I visited this facility
The Forum at Tucson doesn't take Medicaid, which was why we didn't choose this place. It was a hospital setting, but it seemed fine. The rooms that I saw were fine, and they had a lot of physical therapy.

I am a friend or relative of a current/past resident
One of the worst facilities in Tucson. My mother has been there in the rehab skilled nursing side. She has been left in a urine soaked brief and bed, with the urine dripping on the floor, for hours. They don't respond to patients for up to two hours. Patients have yelled and thrown things to get someone to check on them. Patients are left unattended for hours. Facility is understaffed. Patients are told Someone will be right back to help, but no one shows up. Patients are told that they can't possibly get to everyone. Night staff,mostly the CNA's, are awful and uncaring. They need to hire better qualified staff, especially the CNA'S.
