I am/was a resident of this facility

This is a for profit, which is the main concern that they have in this place. The easiest way is to get out is to run out of benefits. It is a nice place to go if you want to do nothing for 23 hours and 15 minutes a day. And you get to ride a gym machine for 45 minutes. It is not a rehab facility. I feel like I escaped the day I left when I was driven up the gates, I cried.

My mom is now in Port Charlotte Rehabilitation Center. Her room doesn't look like a hospital room, so she feels more comfortable there. I went to more than one place and some of the other places, their rooms looked like hospital rooms. This room looked more like a hotel room. She doesn't feel like she is still in the hospital, and that's what she likes. The staff seems to be OK. When I admitted her there, they were nice to me, and they seemed nice to her when I was there. I haven't tried to food, but she enjoys it. They have activities. She is not doing any of them because she's not out of her room, but if she could get out of a room, they have like ice cream socials, bingo, and stuff like that in the common area. It's spacious and it's colorful. There are a lot of trees and a lot of grass. The outside doesn't look like a hospital. The facility looks nice. Value for money seems good.

I was a "resident" in the summer of 2018. It was the worst summer of my life. I was recovering from heart surgery and weighed 162 ponds. I finally escaped on October 4, weighing 112 pounds. After a month, they discovered that I had Gastroparesis. Dr prescribed 4 feedings a day through a peg tube. Originally, the day nurses came in on a regular basis and fed me through the tube. The night nurses did not seem to care whether I was fed or not. Pushing the little button they give you, meant waiting 30-40 minutes and hoping they would respond. Once I returned home, I fed myself and gained all of the weight back. This is a terrible place, and I would not recommend it.

Do not send your family members to this facility. My mother spent just over a month here and was not properly cared for. Do yourself a favor and look elsewhere.

I visited this facility

After a total knee replacement on Jan. 6, I had physical therapy at PC Outpatient Rehab center. My therapists Nicole, Tera and Betty were an amazing team with all having specific specialties. All three were nurturing, demanding and quite professional. The facility has first rate machine equipment, and is very clean. Also, PCRC well stocked with supplementary accessories for balance, stretching and free weights. My program was individualized for me and as a result my progress was very effective. 9 weeks later I am playing golf, riding my bike and back to walking normally. Thank you to all at PC Out-Patient rehab, I could not have had a better experience.

I was here for outpatient physical therapy. The facility was very clean and well-maintained. More importantly, the staff was great...everyone I worked with was professional and friendly..not just to me but to everyone who came in and to each other...I actually looked forward to my visits and give my highest recommendation to this group. My doctor wasn't that sure they did out-patient PT so I would recommend you do a little more outreach to the medical professionals and potential clients. You should be listed as "Best in Charlotte County"...

Don't be fooled by the pretty rooms, high tech gym, and the Bistro. If your loved one needs Skilled Nursing, look elsewhere. This facility does NOT schedule enough nurses and aides. Your loved will often wait more than an hour after pressing call button. One nurse said he was leaving because management spreads the skilled nursing staff too thin. Mistakes made in our presence, glad we were there to stop it ... very scary to think what happened when we weren't there. If your loved one primarily needs just PT, they should be fine.

Beautiful, newly renovated Nursing Home. My father in law was in for rehab and all rooms are private, clean and very nice. He was very happy there and thought the staff was great. He is doing great at home now thanks to the rehab department. Thank you to all the staff at Port Charlotte Rehab!

My mother in-law had hip surgery and was welcomed to the facility. She felt very cared for and attended to. Her rehab helped her heal faster and she said the meals were amazing. It was a nice space where we could still make memories with her for the holidays and I think she was sad when she had to leave. The rooms were very clean and spacious and the staff was professional and polite.

