I am a friend or relative of a current/past resident
Stay away from this facility. The reason they advertise their 5 star rating is that they don't report to the state what is really going here. I visited a friend here recently who had a brief stay (thank goodness). They couldn't wait to leave. The food was horrible and she often went hungry. She overheard employees complaining about rats in the kitchen! I spoke with aids who complained about new management that is running the place into the ground. Bedside manner is nonexistent. The nurses and aides are often rude and non responsive. I saw many unhappy employees and residents. I would never choose to put a loved one here.

I am a friend or relative of resident
We had our mother in for rehab. The facilities are clean the staff is nice to the community and they try to make it a cheerful enviroment. We had a very good communications with the people who did all of our paperwork and help move mom to home, they were caring and considerate. But we had some problems with communication with the nursing staff and the caregivers, as an example, she had an injured leg and the evening staff while trying to move her grabbed her leg to help roll her. They said they didn't know it was injured, (this was the reason she was in rehab) She needed to go to the hospital at one point and the evening staff called the doctor to get the okay on Friday which he did, and on Tuesday she was still had not gone, we were not alerted that the staff had called the doctor or that the doctor had agreed that she could go to the hospital and it was only when we pressed and told them that we wanted her to go, that they was realized it had been approved days before. I felt like we were constantly checking and talking to staff and management and I am sure we were considered by them to be high maintenance because of this, but no matter where you put yourself or your parents you cannot leave them alone and expect everything to go smoothly They did try to communicate with us and make adjustments, but usually after something had occurred to start the conversation. Lastly, if your parents like to have a good meal you will have to bring them food. They have possibly the worst food at any facility that I have visited. When we asked what the meal was the staff just smiled and said they didn't know! It was undercernable as to chicken or fish. We found out that the people who live there take pictures of the food to show the low quality. At first we put it down to "hospital food" but we were wrong, it is grades below hospital food. They did say they were trying to improve it but apparently the low quality has been going on for years. I will still give them 3 stars for the facilities, cleanliness, cheerful environment, good rehab, nice people and they try. They were not mean in any way and they were helpful when asked. They have a church service on premises with an nice pastor. Overall if my mother didn't need exacting care this would be a good place.

I am a friend or relative of a resident
When my father broke his shoulder in 2010, he was placed in Columbia Lutheran for an extended stay in assisted living for rehabilitation. The facility was clean and well-maintained, and most of the staff was caring and invested in the residents, so I wish that I could rate the facility higher. We had a major issue with communication - between staff and staff, and between staff and myself. My father was suffering from delirium during much of his visit, and was very confused and easily agitated. I was not informed in a timely manner when my father was having issues, even though I visited daily, and I was not informed about issues in his treatment until they had become problems. For instance, I was not informed that my father was refusing physical therapy, which was a condition that had to be met in order for Medicare to continue paying for his care. Rather than informing me or explaining to my father, the staff allowed him to skip therapy until we were under threat of being discharged for non-compliance. I also asked several times to speak with a case or social worker about getting him home care or how to prepare for his release. I eventually did get information, but only because I was pushy and called everyone in an executive or administrative position.Overall, the facility is very clean and the residents are safe. But I am unconvinced that the staff has the time or training to ensure compliance or to help residents build or re-build their skills.
