June 2022 I found my spouse covered in feces a couple of mornings. When I got there to check on her a 10 am, the earliest the let you visit, I asked the only person on the floor that worked there for help and was told they did not have an aid for the floor yet and she was a agency nurse not an aid so she could not help. She was only there for 6 days. Every day there was a different issue. It was Friday night when she was transferred there I could not check on her arrival because no one answered the phone ever at night. I could go on and on but my spouse was sent to hospital because of dehidration after they did not feed her or give her fluids for two days because of a problem with her feeding tube. Very understaffed dangerous place. One morning when I got there they had a different patients name on her door. It took 3 days to get a working tv.

We admitted on 12/6/2019. Moms weight 136#. Brittle diabetic, anxiety, hi blood pressure, feed port in belly, tracheotomy on ventilator. She was sent to the ER at Providence Hospital 12/20/2019. Restrained to a bed, unable to open her eyes and fighting, she had no idea who we were, she was cowering away from us when we touched her and kicking her legs at us. ER Dr. ordered a CT scan of her head and an Ultrasound. He thought maybe she had a fall, hit her head, there was no explanation of her behavior. She had no idea where she was who she was or who we were. Upon admittance she weighed 116#, had soars in her anus/vag area, her sugar was in the hi 400's and it took 5 days for whatever they gave her to ware off. On 12/25/2019 she was able to open her eyes knew who we were, it was everyone's opinion she was OVER MEDICATED. 12/20/2019 Nurse Tonya called to say her oxygen had dropped to 82% and they were sending her out. After several phone calls that day, she 1st stated she fell out of bed and was pulling at her trach. I asked if she been given her xanax for her anxiety, she said last night they gave it to her she was agitated, i asked about morning dose, she said we gave her adavant and immediately said I'll call you back, the next call was from Tonya's boss (trina I think) she was loud and excited said shes agitated and wont let us check her sugar, I asked you gave her adavant? when did they start that? She said to Tonya while on the phone with me, you gave her adavant? Tonya said NO! I told her that. From the 1st day we couldn't get anyone to check her sugar, Nurses never knew what meds she was on, meds were never given at the same time on ANY DAY, we asked about her meds and care and were dismissed, or told let me get back to you and they never did. She was there to be weaned off of the ventilator, at the care meeting there was no one from respiratory, we asked where they were, we were told he has another appointment. When we questioned his absence Beverly excused herself and brought back a respiratory therapist who couldn't explain ANY care plan. She also brought back a nurse (tonya) She was very dismissive and told us she wouldn't be calling us every time there was an incident. My mother was referred to as the "white woman" and Beverly was made aware of this.

My mother lived at the Lakeland Center for a year and a half. During that time she was treated very well by the staff. She made friends, went on outings, and considered it her home. The home was very clean, good food, and did very well with State surveys during her stay. I would highly recommend the Lakeland Center to anyone.

If I could give this less than 1 star I would. My adult son was transferred here out of hospital for a traumatic brain injury rehab. I read the reviews and was worried because I live far away and was frantically trying to get here. When I got to see him at the facility, his bed was covered in crumbs with food stains, the area around his bed was dirty with old clothes and trash. The staff doesn't pay attention, if you say something they are rude to you. Ask them a question and you are likely to get "I don't know" for an answer. Items were missing and they tried to tell me I must have taken them home! I received misinformation if not outright lies from doctors and nurses. During the two weeks I was there, his condition deteriorated until he could barely communicate and was rightfully angry about how he was being treated. I could go on and on and on, I have nothing but bad stories about this place. If it's someone you love, don't send them here.