My father-in-law spent 5 weeks here after a severe traumatic brain injury. When he arrived he could not walk, or really perform any other type of daily routine (brushing teeth, combing hair, etc...) He also had difficulties maintaining any type of cohesive conversation. On top of all this, he was an incredibly difficult patient. The staff here really went out of their way to accommodate his erratic behavior, and concentrate on his rehabilitation as well. After his stay there, when he left I would say he was back to 80% of who he was before the accident, and is now almost back to 100%. I would recommend this facility to anyone who needs this type of service.

My mother is in Port Charlotte Rehabilitation Center and I'm not too pleased with them. We haven't had a good experience there. If you push the call button, nobody comes for forever. They've lost half her clothes and things disappear. It's more with the management. She caught something from the urine bag and she's in isolation. She has a urinary tract infection and she is on the IV at night. She got a bed sore that she didn't have before she went there, which is right on her tailbone. Some of the staff members are good. One of the CNAs, Jennifer, is really good to my mother, and an OT named Jill helps out a lot, but I've not been impressed with anybody I've met, especially in the administration. The food is OK, but there are some options she doesn't like. It's a very clean facility, very upscale, and nice looking. They gave me a price list and it's exorbitant.

My Mother is currently a resident there and I cannot say enough how helpful and caring the staff has been to my family and mother. they have all helped making this transition for all us as smooth as could be under the difficult circumstances. The facility is always clean and the staff are always smiling and friendly.. The food is amazing and my mother loves dining in their Bistro. the Rehab facility is top notch and I have personally seen a huge difference in my mothers ability to become much more mobile in her day to day life, I cannot recommend Port Charlotte rehab enough to any family who needs help for a loved one in need of care.

I visited this facility

I would recommend this facility to any family or friends. The facility was exceptionally clean and maintained clean. The staff members (nurses, nurse assistants, physical therapist) are wonderful! My family member knows them all by name and feels extremely comfortable and confident with them. Such a wonderful facility for anyone in need of PT and/or assisted living facility!

I was in the rehabilitation for 3 months. Each person I was associated with from beginning to end provided excellent service. Kimmer O has an excellent team. I would recommend this facility to anyone. The nursing staff was also excellent when I was in the nursing home section.

You never expect your wife to have a stroke-I was beside myself when it happened to me. She was unable to care for herself and I didn't know what I was going to do. When the hospital planner gave me a list and said pick a facility to send your wife to I had no clue! I went to tour 3 facilities and thank God I ended up in Port Charlotte Rehab on the third try - it was amazing to me how the facilities vary from one to the another. Without asking many questions you would never know. When I asked about therapy I found out how different the facilities do it-you wouldn't think there would be a difference, but Pt. Charlotte Rehab actually works in teams and they're all certified in different areas to treat my wife with their Stroke team. There equipment is top notch and specifically made for seniors for the ease of use. With all their encouragement and extremely hard workouts with therapy, my wife was able to do the therapy and walked out of there with a walker which she could not do when she entered. My hats off to This wonderful rehab/nursing for giving my wife back to me!! We are looking forward to going to their outpatient services.

My grandmother was a patient at PC Rehab from July 2016 until she passed in November 2016, and we could not have had better care for her in the last few months of her life. From the CNA's to the nurses everyone was great with her and so patient. The facility itself was always clean and the staff was always friendly. The last few days of my grandmother's time at PC Rehab she was not with it mentally but the staff always made sure she was cared for and had everything she needed when we, as the family, could not be there. The day she died, one of her CNA's that took special care of her sat with he after his shift had ended because he knew she would be gone when he reported back the next day...that is the type if people you find at PC Rehab and I would no hesitate to refer anyone to this facility. I work in the medical profession myself and very picky on these type of things and I was impressed daily.

