We placed our Dad at the Wagner facilty in May of 2023, and my Dad passed away in Jan of 2024. From the moment my Dad arrived, he was given the most compassionate care. The Nurses, CNAs, Administrators, Front desk staff, Chefs and Custodians all gave so much love and respect to my Dad in passing or when he had a need. We as a family were always greeted with kindness and allowed our spaces. The other residents were always ready to chat with us or my Dad. It was a hard decision to place my Dad in care but the staff showed us that he was taken care of in every way. Even in his last days, our family was accommodated with privacy and comfort in saying goodbye. I have so much respect for the team at Wagner. The facilty felt like a home and the courtyard was my Dad's salvation. Every time my Dad needed anything, he was attended to immediately and given so much patience and kindness. I will forever be greatful for the staff for taking care of my Dad when we couldn't provide the care he deserved.

Don't ever send a family member to this facility! Worse rehab in stockton, CA. My aunt was transported by ambulance to the hospital on April 9th,2023. A family member went back to rehabilitation center on Thursday April 13th, 2023 to pick up personal belongings. Social workers there could find my aunts belongings. A cross necklace with mother's ashes who just recently passed away, a notebook with passwords, a fitted shrink wrap to go around her nub that was recently amputated. I then called on Thursday to leave Taylor a message who is head of social workers. Whom a receptionist told me she would be the one to speak to. Never received a call back. Called that Friday the 14th, left a message and still no call back. Call yesterday Monday the 17th and no call back. My mother had to help and call and yell to get one to basically say they lost her stuff. They BS'd us around and have no solutions for the problem and her lost or stolen items. Things don't get up and walk away. They have nothing to say. Some workers were rude and had attitudes as if they they hated their job or didn't want to be there. And others actually showed some consideration. Worse place for any patient to have to go to.

I would not recommend this facility to anyone! They aren’t caring, compassionate, respectful nor helpful. They are careless and irresponsible. My father was under there care for a month and during this time we had problems. Not only did we observe many times how careless they were but we also saw it with other patients. Now as my father is close to being discharged they add more gasoline to the fire by trying to shame my father. How dare you act like my father would do such a thing!!! We will continue to fight for my father since he isn’t able to protect himself. We are his voice!!!! It will be heard!

Unstable management. Frequent change of admin and nursing management. You never know who's in charge month to month. 5 different Admins in just 5 years, several more Directors of Nursing in that time span. Unwelcoming environment and instability and low quality care results. Dept of Health constantly in the facility for complaints, etc.

My dad is at the rehab area of Wagner Heights for skilled nursing care and rehab. He shares a room and he goes to rehab every day. He doesn't like the place quite too much. We've had good experience as far as letting us know what’s going on, but then we’ve had some poor experience like the food is cold and not very good. He also has to constantly ask to do things so he's not too happy, and he's had his clothes stolen. The staff seemed fine and adequate.

I put my 83 year old father in Wagner heights on a Tuesday night for a week of registration. Wednesday day we were there from morning to 6 pm, he was alert happy did his physical therapy ate his meals. Thursday morning at 7, we came into his room there were no sheets on the bed, he was cold, clammy, gurgling, his eyes were hazy. I yelled for the nurse who called 911, while waiting for the paramedics to come they gave him a breathing treatment and another one back to back. My dads room was right next door to the nursing station and the night crew were so negligence, they didn't check him through out the night.. He was sent to the emergency room and put into ICU, I would not recommend this place to anyone. Unless you are willing to spend the night to watch over your love one.
