I am a friend or relative of a current/past resident

The rooms are old but very clean. I believe that the care given is more important than how the physical building looks. T he nurses and aids are very nice to my Dad, they are kind and caring. Everyone that I have worked with social worker, activity director, occupational, and physical therapists have been kind and are working to make my Dad’s stay as perfect as they possibly can. I am very glad to have the peace of mind that my Dad is being cared for at this nursing care center.

Let's just start with my mom, lived alone at 97 and even drove a car til 90 , she is very aware of everything and has no sign of dementia. For ten years she was on home medication hydrocodone five a day for pain and you could never tell. She only had someone come in to do laundry and deep cleaning. Well the tape came off the rug and she took a fall and fractured her hip. The surgeon said she was going to do fine because of her health was better than some 40 year olds he saw. After the surgery they sent her to Oakhill for rehab. That was a month ago. Since then she has been put in a diaper and left in it until she calls me crying, as for physical therapy she is always in a dirty diaper so she can't go until they clean her up and misses days because of this. She has glaucoma and needs eyedrops at night to prevent pressure and blood build up and in a month has gotten it only a few times after countless times from me calling to complain. Pain meds are far and few in between so I had a palliative care consult and they asked for her to be put on what she was on before the fall and they refused. My 97 year old mom is going through opiate withdrawal and the only way to get her meds is to beg and then she will get an oxy that turns her into a zombie. She has had two showers in a month and she has long toenails and her fingers look like she has been digging in dirt. She has all her teeth and they grind her food up so my brother has to bring in real food because she won't eat it. I have exhausted myself and my family by calling daily and spoken to almost every nurse and even the nursing director who was supposed to call me Friday and promised by the end of day. Well it's Monday night. I just wanted the best care so my mom could go home but with the lack of therapy she is weak and now I doubt that will ever happen. They are failing her in big ways and I just get bounced around. I even asked to have the doctor call me last week and still sit and wait. I'm trying so hard to get her into another home because she is now to much for me to even take care of because I'm not young either. She has lost over 10 pounds and that's a lot when you only weigh 98 to begin with and now they stopped physical therapy and leave her in bed all day. Don't send your parents to this place unless you want them to be punished to death. I even sent a letter to someone from this place by someone who left a bad review and was told to contact them so I copied the email and sent it off and no reply, that was two weeks ago. I'm at the point of calling the state and reporting them. I've kept names and times and saved emails and recorded meetings and I'm furious over how they can get away with treating the elderly like this. This is a long review but I'm just trying to save you from what my family is going though .

I've witnessed when a patient needs help getting off a low seated chair...the staff disappears.... letting the patient mess their pants then saying their incontinent.

I am a friend or relative of a current/past resident

This is the worst rehab center I have ever seen. It is the most unsanitary and disorganized facility, even the "nurse's" station was strewn with random papers piled all over every surface. It has a corporate culture, not caring for the needs of individuals. They had no documentation of any of our loved one's medical issues, no records came with him and we weren't notified that he had been brought there from Rhode Island Hospital. He suffers from cancer and neither RI Hospital nor Oak Hill provided him with his cancer medications for the 12 days he was there. Nor did they speak to his family or myself, his health care proxy. They did not ask about his history or his doctors. Instead they made appointments with totally different doctors, and when given his true medical team, they refused to communicate with them. He was brought to Oak Hill for physical therapy due to a fall. It was my understanding that he arrived on November 16 and did not receive any physical therapy until the 20th. On the 19th, I requested that he be released or brought to another facility and the director of social work, Alison, continued to ignore the request for five days until we were forced to remove him against medical advice. We are now moving g forward with competent healthcare. Do not under any circumstances let any of your loved ones be involved with this facility.

I am currently at Oak Hill Health and Rehabilitation Center. I would say the staff is helpful. The room I am in is semi-private with a shared bathroom.