My husband went through alcohol detox at a local hospital, but had lost so much cognitive and muscular ability that he needed physical rehab in order to learn how to walk again. Not only did he have physical rehab needs, but we were preparing for future alcohol rehab. When I learned that I needed to choose a rehab facility for him, I didn't know where to turn. My first instinct was the wrong one: find a place closest to home and to the hospital, one that promised that he could order takeout food if he didn’t like their menu. Funny what your criteria become when you’re under stress. Luckily, his hospital roommate’s wife and I had unburdened to each other, and she knew more about the local rehab scene than I. She recommended Port Charlotte Rehab, which she said had been recommended to her by many others. I made in-person visits to two facilities, realizing that you never really know. But one thing I understood was that it would be important for him to have smoke breaks. Cutting out one bad habit was going to be hard enough; I didn’t want him undermined by having all his freedoms curtailed. The staff I spoke with at PC Rehab were strong, reasonable, clear about the rules, and it just seemed right. My husband isn’t the easiest of people to please, but he was pleased and impressed with all the physical therapists, cafeteria staff, smoke-break monitors, and nurses. Needless to say, I was on edge, but the staff at PC Rehab, from day one, were upbeat and supportive, and suggested that I be the same if he was going to succeed. There were as few restrictions on patients as possible. He was able to basically spend weekends with me, take as many smoke breaks as he wanted, find foods on the menu that pleased him, even make friends among his smoking buddies. He was there for a month, pushed himself hard to walk farther and stronger than they asked or expected. He was determined to walk again, and not to drink. As it turned out, alcohol rehab wasn’t in the cards, as it wasn’t covered by our insurance, but with his strong start at PC Rehab, he’s still sober eight months later, walking perfectly well, and not only back to his old activities, but 100 percent better than he was before.

There is a shortage of nurses no matter where you go in Florida. This place is no exception. It takes them forever and a day to answer after you press the call button. It's very important for family to visit the patient everyday to keep on top of things.

I am/was a resident of this facility

I was suppose to be there for two weeks to recuperate from surgery but requested discharge after 6 days. There are some great nurses/CNA,'s, other staff, however night shift nurses/CNA's did little to take care of me. I needed assistance to the bathroom and I needed ice changed out on the foot. I did not receive the care on the late night shift and had to constantly set my phone to wake me every two hours. I am 71. This is not what I expected. One night, I received no dinner. My cast had to be redone in the Dr.'s office after being soaked at night when ice bags leaked. Transported to Dr. to change cast. I requested discharge asap. Occupation Therapy did home visit the next day. I got out yesterday. Glad to be home and to get some rest. Unfortunately, NIGHT TIME COMES and you are left to deal with it. It was frightening to fear you were going to fall getting into a wheelchair to get in the bathroom alone because they would not answer call bell. Beautiful yes, nice staff yes, but don't hang around for the late shift. I am happy to be out of there. c

My mother was here last year for Rehab. She could not have received better care and attention. I was able to eat with her in their bistro and found the food delicious. There were many spots for us to visit outside of her private room like the library or one of their many courtyards and lounges. Therapy had my mom up and walking and she was getting stronger everyday from their state of the art therapy equipment. Their therapy pools were impressive. You could not ask for a better place to choose for your loved one to receive nursing and therapy care.

After a lengthy illness and status post open heart surgery, I was abrehensive about having to go to a rehabilitation center. I was happy I chose Port Charlotte Rehab as I received the best care possible from everyone at the facility. They treated me like family and always went the extra mile to make my stay at the facility comfortable.

I was an inpatient for one month. No complaints. Very good experience. OUTPATIENT THERAPY IS A DIFFERENT STORY. The authorization process is rocky. NEEDS ATTENTION. Care itself is exceptional but omething is wrong in approval process between facility and my ins cp FLBLUE HMO.

I visited this facility

The nurses seemed to be busy all the time, but apprarently there are not enough of them. Patients lay in their wet/dirty diapers until the smell is so bad, there is not a chose but to bring it to their attention. The patients are only checked upon when they call the nurses station or if it is medication time or meal time. There are a number of patients that are already able to get around by themselves and are ready to go home. Patients are accually there and are not allowed to leave the facility for a home visit or just a day outting. It seems they are prisioners of this facility. I would not recommend this facility for patient care. They do have a good rehabilitation program, however, but should be used on an out patient basis.
