I am a friend or relative of a current/past resident

My friend stayed at The Pavilion at Piketon. We weren't really pleased with the care. I don't know their financial budget, but it really needed some maintenance. It wasn't very clean. There was easy access getting in and out. It was really nice as far as the entrance to the building, but it was kind of choked down once you entered the doors. There was just kind of a small hallway, and with COVID and all the changes, maybe it was a good thing, because people would check in, but we were never held up during that process. The lower level management staff seemed fairly nice, but I think they have a very high turnover of employees. I don't know if that's the nature of the business, but it seemed like every time we'd go in there, there'd be somebody new. We visited him several times throughout. They take them all to the cafeteria for bingo and bible studies. They have an art thing. There were people speaking. We've never really been a part of that, but we would see groups of people in there on occasion. They have a bulletin board out there with activities and stuff, kind of like a calendar thing. He wasn't impressed with the food. He was not on any kind of special diet or anything, but it was usually cold. It wasn't very appetizing, but I've never had the food personally. The staff seemed pretty nice. The limited times we engaged with them they seemed pretty nice and pretty knowledgeable. The patients weren't very clean, even my friend himself. You'd see patients in the hallway, and they just didn't seem very clean. My friend was bedfast, but he looked pretty rough when we'd go in there. My wife would clean him and shave him and cut his hair and stuff like that. There were some repairs needed. He paid $10,000 a month to stay there. He was a large man. His bed knocked some drywall out and some electrical stuff there, and they put a panel up over it, but it took them almost a year to get that fixed right. The maintenance and just the cleanliness of the place would go a long way. His bathroom didn't even work. We had to go to another bathroom.

I cannot begin to tell all that is wrong with this facility. I cannot believe the state of Ohio allows such low standards of care for human beings. My sister was sent here from Select Specialty in Charleston WV. They had weaned her off the Ventilator but she still had the trach and since she was good otherwise, administration said they HAD to send her to the first accute care facility that had an open bed and took trach patients. We were told we had no say at that point and it could be anywhere in any state. This ended up being The Pavillion in Piketon Oh. My sister was in great shape other than having the trach. She was mobile. She could talk. The PT's were amazed at how good she was. This place was filthy. Her bedside table was decrepit and the drawers were still full of prior patient personal belongings. The closet was also full. Her table that she had to put her food on was broken and nothing would stay on it. The ice chips she was given were from an ice chest in the lounge. Just a bag of ice dumped in and any resident or visitor could dip thier cup into the melted water for ice. My sister, within a few days, began to get weaker and weaker. We repeatedly asked for the Nurse Practitioner to do something. She ordered a chest xray and bloodwork and said both were fine. My sister kept getting worse, became delerious and fell on 2 occasions. The response time to her light was over an hour.. She would need suctioned frequently but was ignored. After a few days we insisted on her being sent to the ER. She had bacterial pnuemonia, congestive heart failure and other life threatening issues. She was put back on a ventilator and sent to Grant Hospital. They saved her life. But she is now in a trauma facility again fighting to get off the ventilator. The Pavillion in Piketon needs to be shut down. Please do not send a loved one there.

Now that she is finally out of there, I can post this without fear of her being mistreated. If you have enemies or anyone you REALLY don’t like, I suggest you send them to Pavilion at Piketon! My mom was there for a little over 2 weeks with absolutely the worst care. I witnessed the nurses giving her medication then telling her it was something different, dirty doesn’t even describe how the facility is and they lack basic morals. I sat in her room last week for 1 1/2 hours while her call light was on. No one came. The air vent next to her bed was full of dirt, dust and mold. She couldn’t put her things in the nightstand size dresser because it was broken and nasty. They never changed her sheets, I had to request them and do it myself. They never gave her a shower, offered her one or even tried to assist. The staff is condescending and rude, 90% of the time. They didn’t even have a pulse oximeter that worked properly, to get her stats. Oh and they have a TV sitting on the sink, which I’m pretty sure is a code violation of some sort. I’ve had 3 meetings with them over their lack of competence in 2 weeks. If you can’t run a facility for people who need cared for, you’re obviously in the wrong business. Anytime I would talk about transferring her, they would either brush it off or treat me as if I’m stupid and don’t know how to conduct the transfer through insurance. Well today, they picked the wrong one. I’m glad I finally got her moved somewhere, where they will actually care for her. Not just let her die on the C-Wing.

I am/was a resident of this facility

I had to be in PHM several years ago for Physical Therapy and treatment of foot ulcer. They have a very caring Nursing Staff in that they p are personable and caring. The Nursing Home was kept very clean and residents and patient areas were very clean. They have a very nice and new area for Physical Therapy and patients were housed very close in location. A plus in my book. They need more staff for evenings and night shift. The only reason I gave them 4 stars instead of 5 is the fact they need more nursing staff in the times I mentioned, my overall experience was very well. I miss the staff that took care of me. :)
