My Experience with Transitions have been not very Good. The evening crew of nurses spend alot of time on there cell phones. Also taking really long breaks. When you ask them for help-they seem like your bothering them. And the residents I feel are not being taking care of as good as they should be. The housekeeping is poor. It smells so bad it makes me sick to my stomach. The food is not very healthy. It seems thrown together. The ceiling tiles have been stained with mold and sprayed not taking out to find the leaks its nasty. When the state comes to visit they make it look like there doing there jobs but there are not. People just don't know what really goes on here. :(

I visited this facility
The visit at Transitions was fine. I went after-hours, so the admission person wasn’t there, and I was shown around by a nurse. It was fine. The rooms were crowded with up to four people per room. I talked to one staff member who was very nice. The location was fine.

I am/was a resident of this facility
This facility will help those who aren't that afflicted. But there are no miracle cures here. Look elsewhere for severely disabled persons looking for restoration of ability to walk. Also, there is a severe deficiency in its equipment. Ownership has been advised of this situation, but doesn't seem to care. The staff, on the other hand, is for the most part, well trained and do care about their patients, though ownership's lack of concern occasionaly creates care deficiencys. Equipment is also a weak spot that ownership is aware of, but chooses not to act upon. Food is nominal.

My father needed more attention so we sent him to Transitions Healthcare for rehab. It's fairly clean. They keep the floors clean. It's a semi-private shared room. The food is marginal, it's not the best. It could be improved if they work on them a little bit. They have musicians come in so they try to give you some social activities. They supposed they did rehab for a couple of weeks but they couldn't. He couldn't progress any further so they took him off the rehab. He is always clean and fed and when I'm there, the staff seemed to be OK. I don't see any issues or problems.
