This is by far one of the worst long term care facilities that I have visited. Their unit that houses dementia patients often has 1 nursing aide to care for 20-30 patients. My family member was put to bed with wet clothing that he had voided in and his room smelled like urine at all times. We had to request that his room be cleaned .ore than once & his floor scrubbed because of urine & feces. His clothing was lost even after his name was written on them with indelible black ink. The meals are substandard. If you love your relative please do not place him in this place with poor staff, poor, non nutritious meals & unclean. I have never seen them engage in activities with their dementia patients. No activities as all. Your loved one. , particularly if he or she has dementia, will receive poor care. I would give this facility a rating of F- ( F minus). Run, don't walk, away from this place!!

My father - 95 - when discharged from the hospital after a fall was placed for 5 weeks at Ashland Nursing & Rehabilitation Center. The PT (Physical Therapy), OT (Occupational Therapy) and Speech/Cognition folks were all very good - several were extraordinary including the head of PT, Yusef, and his PT associate, Dave. They all had incredible patience with my father who was cooperative some days and less so other days. They encouraged him to press on just beyond his comfort zone so that his health and ability to perform daily functions were as good as could be hoped. The PT room is moderately large, clean and well set up. The key though, are the people who went beyond expectations and in terms of genuine care, expertise and patience. The staff were also excellent in working with the patient's family - explaining their plans and efforts with the patient. And providing guidance for after discharge. Based on just the PT, OT and Speech staff, I would have rated Ashland at 5 stars - the facility is not new and there were occasional nursing/aid staffing challenges - so my rating is 4/5 stars.

The staff at Ashland Nursing and Rehabilitation Center was nice and pleasant, but when we first came in, there were no linens, towels, soap, paper towels, and things in the room. The place wasn't what I call "clean." My husband had fallen twice while he was there and hurt his arm. With him, he needed more than the care he was getting. He needed somebody to help him. I was there late in the afternoon and his tray for breakfast was still sitting in the room. Plus, they lost some of his clothes and I'm still trying to get results from that. When we brought him home, his legs were real swollen, because he was spending the whole day in the wheelchair. So, it just wasn't a good environment. Maintenance was not good either. I went in there and there were crumbs on the floor, under the bed, and things that had dropped and nobody got them up. It's all one floor though, which of course is nice, but I wouldn't recommend this place at all.

I realize that Ashland does not have the newest paint, the newest flooring, the newest furniture or, frankly, the newest anything. However, the love, devotion, compassion, and caring I witnessed staff members give to my father-in-was absolutely astounding and brought me to tears. Despite SEVERE understaffing, NO resident was left without a kind word, a little joke, a pat on the hand, and my FIL -- who was completely incoherent and bed-bound was treated with nothing but love and respect by Keisha and Kate. He was ALWAYS clean, turned, and had his hair brushed. Say what you will about the appointments of the place, I'll take loving, dedicated staff over the newest decor and freshest paint ANY day!

My father-in-law is in Ashland Nursing and Rehabilitation Center. They have 24-hour care and one-on-one care. Everybody's very helpful. The nurses are available around the clock to answer questions. The rooms are either single or shared rooms. They have a TV, dresser, bathroom, blankets, beds, and air conditioning. They do arts and crafts. They have bingo. They get haircuts.

THEY STILL SENDING HIM THE WRONG STUFF AND THEY NEVER COME TO HIS ROOM TO SEE WHAT HE LIKES. AND THEY R STILL. SHORT HANDED ON 3 - 11PM AND 11 - 7AM. AND IT TAKES THE CNA'S FOREVER TO ANSWER THE CALL BELL. HE NEEDS A SETTER
