I am/was a resident of this facility

I love the rooms but the janitor doesn’t always clean the room. He doesn’t move anything to sweep under. He doesn’t move things to mop. Thus lots of germs hide in the room. I do not receive my medicines at all and have repeatedly asked for them. No one comes to get me for rehab so I lay in my bed 24/7. They write untrue documentation to my insurance company saying I refuse services and medication. I have called Adult protective services which is a joke in Texas. Now I know to call others. If you are considering this place I say run away as fast as you can! I have not had a hot meal in a week and have been force to eat stale bread and a thin slice of lunch meat. A half serving of overcooked broccoli. Not USDA quality. The staff talks loud at all times of the day and night. It is not uncommon for them to talk loud at 3:00 am and they always have the TV up loud overnight. It is a terrible place

My grandfather was placed in skilled nursing at legend oaks after he fell and broke his hip. Upon intake, no notes about his care were followed by nursing staff. He left a week later because of complications. Upon readmitance, the staff starved my grandfather for 4 days. Refusing him food through his feeding tube for 3 of them, and not checking the machine the 4th. They do not follow NPO orders. And they were cited for several things this week in regards to not following doctor orders, not following am npo, and essentially for starving an 80 year old man and laughing when he asked if he was going to die. If you value your loved ones, do not send them here. All citations are public record, and i suggest you look at them. Don't bother asking to go to activities. The nurses will lie so they don't have to take you. Don't look for kindness from any nurse. As soon as you leave, family that can't defend themselves are treated poorly. You can sign a waiver to have a camera in the room. I highly highly suggest it.

The rooms are just ok. I had to switch rooms, the first one was absolutely awful. The bed in that room was not appropriate for someone who needed physical therapy. The staff for the most part were friendly. There are a couple who need to change professions. They obviously hate their job and are very bad at patient care. On the other hand the rest were very attentive and caring. The PT & OT staff are outstanding

My dad was there for rehab on 07/01/2022 from 08/18/2022 from a fall and break he’s hip had surgery was doing good just to need help on walking again he was in good health….nothing else. and he went from doing good to bad to end up in the hospital he got not one but 2 kinds of infection for a something that could been helped if they where on it just to clean it but they took there time on everything and poor cleaning and very poor and careless never keep up on he’s med and didn’t clean him up when need to plane lazy on them so in the time my dad got worse had to call 911 something so small went to almost death oh and the workers was yelling at one another in hall way very unprofessional to get anything done we had to push them to do anything thing even one of the nurses rolled her eyes at me when I was telling them what had happened and why he end up in the hospital because of them not clean him up!!!! I we’ll never and I mean never take anyone there poor places I would Invice no one to take their family members are friends there……..

My brother is at Legend Oaks Healthcare and Rehabilitation. The layout is good but I think it was a little bit better when it was brand-new. It's a nice facility, and the rooms are nice and a pretty good size. Each person gets a television that they can watch, the facility furnishes them with a TV. If you're a Medicaid patient, you have to share a room with someone else, but they try to put you in with someone that would be pretty much compatible with you. The room is big enough for two people and they have a curtain that can be drawn if you want privacy. They have their own little door too. Those rooms don't have refrigerators in them but private rooms have little refrigerators in them. Some of the staff, I think, are there just because of the money. I don't think that they're there for the people. Some of them are there for the people, but some of them aren't. The nurse that takes care of my brother is extremely good. The nurses there are good. They have bingo, crafting, singing, and sometimes they'll have a parade. My uncle who's also there very much enjoys all that. They have a wonderful physical therapy facility and wonderful physical therapy. They have a great big dining area and the food is pretty good.

Don't be deceived by the newness of Legend Oaks Rehab. My 91 year old uncle was there for 3 weeks and we found most of the nursing staff to be apathetic, unresponsive and not proactive. Apparently it's ok to allow the patient call buttons to ring for 20-30 mins during peak times. When you're visiting, you'll continually hear the alarms for the call buttons. When he was short of breath, instead of doctors figuring out the root cause, they put oxygen on him. At the hospital, we found out he has blood clots in his lungs from inactivity. He was in excruciating, burning pain from urine that had backed up (due to an enlarged prostate). They gave him meds after finally getting his urinalysis results back. This was the one time that he received alot of attention because he was yelling out in pain for a few hours. He was transferred to the ER and we moved his stuff out. No other family members will ever be going here again!!
