I am a friend or relative of resident
My 81 yrs. old Mom, non-English speaking, was transferred from hospital to Allis Care Center for rehab after falling at home due to pneumonia. I have spent 4 days helping her and doing Allis Care Center staff job, thus our experience with staff and generally this facility is HORRIBLE!!!!! Here is why: 1. Blood thinner was not transferred from Freudtert to Allis Care Center Medical Sheet and was not given to Mom until I inquired about missing medication. 2. Friday evening (1/16/2015) upon admission, a walker with broken brakes was provided to Mom by staff by name Shanika (I believe, hope they can check her name and schedule).The same night I left my cell phone number and post it not on TV with my cell phone for staff to call me if Mom or they had questions. Apparently, I found out next morning from Mom, she pressed alarm /call button for nurse for help to go to bathroom. No one showed up whole night! I took her to bathroom and washed her at 7:00am. She said that she couldn't use walker as it was different. 3. On Monday morning physical therapy staff was terrified about unsafe walker, and took it away expressing a highest concern regarding Mom’s safety. Monday morning PT staff noticed that breakfast was not delivered, which was second day in a roll, according to my observations. Staff that I asked for breakfast responded with attitude that I should have asked someone else as she was busy. 3. Attending doctor, as per nurse that was helping on Mom's floor, made a mistake prescribing medication for cough, so pharmacy held it because of interactions with blood thinner. All time spent there, including at discharge as well, Mom was not seen by doctor, not to mention that no one even cared to check her lung. Results of x-rays, results of blood sugar level, blood pressure level or temperature were not communicating either to Mom or to me as her POA. Thus her health was worse and she decided to leave. Discharge paperwork was mailed to my home address, as social worker was concerned that I could pursue a law suit. 4. Upon request for discharge admission staff and social worker of Allis Care Center apologized to us stating they were not aware of issues. Interestingly enough, wouldn’t you think just from entering building and smelling urine that staff is not doing what they are paid for? Wouldn't the first impression of nurses and supporting staff on personal cell phones be indication about lack of management? 5. Lastly, for all case managers, especially Nicole from Freudtert, previously bad comments about this place are current. From my perspective as a daughter, a care giver and being case manager for the last 15 years, Allis Care Center needs to be closed out due to lack of safety, unsanitary conditions, non-responsive staff and neglect of patients.

West Allis Care Center had been one of the best nursing homes in the city several years ago. I recently had a friend there following a broken hip. When I visited the entrance was cold and uninviting. The noise level on the nursing unit was high with residents calling out and the staff calling back and forth to each other. My friend required oxygen. The home did not have piped in oxygen but administered it with a compressor unit hooked to the back of her wheel chair. It was LOUD. She said it was like that all night also and made it hard to sleep. While I was there she asked an LPN to go to the bathroom. The LPN (Licensed Practical Nurse) said she would get her aide to take her. 20 minutes passed and no aide showed up by this time my friend had an accident. There was no reason why the LPN could not have taken her .she learned how to do that in her training. At meal time they asked my friend if she was going to eat. My friend said no and they left it at that instead of just coaxing her and taking her to the dining room. I took her there and she did eat about half. My friend was there 3 days before physical therapy was started and that was the main reason she was there. I would not recommend this nursing home to anyone.
