Review of Enclave Rehabilitation Facility I had a most unpleasant experience regarding the care of my husband on Sunday, November 26, 2023. My husband is eighty-eight years old, has dementia and is in a wheelchair. I was in the room with him as he slept in his wheelchair. All of a sudden he tumbled forward and fell out of his chair onto the floor. He hit his head on the floor, leaving a gash on his head and a pool of blood on the floor. I immediately went into the hall to find someone to call for an ambulance. I had been told that any head injury should be seen by a doctor in the emergency room at a hospital. The hall was empty and I think if I hadn’t been there he would have lain on the floor for who knows how long. Two employees came and held his arms and legs and put him on a bed. It’s a good thing he didn’t have any broken bones or that would have made his injuries more severe. I told the nurse I thought he should go to the hospital since head injuries should always be checked out by a doctor. He said “not necessarily”. He said he would call a doctor though. I sat next to my husbands bed, and after waiting quite awhile I went to the nurses station where this nurse was sitting. He said the doctor hadn’t called back yet. I told him again that I thought he should go to the hospital. This nurse said with a head injury it’s only important to go to the hospital if the patient is on blood thinners. (My husband was not on blood thinners after a previous fall when the doctor told me it’s always important to have it looked at by a doctor.). I continued sitting by my husband. This nurse came in to tell me the doctor still hadn’t called back. I said that I was going to stay by his bed all night. My husband’s eyes were closed, he hadn’t been responsive since the fall, and was unaware of what was going on. This nurse said it would be against their policies to allow me to stay all night. (Even the hospital allows a family member to stay overnight in the patients room.). I said if I can’t stay then I’m taking him out of this place. I said I am not going to leave him lying here all alone! Sure enough, when the doctor did call back he said yes, he should be taken to the hospital. An ambulance was called and my husband was hospitalized. He remained in the hospital for a week, was discharged, and moved to a different rehabilitation facility - never again Enclave!!

The staff are friendly, there is always cleaning of the floors to help keep them clean and from smelling. There are a lot of activities and the one person Activities Staff is phenomenal, she provides wide variety activities, helps with admissions, resident transports, and is assigned entire hallway of residents that she visits and contacts family members regarding if there are any issues needing to be addressed. There are several nurses that are very good, helpful, and it would be more helpful if newer permanent staff was not rotated after orientation. As an RN, I know the challenges of keeping very good staff. Good pay, respect, adequate supplies, and support from leaders is the first step, to longer term retention! The loss of multiple staff due to low pay, and a couple stupid policies indicates that cooperate is not responsive to keeping high quality staff. The food is not consistently prepared or adequate for special diets, nor is time of meals that have to be delivered to residents that eat in their rooms. The rooms are nice. The head of housekeeping personally also does a great job-again better pay would be a step to retaining her staff. Fortunately for many residents their families/friends come and help their loved ones

I had a family member here and i really liked the staff. I really enjoy the therapy dog Bentley. The staff was very knowledgeable, and always willing to go the extra mile. I would highly recommend this facility.

I am a friend or relative of a current/past resident

My mother was admitted to this facility this evening. I told the nurse and director My mom was wanting to get out of the bed. When asked the nurse and director if they had rails for the bed so she doesn't fall. They said no they don't have rails on any of the beds there. Instead they put pads on the floor on both sides. Both every cold no compassion. Not a good feeling My mom there. Left very worried and heart broken.

The nursing staff is completely inattentive. My 85 year old mother developed a bladder infection and was unable to get to the bathroom on her own. The nursing staff could not find time to help or call the doctor. Medication did not arrive for over two days. When questioned they would only say it didn't happen when they were on duty. They also will not return phone calls. The rooms are completely without personality, basics such as soap were unobtainable and it wasn't clean. Repeated requests for soap or even coffee were readily agreed to but never forthcoming. Mother was supposed to be there for rehab but checked out in worse shape than when she arrived. Never would I recommend this place to anyone, ever. The physical therapists were capable and some of the non nursing staff were friendly.

The rooms and the actual Enclave itself, are clean. Our HUGE complaint is with the staff/ nurses! Mom broke her hip & is supposed to be getting HELP, so she can GET BETTER & return home. These people seem to want to make her an "invalid!" The so called rehab is very short & then she just sits FOR HOURS UPON HOURS! (20 minutes a day of rehab x 5 days) Broken equipment, negative attitudes AND THEY NEED A NEW COOK!

We had a family member placed in the facility to recover from a stroke , he was placed with a feeding tube and was also told that he was bed ridden when sent to a home for care , this place had him sedated and it seems as though he was never assisted in physical therapy even after 3 months in this faculty, we were never called about any updates and as well due to covid no one has been able to see or talk to him..

My mom moved into The Enclave. More or less it's good, but there are issues sometimes. My mother fell about five or six times, and they just go to the residents to check maybe every two hours. They don't monitor them. Sometimes I ask them for something, like compression socks or an extra mat for my mother because she tries to stand up from her bed, and after a month and a half it didn't happen. No socks or mats. Some patients have to wait about 45 minutes to get their food, and I don’t know why they take so much time. When the residents there need something, like a Kleenex or coffee, there is nobody there. If there's someone there, they don't care or they don't listen to them. The rooms are very good though and they have a beauty salon.
