    I visited this facility

    Oak Springs was a mess for me. It was terrible. The beds were in rooms about the size of one of my closets with a curtain between them. It was a very crowded, hospital-looking place. It did not look clean, and the people were very grungy-looking. The care and the smell were a no-no for me.

    I am a friend or relative of a resident

    My mother was in Oaks Springs of Warrenton. She fell at church and broke her hip, and she was in there for rehab. It's very nice. She was in a shared room, which was small, crowded, and loud, and I didn't like it. She didn't like the food in there, and she didn't eat very well. They had church, singing, bingo, crafts, and physical therapy. The have a director that takes care of all of that. I didn't like the place itself, but the people were very nice.

    I have visited many nursing facilities; most are the same. Oak Springs has the same challenges at times; however, the staff are willing to do whatever it takes and the residents are happy and taken care of. Oak Springs feels more like a home than a sterile acute care facility- In my opinion that is what matters.

    I am a friend or relative of resident

    I waited nearly nine months to write this review as I wanted to make sure that my comments were not grief-driven rather than objective.My father passed away on November 23, 2014. My father was a resident of this facility in the Fall of 2014. His stay was little over a month and he was pretty miserable most of the time he was there. Some of the staff were decent people who cared. The management of the facility left much to be considered. Within a few days of my father's stay he was given a roommate. This was not unexpected. However, the roommate was totally incompatible with my father. My father had physical issues, not mental deficiencies. The roommate my father was assigned with had mental deficits and started the relationship by refusing to use the bathroom and urinating in the trash can. This was done on several occasions; twice in my presence. It took complaints to have my father moved to another room even though staff was aware of the problem. Eventually my father was moved to another room. Many of the nurses assigned to my father (less one) lacked compassion, smelled like stale cigarettes and appeared to be simply drawing a paycheck. Things really started to go south when my father's medicare (and direct payment was implicated). My father was unable to do the physical therapy. He was VERY sick. When this happened, the focus changed from making my father comfortable to figuring out how to discharge him. This was NOT the therapist's fault. He was a decent man. However, the people in the social work department were in a word PATHETIC. They knew little about my father's circumstances. We got conflicting information (when we got information). And please understand, we were not uninvolved. Someone from our family visited my father at least every other day. Once the direct money trail ended, it was all about business, not care. And understand, our family had some ability to pay. But the people in the social work department were so uncaring, so confused, so incompetent, our family removed our loved one from that facility. By the time we removed our loved one from this place, my father was not eating, had given up any desire to live and told me directly that he wanted to die immediately simply so he would not have to be in this place. To underscore this point, my father died peacefully about a month later at a place he loved and cared for by people who really cared about him. DO NOT PLACE YOUR LOVED ONE AT OAK SPRINGS. YOU WILL REGRET IT!

    I am a friend or relative of a resident

    My mother is in Oak Springs of Warrenton. I picked them because they have an Alzheimer's unit that has full care. This place takes beautiful care of her. They have medicines to make her stop crying. They bend over backwards to get her back to where she is actually smiling, and I love it. I love that place. It is very pretty. They have a garden with tomato plants for each different unit. They have rehabilitation, a full long-term nursing care, and full long-term nursing care for people with Alzheimer's and dementia. The floors are clean. The food comes in on a serving cart that they bring to the table. It is not pre-plated; they plate it right there while they are waiting, and nobody gets cold food. It is absolutely wonderful, and the set-up is unbelievable. They have activities like music, movies, and karaoke. A guy comes in and sings. They play something called jingo. They have 3 or 4 different churches that come in on Sunday mornings. Every other week, they have a black gospel choir that comes in about 2:30 in the afternoon. I witness it when I come in there, and my mother loves it. They just think of just about everything.

    The nursing/care provided by the facility is not good for people who need the support going to the bathroom, dressing, preparing or cutting up the appropriate foods, regular vital checks, etc. Patients wait for hours for someone to respond to them.

    I am a friend or relative of a resident

    This facility has a nice facade to it with most of the staff being very friendly. Since, my relatives have been there I have seen that the CNA's do not treat the patients correctly in some cases. Considering that there are two wings to nursing home, there are differences to the types of CNAs that attend my grandparents. Some are caring and do their job, but others will allow my grandfather to sit in his own filth for hours on end. The only good thing I can remark about this facility is that they do have plenty of activities for the residents and family to attend and participate in.

    I visited this facility

    The grounds are spacious and open with a friendly staff. The staff is very enthusiastic and involved in the lives of the residents, and they put on regular activities. There is a large recreation room where residents gather for snacks and occasional parties. There seemed to be a calendar of daily activities so that there was always something going on for those who needed socialization and mental stimulation. I remember there were games in the TV room, and many of the residents knew each other well and loved chatting for hours. They were mostly free to spend their days as they wished, and the staff was very helpful and encouraging. My extended family sat around in a semi-circle and watched sports before breaking out into a sing-song session, and everyone was entertained by our shenanigans. I was impressed by the overall cleanliness of the facility and the attentiveness of staffers. It was also very peaceful but still full of life.

    I am a friend or relative of a resident

    My grandmother was a resident of this facility. She entered the facility needing more care and was in the skilled nursing unit.. she received excellent care in this dept. The nurses and CNA"s had smaller caseloads and were very attentive to her needs. Unfortunately as she got better she was moved to the regular floor and unfortunately she did not receive even close to the same level of care! We ended up pulling her out and moving her to America House since her level of care that she needed had signifcantly decreased and she received far better care there!!
