Avoid this place at all costs. My room was dark and furniture was very old. Food was terrible - I didn't eat any of it. Most of the Staff was terrible - had to ask several times before they would help. I got out of here after 2 days. Don't understand how Medicare gives this place 5 stars - strange.

Room was small and very basically furnished. Few lights kept the area dark. Air mattress was inflated for patient use, but left on the floor blocking access to the sink for hours. I spoke to staff because it was a trip hazard for my mom, but it took 15 minutes before they figured out it belonged on the bed and placed it there. Food sometimes was burnt pancakes or cold sandwiches for dinner. We brought food for Mom instead. Would never allow her to stay there again.

My husband was recently transferred to this facility after back surgery. I visited him there yesterday. The facility appears well kept from the front lobby, but the rooms are dirty, the furniture is old and in disrepair. The staff is very unfriendly and acted as though I was bothering them when asked questions. They have COVID + patients but provide no masks for patients or visitors. This place needs to be seriously evaluated. Deserves no stars!

The facility is old and dirty. The food is the worst I have had. There were piles of food that I couldn’t identify or eat. The bed was so old it must have come from the 1050’s or 1960’s. The staff was very unfriendly and were in and out of the room as fast as possible. They would come into the and the lights would be off and they would walk out and leave the lights on and I would have to call them back. This place needs to be replaced

This place does not deserve even one star. The staff is unresponsive and uncaring. The have no concern for dietary restrictions nor will they inform family of any care. Was there around 5pm, no nurses were in site. 3 CNAs were sitting at a desk laughing and having a great time while alarms were going off. They could/would not provide any update on care. The place is unclean and disgusting. Please, do not send your loved ones here as they will not be treated with respect or dignity.

Horrible nursing care horrible food . COULD NOT IMAGINE MY WORST ENEMY GOING HERE. No one ever around took forever to get a ca light answered. The beds were a thin plastic mattress very uncomfortable. Once was lying on my stomach because could not turn over and 2 people walked in and closed the door and walked out offering no help. was not very ambulatory w a 3 place pelvic fracture. Minimal pt why I was there and was kicked out after 5 days to go home and fend for myself. BAD BAD BAD,

My mother is in the rehab facility and I think the care is absolutely horrible. Twice the nurse has had to be told to give her clean bed. They don't appear to have any concern for the patient, nurses roll their eyes at her. The facility in rehab wing gives promising suggestions , but nothing has followed through. I can't get help from anyone that does any good. I call and call for assistance only to get voice mail and never get a call back. Only can I talk to someone when I just walk into the office. Now I have one day that she will be released and I have no clue what to do seeing how we live out of state. I came to make arrangements where I live and now 45 min. before people leave for the day I am being told she is being discharged tomorrow and again I cannot get in touch with anyone

Hell on earth. If you want your loved one to experience neglect and theft, you will find it here. If you want to call 9 times in a day and not be able to get anyone at the nurses station to answer the phone- you will find that here. Grossly Understaffed and consequently inconsistent care. This should be the last option you consider when considering a long term care facility.

Horrible service, staff is rude and seems very uncaring for patients. 'room' was a tiny curtained off area of a shared room, only room for a bed and 1 guest to stand. I know they are overworked/understaffed, but it's very upsetting to see patients standing in the halls literally screaming for help because they're being ignored. My mom was transferred on a Sunday afternoon, given no meds except Tylenol until Monday morning, and a 5 day old sandwich on Monday afternoon was all she got for lunch. Repeated calls for assistance were ignored by her and her roommate, her IV line was full of blood for 2 hours before someone came to help fix it. When asking 1 person for help with her IV, he simply said: "We're serving dinner now", and walked away. Asked nurses, assistants, therapists and case worker multiple times to be transferred back to hospital before request was granted.

Although my mom was only there a short time, I visited her everyday for about 4 hours a day. Her room was clean and organized. The food was good because yes, I tasted everything! The staff was amazing to her and to me. Thank you to all who helped my mom. You are very much appreciated by my entire family

my mom is short memory challenged and a staff member from one staff member was yelling at her to “GET BACK INTO BED!!!” My mother says nothing bad about people as meek as you get but she says the staff is rude and mean. example conversation upon checking my Mom out the ADMINISTRATOR said her staff wont be helping my mother to the car. my father in response said “ dont you care dont you care” the ADMINISTRATOR RESPONDED “ Thats your responsibility.” who talks this way and no tact no sensibility? Terrible experience thank God we got her out if there!

My experience was extremely bad. Both me and roommate requested to see a nurse we finally saw one after 3 hours. I was not given my prescribed meds. I was also not allowed to have my breathing treatments so after a few days my breathing got progressively worse. I told the nurse she called doctor who prescribed anti-anxiety meds I have Copd not anxiety. So I called 911 myself got my treatments and feel better. I then was discharged ama altho i refused to sign ama papers. They are veryyyyy understaffed .I wouldnt send an enemy here.

My father goes on respite 3 times a year to Coronado. The staff is friendly, and makes him laugh through the days. He enjoys his time and considers some of the staff as family. He said he liked trying all the different foods, since Mom lies the same stuff. Mom always knows he is cared for when she has to be away.

