I am a friend or relative of a current/past resident
My father was a resident here for 6 months and the management of care was horrible, This facility needs a major oveerhaul of management and admin. I dont blame the staff its management. They lack everyday supplies always no towles no pillow cases. They skim you on your food. They serverd my dad 1 piece of toast and 1 piece of bacon for breakfast. When asked for more food was told they were out. Facility was dirty and understaffed. Would not reccomend to any person or animal.

Some of the staff members at Jacob Healthcare Center are good, and some of them are not. In addition to the caregivers, there are med techs who provide the medication. We have three meals a day, and I'm in a shared living situation. I have been here for almost a year. There are many things here that I am not happy with, so I am not ready to recommend this facility to somebody else. They could improve on the food. I personally don't have enough space in my part of the room, but that's because I'm a professional artist with a lot of information and tools that I have to keep here. So, it's crowded for me.

The title of this place should be Purgatory. That is how I see the people who are put here feel about their lives. If families are concerned with the care and happiness of their family members, they need to know how this place makes their loved ones feel each day. I could write a three page review on the daily routine of the incompetence of the staff and caregiving of this place as I practically lived here with my grandmother, just to make sure the care was right. Without details, I can tell you she only lived there as long as it took me to learn that just because a doctor told me she needed to be in a nursing home, doesn't mean I have to leave her there. It just means she can't take care of herself. But I stayed with her every day. Some days till 1:00 in the morning. I was basically was taking care of her because care was not that good there. The day that I broke down and cried for her was the day of reconning for me; that she was going to die from neglect if I didn't come every day. This was the day that I had to go to my daughters school for a function. I went in at 6am like usual but this time I told staff that they would have to take care of her for breakfast and for most of the day. Starting with a diaper change. (My grandmother was blind and afraid to walk) I waited for them to clean her up just to make sure they were going to do it. I had to call them twice and it did not happen till I went to the desk and insisted it be done. I walked with them while they pushed her wheel chair into the community room where she waited to go to breakfast. I told them I would be gone most of the day if I came back at all. I came back at 11:am and there she was still in the same place in the middle of the room soaking wet and starving. I could tell she had been crying. That is five hours with no care only about twenty yards from the staff desk. Latter that day I had to leave feeling like I was a banding her. That was only one day of many just like it. The day I learned I was aloud to take her out of their was during the holidays when I was plotting to take her for the holiday and keep her. I asked the nurse care giver why their were no family members visiting. She said the families don't come visit them. They just bring them here to die. I said well my granny is going home as soon as the doctor says she starts drinking and eating more solid food.(granny was in after dehydration from not being able to swallow) The caregiver said, "No she's not. They never go home after they come here. Their kids bring them here to die. This is the last place they go. They come here to die." The next morning I checked my grandmother out and took her home in an ambulance. Her health improved impeccably. The staff's attitude at Jacobs is morbid. As far as I am concerned that facility should be shut down. I would give a negative rate if I could. But since the program wouldn't go to next step with no star, I had to give them one star. This was my first experience with a nursing care home. But since then I have been to many nursing homes in San Diego County so I have plenty to compare and this facility stands alone in terribly bad care and quality.

I visited this facility
I was just at Jacob recently. It was decent, I saw smiles and had warm welcomes, from staff and residents alike. It felt calm and relaxing.

Jacob Healthcare Center is closer to San Diego where I live. Mom has been there for about four months now. It's okay so far, but I think the food could be better.

I visited this facility
I felt that the Jacob healthcare facility did not care for their patients. I believe that overall nurses take on the personality of the facility they work in. They showed no pride and courtesy towards family or relatives visiting. There was no pride taken in keeping the grounds clean for patients and no effort was made to welcome families. I was dismayed to see my grandfather struggling to feed himself, food that he never really had to eat in his life. If we did not bring him food, I wonder how much weight he would've lost. It was heart breaking to see my grandfather in a place where there was no privacy for us to visit with him and that his bed ridden room mate smelled of urine that it made it hard for us to visit. If we were the kind of family that checked on him often I wonder how it was for patients that never had family to check up on them and insure that they were competently taken care of.
