I'm very happy with Falcon Ridge Rehabilitation. My friend has COVID right now, but they've kept me informed of everything. The nurses seem nice and the place is clean. I'm very happy with it. They've done an outstanding job for my friend's condition. The upkeep is very clean and very organized. The yard looks good. It's very open. I like that. It kind of gives you a warm feeling that you're not in a nursing home, but you're in a home. The services are very good and very nice. Even on their days off, I text them and they've answered. So, they are wonderful. They have something every day for the residents and they keep them active. They had a Halloween party. He is not in a condition to do a lot of activities, but I see that it looks like every day, they have three or four different things that they can all participate in. The food looks good, too. It's OK.

Do not use this facility for rehab. As I see others have had the same bad experience as we did. My husband was over medicated, went in on Tuesday and by Thurs could not stay wake up, could not feed himself, lost his voice, and had not received any rehabilitation, They simply had him over medicated and laying in a bed all this time. He was so medicated aids could not lift him to get him to the bathroom and when they would get there in time ( which was not always the case) they used a urinal which caused a cut and blisters on him. Had to have him moved to a hospital doctors said he was over medicated It took a week to get him back to normal. Also not enough staff to take care of amount of patients.

This place is terrible! Do not place your loved one here! They have given medication that was not prescribed, to my mom. They have treated her with disrespect and refused her basic items such as a pillow. The topping in the cake was, they took her to an appointment outside the facility that was not even scheduled and left her there all day! When asked where she was, the staff had no idea where she was. They didn't even know she wasn't in the facility! Communication is not-exsistant. The phone system doesn't function and you can not reach them by phone. I left a voicemail for the admin and he never returned my call. Now I'm unable to even reach a voicemail. The phone rings and then disconnects. Overall, a wretched, inhumane hole in the wall!

My grandmother was placed in this facility for approx. 5 months until she passed away. I have not experienced a facility that was clean, attentive, and courteous as Falcon Ridge Rehabilitation. The staff answered all questions the family developed and they put her health first. Thank You for your service and I will not hesitate on recommending this facility to others.

I have a loved one who is a resident at Falcon Ridge. My loved one has been there for over a year. The original management and staff displayed a level of care and concern that would make any family satisfied and happy.. I knew I had found the right place for long term care. However, since most of the original management staff has departed, the level of care has been greatly diminished.. CNA's show disrespect and a lack of care for the residents and at times my loved one's needs are not being taken care of. There has been a constant turnover of care staff and their morale is low. There are some management who talk down to the care staff in front of the residents and family members, and there always seems to not be enough care staff to take care of the residents' needs. In my opinion, management seems to have the attitude of trying to be more in compliance with State Regulation than meeting the needs of the residents. I understand compliance, but caring for their residents is why they are there. There is staff that are genuinely doing a good job and try to smile through being short-staffed and a lack of support from management. Something has to change or I will be looking to find better living and care arrangements for my loved one.

I cannot express how outstanding your complete staff is, I want to especially recommend the physical therapy staff I have had treatments in other facilities and none can compare with the group you have in your facility. Rehabilitation
We offer physical, occupational and speech therapies.
The clinical team at Falcon Ridge Rehabilitation will evaluate each residents unique circumstance and develop a comprehensive rehabilitation program. During daily treatments, our therapists will work toward improving independence, strength and confidence.

- Stroke, Parkinson’s Disease and other neurological disorders
- Respiratory disorders

The overall goal of the Rehabilitation Department at Falcon Ridge Rehabilitation is to strengthen bodies and reinstate a level of independence for each individual. Under the guidance of the Director of Rehabilitation Services, the therapy team conducts a thorough evaluation and develops an individualized therapeutic program designed to help the individual restore function though actual theraputic activities using our ADL kitchen, mobility garden, and transitional room.
Pulmonary Care Unit
Falcon Ridge Rehabilitation is pleased to offer the following services.
- Pulmonary rehabilitation
- IV and respiratory certified nurses
- Tracheostomy care
- Trach weaning, decannulation
- Providing oxygen up to 10 liters
- BiPap/C-Pap; suctioning every 2 Hrs
- Cable TV and telephones
- In-room dining
- Fully equipped rehabilitation gym
Wound Care Program
We specialize in treatment of decubitus ulcers, post surgical wound care, wound VAC, specialty beds and ostomy care.
Physical Therapy
Our team of licensed physical therapists can assist you with regaining your strength and mobility. They will develop an individual plan for you that takes into account your tolerance of activity while at the same time striving to meet your goals. Our team is experienced with rehabilitating everything from hip repair procedures to knee replacements to recovery from acute illness.
Occupational Therapy
The goal of occupational therapy is to help you be as independent as you possibly can. Occupational therapy helps in the rehab process by analyzing one's activities of daily living and helping them adapt or regain their functional mobility through activity and or the use of adaptive equipment.
Speech Therapy
Speech therapy provides diagnosis and treatment for a wide variety of communication disorders such as articulation, comprehension and expression of language, as well as chewing and swallowing difficulties. The speech therapist will also evaluate your meal textures and consistencies and make recommendations accordingly.
