I visited a couple of rehab facilities before having surgery in June. When I entered Stonegate, I quit looking. It is lovely. Newer, clean, beautifully decorated. Sadly I found out that is not the most important thing to look for in a facility such as this. I was simply left alone for most of every day, thinking I could just as well be home. When they finally got around to bringing in the physical therapists, I was anxious. But my therapy, each day, amounted to 2 therapists helping me to sit up in bed, lowering the bed to just above wheelchair height, placing a board between them and sliding me into the chair. Then I was left alone again and told to sit up as long as I could and call them when I wanted back in bed. That was it. I was told they were short staffed. Some nurses and CNAs were very pleasant and helpful. After almost 2 weeks, one finally helped me to the shower! Again, I was told they were short staffed. The food reminded me of school lunches 60 years ago! Probably due to the shortage of kitchen staff. One day I just didn't feel right. The doctor was brought in. I'm sure I was very anxious by then, and I said I thought I should go to the hospital. I was not in pain, but I knew something was off. I did my best to explain how I felt, but was told I was just overly anxious and asked if I had a history of anxiety. Feeling chastised, I stayed. The next day I was helped to the wheelchair again and left alone. I was feeling worse by the minute. A nurse walked in and asked why I was sweating so profusely. I said I felt awful. She got another nurse who exclaimed that she could see sweating pouring out of my pores, and ran to get the doctor. I started dry heaving violently. Someone, I think it was the doctor, asked if I had just had a shower! I'm not sure why, was I that wet? A nurse was holding a tray under my chin in case I actually threw up. Other than that, no one was actually doing anything - not taking vitals, nothing. I finally screamed for them to get me to a hospital. Instead of calling 911, they called the service they used to take patients to appointments. They were slow. I was taken to Parker Adventist emergency. After scads of tests and scans, I was told I had a saddle pulmonary embolism and that I was lucky to be alive. I told them what had happened, and they said it was good that I finally paid attention to my instincts, that I had had what they called an "Impending sense of doom". When I was sweating and heaving, is probably the point at which the embolism was passing through my heart. I had damage to the right ventricle. I spent the next couple of days in Intensive Care where they found another blood clot. The cause? Probably being in bed for the past 3 weeks, the last 2 at Stonegate. Avoid this place if you can.

The Stonegate facility was very clean. My mother received better care from some of the nurses and CNAs more so than others. Thank you to Hailey for your genuine care for my mother. You were always available when needed and gave my Mom her medicine on time. Hailey, you treated my mom with dignity, respect and care. Thank you to Leona and Michelle, you were the best CNA's! Thanks for attention to my mom and your desire to make her comfortable! Paige, you were great with all of my questions! Thanks for your patience and all of the information that you provided to me. MaryBeth, thank you for understanding how much I wanted to be with my Mom. I appreciate your compassion.

My dad is 92 and had to worse experience of his life at this facility. The staff does not watch over the patients very well. My dad fell and broke his hip. It was the cleaning staff that found him. They did not get the break diagnosed for 7 days and then the Ortho said we got him in too late. If a patient can't advocate for themselves, don't leave them here. My dad is now wheelchair bound for the rest of his life.

The building is newer and spacious, but inside the operation and management leave a lot to be desired. I will say that in my opinion most of the problems that I observed regarding Stonegate come back to failures of Management. There is a major problem with the Therapy group within Stonegate, this group; both Physical and Occupational operate as a kingdom within themselves. They make decisions and ignore client/resident requests and even direction and orders from Doctors. In the case of my father who was a resident, multiple doctors submitted orders requesting additional therapy to improve his level of life. Orders were given asking for additional therapy to increase blood flow and activity in the lower legs and feet. The Therapy group ignored all orders. They stated my father had plateaued and no additional therapy would be useful or provided. We appealed the decision to take him off rehab and move him to long term assisted living, we won 2 rounds of appeals. Yet once the Rehab group made their decision that he had plateaued, no additional therapy was provided, even though we had won the appeal and he should have received at least 1 week extra of both PT and OT Example, we asked for a recliner chair to be put in my father’s room, so that he had an option for sitting as opposed to either the wheelchair or the bed, we were told that the Rehab group disallowed it. It was not a matter of money, we offered to private pay; it was the Therapy group just did not want to do anything. The LifeCare Center of Stonegate has a reputation with the local hospitals that they are a good Therapy group, but I think they are good for the Insurers, because they get people off of rehab (which is paid for by the Insurers) as quickly as possible. We raised concerns on this with the Executive Director and he ‘investigated’ and came back with the exact same message. I one time asked him ‘who ran Stonegate’ and he stated that he did. Obviously that is not true, the Therapy team runs the organization, because they can do whatever they want. There is persistent understaffing in the CNAs in the various wings. In addition, because of the persistent understaffing they often have 2 CNAs when they are supposed to have 3 in an area. This means that the Long Term Care residents have to wait longer for attention. It also affects the quality of the staff on duty at any one time. The full time permanent staff; Nurses (RN and LPN) and CNAs are mostly good and caring. Some of the CNAs are not trained enough and again this can result in a lower quality of care for the residents. Recently Stonegate has been cited by the State of Colorado for 2 grievances related to inadequate care. There is also a lack of teamwork within the resident care group, the LPNs and RNs on duty do not help the CNAs in daily care activates for the residents. All they do is give out medicine. Additionally to combat the understaffing, Stonegate will bring in temps or Traveler (longer term temps) and you can get a very mixed level of capability. Minor but important issues are failures at ancillary areas; the Kitchen in general. The Kitchen Manager who also cross fills to cover the front desk will bring her dog into the building. Service dogs and other animals are allowed in the facility, however we observed on 2 occasions the manager bringing her dog into the kitchen, which is absolutely against all health and safety rules. The overlay quality of the food is terrible and the staff responsible for serving the meals was underwhelming.

My husband was in rehab for 10 days in December 2019. I also spent that same time with him. From the front desk to all of the staff, we were extremely appreciative of their friendly attitudes and level of care. Menu choices were given and also alternatives for the 3 daily meals. The thorough PT and OT evaluations were amazing. So was their daily interactions with him.

Content: This was the worst experience of my life. My mother was admitted to this facility on a Monday. The facility appears wonderful; but looks are deceiving. My mother's medications were never given to her on time or even within an hour of the time she was supposed to receive them. She would place her "call light" on, but no one would respond. Upon visiting her, I found out she had not had a bowel movement in over 9 days and none of the nursing staff seemed to be aware or had checked! She was not given a shower for 11 days and only when I contacted the Director of Nursing did she finally receive a shower. Nurses and Assistants say they are understaffed but administration refutes these claims. If they are not understaffed, then they are the most incompetent bunch of nurses I have ever encountered. The Physical Therapists and Occupational Therapists were wonderful which is the only reason I give this facility 1 star. DO NOT LEAVE YOUR LOVED ONE HERE!

This facility exceeded my expectations. My husband was at the place for just over 3 weeks. I met some of the most caring people, nurses, nurses aides, therapists. Therapy did a great job and pretty much wiped him out each day, it was so wonderful for his recovery. I didn't experience any issues with his medications and I'm usually pretty on top of what he needs. My concern was him being ready to discharge back to our house in Lone Tree. When the time came he was ready to go home. He said he'll miss being at this place but he's so thankful to be back home.