Very dissatisfied. Coronado does not communicate what has been going on with my grandma's care. It's impossible to try and talk to your loved one. We get a hold of a nurse but then they hang up. They have our number to call back, but they don't. Very suspicious activity. We haven't heard or been able to get in contact with our loved one there for five days. Once we did, we learned that she took a fall and now has pneumonia. We've called the director and have gotten no response or answers as to why we were not informed on any of this. Highly dissatisfied and disgusted.

The dietary manager promised, but has never visited my room. Our menus were never given to the dining room. As a diabetic, a lot of the foods cause my sugar to spike. It is common that I get my meds 3 hours late, which puts my pain off, which means I hurt more after physical therapy. Rooms are basic and bland. My room is on the area where the employees smoke, which bothers me as a non-smoker. (It comes through the ac.) Male patients are allowed to come into female rooms constantly (I have a male dementia patient coming in constantly). It is common that our call light doesn't get recognized for 45 minutes or more. It is hard to get the CNA's to pay attention to us.

I was in this facility visiting my girlfriend after her accident. The staff was responsive, caring and very nice. They truly have gone and beyond. Therapy has been wonderful. We are grateful to everyone at this facility.

Most of the staff is knowledgeable, courteous, and competent. From a patient's point of view, they do a great job, the food is excellent for a institutional environment and there are plenty of avenues to keep from feeling bored, which can lead to being depressed. I would recommend it to any of my relatives looking for a place to recover. The nursing staff is there for the patients when they find any new maladies or symptoms. The physical therapy department is almost too diligent in keeping people healing at a faster pace physically. They push a little too hard at times, but they are good at keeping patients moving within a time frame and not above one's strength level. I have had physical therapists that push beyond a patient's limits and that leads to doing well in physical therapy but not being capable of functioning for the rest of the day or longer. If you are looking for a place to keep yourself, or someone you know, active and continually under observation for any problems, this is a good one.

I visited this facility

My father needed IV antibiotics that he couldn't receive at home. The people were friendly and helpful through the process. The Dietary manager personally came to see him, because his appetite was low. He found ways to get him to eat almost everything on his plate. The Director of nursing was always smiling and would go out of her way o make sure you were doing well. I highly recommend this skilled nursing facility.

This place should not be allowed to operate. My friend is there right now two days after a hip replacement. His room stinks because the roommate smokes, there is NO walker available so he can't get out of bed to use the bathroom (he asked 3times), and now he has been pushing the call button for 15 minutes with NO response! It is Thanksgiving but he is going to try and get moved tonight.

I am the step-mother of a patient of ths facility. She was diagnosed with ALS on the 1st of October. This place is a Joke. The communication between Doctors, Nurses, CNA, therapist and case mangers is zero. One person says something the next person comes in tells you different. She was sent here with a feeding tube because she couldn't swallow. The only good was that they taught her technics on how she could swallow food. CNA tells her she can Eat case manger says NO she can't nurse tells her tube will be remove another tells he NO. Bedding was asked to be change due to blood on sheets due to them poking her 5'x for IV. Called in specialist for 6 poke. CNA took off bedding and never came back with clean sheets I went into the hallway & took them off a cart. Called to complain again today to case manager because now they say she is refusing food from tube when they said No! And to tell them about that she was scared because a patient had passed away a couple rooms down from her. She was told that they would tell her 4 days in advance when she would be leaving. Not even an hour after I called the case manager went into her room to tell her she is leaving tomorrow! These people need to learn how to communicate with each other. If you are looking into a Cheap Hotel room this is the place to Go! The only reason I gave them a 1 Star is because of the therapist that Help her.

My mother has been at this facility for a few weeks after being in a car accident. My brother and I were quite concerned with placing her in a nursing facility, however the Doctor from St. Joes hospital assured us he would follow her at the care center.. The rehab team there has been wonderful. They have worked with my mom and all her little quirks. She is even better and stronger then when she had the accident. The nursing staff was friendly and helpful. The director of nursing took time out to answer questions and help with adjusting to the facility. The food was good, not a lot of choices. They did try to accommodate my moms picky palette. Overall, the experience was very good. I would take my mom back if she ever needed rehab again.

My relative has been at Coronado for almost 2 weeks. He has been using the same water carafe and neither it nor the plastic straw in it has been cleaned. His sheets have not been changed. He does not get his pain meds on time, and occasionally has not received them at all. He emptied his own wastebasket and today emptied his own plastic urinal because it had been full for over 12 hours. He has not seen his doc (facility doc) in almost a week. The staff folks are nice so I don't know if that Coronado is understaffed, or what, The facility itself is clean (-ish) but old and in need of maintenance - the paint is peeling off the walls in my relative's room. There is a foul odor that's quite pervasive. While the menus are wholesome and described in a most appealing manner, the food itself is unappetizing (sloppy joe with cold, hard French fries, for example). I wish my relative had been given a choice as to what facility he would go to upon release from the hospital, as the quality of care he has received is poor.

