuum of care and lifestyle choices on our campus to better meet the needs of Hampstead and the surrounding communities. Stonebridge at Woodbury provides Assisted Living and Memory Care services, and Woodbury Wellness and Rehab offer Short Term Rehabilitation as well as Long Term Care. With these various service levels on our campus, our residents can easily transition from one level of care to another as their needs change, without ever having to leave home.

Our rehabilitation program is as unique as the residents we serve. Our team, including our doctors, nurses, therapists, dietitians, social services and support staff, work with residents and families to meet each individual’s specific needs. We offer personalized care planning for residents who are recovering from surgery, illness, accident, injury, or disease to facilitate their healing process and enhance their quality of life. With Medical Provider services daily if needed, on-site Nutritional Services, on-site Therapy Services, and other services such as X-rays, labs, and counseling provided right here on our campus. The goal is to return you or your loved one to a home environment that will be focused on with intensity with discharge planning beginning the day you arrive. . We will all work together as a team to help ensure that you have a successful transition back to your home as soon as possible, and with all the tools you need to help you stay healthy once you get there.

Woodbury Wellness Center strives to deliver the highest quality rehabilitation services to our clients and residents. Our rehabilitation staff is skilled in treating most conditions, including orthopedic, neurological, surgical, cardiac, oncology, and other diagnoses. And once your transition back home, you can elect to continue your therapy rehabilitation services with our in-home therapy programs. If you are in need of Long Term Care services, Woodbury Wellness and Rehab provides comprehensive services to those requiring an ongoing inpatient greater level of care than that offered by independent or assisted living facilities with our extended care services. Extended care promotes autonomy and choice, and offers a variety of services, social activities, and recreational opportunities while providing ongoing clinical care to promote and enhance your quality of life. Our community provides excellent 24-hour medical supervision in an environment of compassion and nurturing by our highly skilled staff.
