I visited this facility

We were very pleased with Southern Hills Rehabilitation Center. The lady that gave us the tour explained everything, but since it was a Saturday, there was no one in their main office, so we couldn't talk with anyone but the lady that gave us the tour. They've got probably everything that my mom needs. The overall condition of the place was fair. The only negative thing that I would have to say is probably their shower room. They've got one shower room for each hallway and it looked kind of old.

The people here are rude. They obviously aren't concerned about how well they do their job, or in other words, how well your stay with them is. They do the bare minimum, and I, and my grandmother, had to ask for help with her bedding and cords several, several, times before receiving the help. This place is a dump, and mostly because of the people who work there.

My mom was at Southern Hills Rehabilitation Center for three weeks, and then insurance said she had to be transferred out of skilled nursing, so she's in another facility now. We were very happy with them. She got great care. Sometimes they were short staffed, but I think everybody is nowadays. She had excellent care. The nurses were very good at taking care of her. We don't have any complaints at all on them. As far as activities, my mom is paralyzed except for her left arm, and she had just had major back surgery. But the time that she was there, she mainly was just in her room and didn't get out a whole lot, so she did not participate in a whole lot of activities. But they would come to her room and do physical therapy. On the days that she felt up to it, they would take her down to the actual room to do physical therapy, but a lot of times it was in her room. If I had any complaints at all, it would have something to do initially with the physical therapy during the first couple of weeks, and part of it was her own issues that she had. She was recovering from surgery, so she didn't feel good, and then she developed some bad blood clots in her leg that limited her for the first couple of weeks. So the first few times, the physical therapist would try to have her get up and out of her bed on her own, but she's paralyzed, so she can't. They would come in and say, 'Okay, get your shoes on and get out of bed,' and she was like, 'I'd love to do that, but I can't.' So I don't know if they were not reading the charts well enough to know what her limitations were, and I'm not sure what this deal was. I was up there two or three times a day every day, and I came up there, and my mom was in tears, and I asked what was going on. She told me kind of what was going on, and I called up the nurse and we had a little meeting, and it was resolved very quickly. So they were very professional, and it was great from that point on.

Rooms are too small, looks like housekeeping hasn’t visited the rooms in a long time. Needs a paint job. Walls in the rooms are all scratched up, paint peeling, windows falling apart, filthy floors, food is horrible, food is delivered late and delivered cold. No one checks in on you enough to see if you’re thirsty, need diaper changed, to feed you in a timely manner. Not enough staff working there especially after 5pm. Found out that several patients had to go to the hospital with UTI because they weren’t given enough fluid throughout the day. This place is where you send a loved one if you don’t care enough. Place is horrible!

My mother was in Southern Hills Rehabilitation Center. The facility is kind of rundown, but she had excellent care there, they took really good care of her, but we wanted her in an updated facility. The place is very old, they don't have phone lines in the rooms, which was a definite negative because my mom is used to calling us. We had to get a hold of somebody at the front desk and have them take the phone into her room to talk with her. We tried a window visit, but because the window was old and occluded; it was kind of difficult to see her. The facility needs a major update, it's not very clean, but as far as the food, it was excellent, and the care was five-star. She kind of actually misses the people. My mother is not a real people person, but she said they were really so nice to her, and they took such good care of her that she kind of misses being there. She said everybody was so nice. The room was small, like a hospital room, and it didn't have any furnishings in it at all. It only had a bathroom and a toilet and a sink, there was no private shower, there was no furniture there. It did have a TV, and they were trying to make improvements in the place, but when I went inside I could tell, I'm a fairly clean person, and I was desperate to get a hold of a broom, 'cause it needed some serious cleaning. I don't know anything about the activities. My mom was real sick, but they did physical therapy every day, they did a good job with that, some occupational therapy, some speech therapy, and she was still recuperating. They were charging a little too high for the facility, the care was good, but they need some work, big time, on the facilities.

Was at facility 5 years ago. Was great. Change of ownership. Was there after. Nursing procedures not followed. Call lights unanswered for up to 3 hours. Trash removal spotty. Medical care sub par. Physical therapy space poorly equipped (no cycling machines, for example.) Staff overworked or just don't care. If you have a say in the choice of facilities, avoid this one.

I have seen good nursing homes and bad nursing homes. This nursing home is great! They give excellent care along with excellent therapy. With the culture change is a big deal these days and they are definitely keeping up with that. The rooms are private for therapy residents and that is a big deal for those residents and their therapy. Staff always has a smile on their faces. There are multiple activities going on and all residents are welcome to participate if they want. They offer anything from doing their laundry, resident trust, supplies, beauty shop is all included. They take all insurances from Medicare, Medicaid, private pay and some insurances like long term care insurance. Just give the facility a call they will tell you everything!

The interaction between associates and residents is second to none. You're greeted with a smile as soon as you walk in and can tell not only are the residents happy but also their employees! I was very impressed to find out their physicians are in the building 4 days a week and are always available by phone. I would recommend this facility to family and friends looking for skilled therapy to return home, long term care or employment!

I am a friend or relative of resident

If your loved one needs a lot of assistance do NOT chose this place. The therapy was amazing but the lack of everyday care left a severe pressure sore on my friend so he was sent off to a hospital then a post acute care facility to get it treated and lost all the benefits of the therapy he received. It's really sad but so hard to find a place that offers top of the line therapy AND daily care. We are currently looking for his next residence because we will not allow him to be sent back to this facility.

If you love your family member don't even think about this place, the staff changes frequently, my mother was there for 2 years and we checked in on her every week! She became very ill and they told me my mother was exaggerating I tried to convince otherwise when my mother was on her death bed I'm the one that made the call to 911 When I went up there and found my mother lifeless they still insisted that she was Ok, My mother had pneumonia, she was in renal failure! They had did a urine specimen from my mother who was complaining about burning when urinating Two weeks previous and they never checked on the results, Which made my mother septic and very I'll because she wasn't treated! No person should ever have to go through the pain and suffering that is still continuing like my mother is. I feel sorry for the other residents who don't have a family that checks on them often.