The level of care in this place doesnt even register. When u have incompitant case managers and lazy social workers, Coronado is what u end up with. My moms nurse discharged her with perscriptipns we knew nothing about.

DO NOT use this facility for any reason. Please find another location by any means. I tried calling the facility and there is no answer and there is no answering machine. My family member is still waiting to meet with their social worker, meanwhile urgent issues are no being addressed.

My mother is a patient at Coronado, she has received the best care! I could not be happier. The staff is friendly, her doctors is amazing and speaks with me whenever I have a question. I read through the other reviews. The wound team is there 7 days a week and sees her. she has a wound from before her admission and she has MRSA.I was concerned with the other person stating the staff did not know how to handle a person with MRSA so i called around to other facilities and found out this is a very common thing and using gloves and hand washing is all t that is needed. I also went looking for gloves on my next visit and YES I found them. I went into the building looking for hand sanitizer dispensors there was one at the front enterence and on both the nurses stations I went to. Her therapy has been top notch!!! My experince has been more than wonderful. Thank you Coronado!

This facility is apparently mainly a nursing home. However, they equally advertised themselves as a physical rehabilitation facility. The rehabilitation room is small and sparse. My husband entered with numerous MRSA wounds, which included his amputation wound. It took them 3 days to clean and bandage a severe wound with a 1 week old skin graft (due to MRSA). I had to threaten them with legal action to get that done. I could not find a hand sanitizer dispenser anywhere. No one took proper precautions for MRSA. It was very difficult to speak with a doctor, a nurse practitioner, or even an RN because there are very few of them to spread around. They always seemed to be on-call. The bottom line is, it MAY be fine as a nursing home, but as a rehab facility for an amputee that needs supervised medication (in our case an IV) and serious physical rehabilitation work, this place fails miserably. I would never recommend Coronado Healthcare to my worst enemy.

I visited this facility

My grandma had a knee replacement surgery a few months ago and she stayed here for a short time. The facility is modern and has a very warm yet rustic feel to it. I sampled some of the food and it was extremely good. The overall care was superb it was a great experience. My grandma literally had a service button for whenever she wanted something. The facility had full size gym with physical therapy rooms for recovery. The rooms themselves were amazing, My grandma had a flat screen television, her own bathroom, and a small balcony for if she wanted to read outside. The overall care was great and my grandma extremely enjoyed her stay here. I have recommended this facility many times to other elders in need of care. They are sure to enjoy it here because the staff is what really makes this place. My grandma had two nurses specifically for her whenever she wanted and she enjoyed that and it made her feel more at ease. It is a great place and I highly recommend it to anyone looking for a great nursing home.

I visited this facility

This is a 184 bed assisted, skilled,and memory care facility. They have two care units in the building with 25 skilled beds and medicare and a long term care unit, They also accept respite and rehab residents. The building is a one story red brick building with trees, flowers and grass. It is well maintained and clean. There is a freshness in the air inside which I noticed on entering. They have two nice courtyards with grass and flowers and lots of places to sit. They have a nice dining room but there is a set menu. They serve three meals and snacks. They have a beautician. The rooms are mostly semi-privte with a bathroom. They are handicapped equipped with a call light system. It is a typical nursing home setting but very clean. They have a house doctor, DON, ADON,registered and LPNs on duty, All caregivers are CNAs.They have physical, occupational, and speech therapy. They have transport to doctors and appointments.The staff seems friendly to me. They do lots of activities as exercises, bingo, arts and crafts, entertainment, and parties.They go out shopping and other events. This seems like a nice facility from what I saw and residents get good care.

I visited this facility

This 150-bed facility, which offers skilled nursing, rehab and respite care, is in a two-story building located in a high crime area. There are some security guards, but they don't seem adequate. Visitors pull up into a parking lot, which is not aesthetically pleasing. There is a hospital nearby, but not many other conveniences. And there is a little bit of grass on one side of the building, but no patio or outside sitting spaces for residents and visitors to enjoy. Inside, there is a compact, cramped feel. The décor is cold"”mostly tiled, without a homey feel. There is a reception area when you walk in, but visitors get no friendly greeting. The rooms are mostly shared"”and they're very small and in close quarters. There are a few activities and movie nights with popcorn, some arts and crafts, and programs for those in need of physical therapy. But no religious services are provided. And residents complain there's not enough to do to help pass the time. The food is not very appetizing; residents frequently complain it is cold. There are pureed and easy to digest choices, though. There have been problems with temperature control in the rooms; many residents complain about it. The solution seems to be to push those who are too warm into the hallways. There are LPNs and CNAs and an RN on staff, but the care is not very good. Residents are not given adequate attention"”some left without water throughout the night. There was a resident who fell and was injured"”and family was not notified until several days later. It is horrendously understaffed, especially on the night shift"”and there is a very high staff turnover. There is simply too much work for the staff on hand. There is a lot of complaining by the residents, who are generally younger and more alert than at most care facilities"”most of them in wheelchairs, with brain injuries, alcohol dementia, and strokes. The complaining also makes the whole place feel unpleasant. Mostly, it feels like a sad place. The cost is high for the area"”about $5,000 per month for a semi-private room.
