I wouldn’t give this place anything.. my brother had a Brain issue they totally neglected him… I wasn’t there for a couple days came back his room was a mess… he was in need a medical attention! Within 16 hours at hospital he is 100% better … this place should be shut down! I will be placing a formal complaint !! DONT SEND YOYR LOVED ONE

To be completely honest I wouldn’t even give this facility one star if I had the option. My grandmother was there for four days and not once was she based or changed out of her clothes. Staff left her in places they should not leave her, and left her and her soiled garments on top of dirty linen. When my mother, who helps take care of her, went into the facility today she was completely mortified by everything that she saw. The facility was highly unsanitary, dirty floors in the room and bathroom bowls. The staff was nervous as if they already knew that they did something wrong, and still would not comply with what was being said after my mother had asked for the Director of the place. If anyone is thinking of sending their loved ones here, Ron! Please do not send them here it is not safe nor is it sanitary.

My mother is in Central Park Rehabilitation and Nursing Center. It's fantastic, it's a five-star facility, the staff is very professional, and everyone has been very supportive and very helpful in this transition. She has been as comfortable as she can be, she's been there for a couple of months now, and she's on the first floor now which is in the nursing facility. It's great, everybody has been fantastic, and I really don't have any complaints. She said she likes the food, I'm in constant with the nursing staff, social worker, and the case worker, and she's eating just fine. The food is very substantial, and of good variety. The facilities were very nice, very clean, and very secure. I've been able to see just the lobby, the outside of the building, and through the window I was able to see the inside of the room area, but everything looked very clean, well-kept, and very updated. They have music time, there are individual TVs, but there is also community TV, they have different programs, community things and individual things.

We chose Central Park Rehabilitation and Nursing Center for my mom. It's relatively small. There's four floors, but they really only have three that are for residents. The second floor is reserved for admissions and analysis work, rehab basically. The ratio of staff to residents was good, and when I reviewed the scoring by Medicare it received very high scores in relation to care for long-term patients. So far, she's had no complaints about the food. They also have alternatives, so if the resident is not pleased with what they're offering for a particular meal, they have alternatives. My mom's happy with that because there were a couple of things that she didn't like, and they were able to come up with a substitute. I actually observed a couple of meals that my mom has really enjoyed. One in particular was a tuna noodle casserole that was very reminiscent of what she used to make for us when we were growing up, and so she hadn't had that in a long time. So she was very pleased. Her room is semi-private. It comes with a bed, a dresser, and a chair. They have their own closets and there's one bathroom for the two residents that are in the room. It's pretty spacious. They encourage you to customize your space. So we have been bringing stuff in for my mom that reminds her of home, it's very personal, and she has on the wall pictures and stuff that make her happy. Most of the stuff that they have been offering, they do art classes and so forth, she's been getting involved in, but she hasn't really done an awful lot. She's just been staying in her room trying to get acclimated.

My friend was a resident here. She begged me to take her out of the facility within 3 Days of her stay. They are understaffed, cannot keep staff, staff are rude. When she was finally discharged (they only allow you to leave on their terms), she had no home health ordered, incorrect prescriptions, and no equipment. Her primary care doctor had to order oxygen. It was an overall horrible experience. Most of the rooms are shared and they never pull the dirty curtains. Rehab is the only department that will get you up for activities. Staff are Constantly yell outside room. Horrible discharge planning.

I am heartbroken to have lost my Dad. His last days were at this facility, and I can’t help but think that serious missteps were made. It came as a shock to me to find that this facility has no AED defibrillator available. Curiously enough, Florida state does not require one, but with these life-saving devices increasingly available in gyms, restaurants, and schools, it apparently is only an OPTION for skilled nursing facilities where patients and staff are most likely to be enduring stressful situations. Go figure. The administrator and risk assessment manager of Central Park did not allow me to talk with the nurse who was last with my father in his final moments when he tried to go to the bathroom and fainted, requiring cardiac assistance. The risk assessment manager even went so far as to make up what she recalled my father saying in his last days. (She later confessed that those weren’t his exact words, but her “perceptions.” The final notes on my father’s passing are unclear (and grammatically incorrect), but one thing is certain----the doctor on site was nowhere near, nor was an AED defibrillator. My father was 81---he had an aging heart—but he was only there for HIP REHAB. He had an extremely strong will and desire to live, and he had a strong mind. On my last phone call with my father, he was concerned that the staff was not doing enough to help him. To find out that a simple life-saving device is nowhere to be found in a rehabilitation facility and to be denied the ability to talk with the ones who may have last spoken with my father is unconscionable. Please keep in mind that there is a lively, alert aging population who want every effort to help them recover from surgeries to hips, knees, etc. If you are considering a facility for your mother or father’s rehabilitation please look elsewhere.
