My mom stayed at Palos Heights Rehabilitation last year. My mom had to share a room. It's clean, and the food is pretty good. It's pretty decent. The nurses were so nice, and they worked so hard. They were on their feet all day, and they took good care of my mother.

Palos Heights Rehabilitation was a nice and well-kept building, but there was no communication from the staff. None. Like they sent my husband to the hospital and didn't even tell me. They were always understaffed, and the food wasn't good. Although the nursing staff was particularly good, it wasn't worth the money they wanted. The service that my husband was getting wasn't worth it. His room was dark and dank. It was a nice facility, but it was not lively. Maintenance was OK.

Mom was only in this facility for one week. She came for rehab but did not receive therapy. She declined a lot in that one week. The facility is old, dirty, and much is broken (beds and tv). Food was gross. After one week we moved her to a different facility. She is thriving now. Had we left her in Palos Rehab, we shudder to think what would have happened.

Dirty conditions all over- floors have stains and dirt build up, the room walls are damaged, the particular room had ants on the food tray and walking on the walls. The IV pole base and food tray base are filthy. If you can avoid this place, do so please. The staff may try, but some have a bad attitude and not very helpful. Very sad to see this- should not be allowed to operate this way. Illinois Public Health needs to be alerted!

My mother is at Palos Heights Rehabilitation Center. She is in rehab. There's plastic up all over the place. I believe it's an attempt to stop COVID. You have to fill out a questionnaire on the computer every single time, and that takes more time than is really necessary. You walk in and there's someone there to greet you at the door, but after that you may not see another staff member at all. The facility itself is pretty much the same as it was 15 years ago when my mother was there. Nothing has changed. It's the same equipment. They do have new paint. They went from bright yellows and blues and pinks down to brown. There's nowhere near enough staff, and the staff is so overworked that they say they'll be back, but they don't come back. I wouldn't want to try the food. There's maybe one activity a day on the far side of the building from the rehab portion. The room is the most boring room ever. There are no curtains. There's one bed per room. It is dark, dingy, and not always clean. I will never ever send anybody I know there again. My mother has been there but has not been allowed out of her room yet.

Do not consider this facility for yourself or loved one. My family members health declined drastically in the short time there. Albumin levels in the toilet 20 lb weight loss in two months dehydration and multiple emergancy room visits for falls and infection . Please do your internet research check IDPH website and CMS websites. Call the Ombudsman for this facility and for heavens sake find a better place. Rodents , filth, rotten food, insects, residents and patients left in filth. Horrible place it deserves zero stars, it actually should be shut down. The building is so old it’s a hazard, there is no way possible they’re up to fire code and they’ve been cited multiple times.

When visits are not allowed due to COVID, one would hope that staff would be more understanding about providing updates to family members. Don’t expect that here. YOU have to make the call and when you do, staff rush you off the phone, don’t review the chart, say they aren’t sure what progress has been made, and discourage you from calling for updates - “we will call you if anything drastic changes”. Reports from AM and PM nurses differed day to day - AM he is on 2-3 L of O2 and PM states only the use of inhaler. When family expressed concerns to staff, they reported they did not have a baseline to go off of and didn’t even know how he ended up there. My loved one uses his call button, but often times has to yell out for assistance. When that also fails to get the attention of a staff member, he has had to call family to ask them to call a nurse for him. After calling a nurse, I have then called my loved one to ensure that the nurse followed through. My loved one now repeatedly cries out saying that he wants to return home and feels that he is only getting worse. He requests to go to a rehab facility and when reminded that he is currently at one, he states, “Some rehab facility...all I do is lay in bed and watch TV. For therapy they sit me in a chair for a few hours and leave me be until they get me back into bed again. It’s a joke.” Every time we talk, he now says he would be better off being “six feet under” than at Palos Rehab in Crestwood. Extremely disappointed with this experience.

Some of the people that pass out the medication need to be trained better. When I asked about meds I should be getting, I was told they weren’t on the list. They were the day before and the day after. Either they were out of the drug or missed it on chart. I also watched one girl roll the pills in her hand before she dropped them in the dispenser cup. So glad those were not my pills. Food was good. Activities were fun. Some of the staff went above and beyond.

Rooms are small & old. Nursing staff poor, left my husband in the dining room by himself. He was experiencing flu like symptoms & his nurse had no clue. Fortunately I visited early that day & requested for him to see the house doctor. He would had sat there all day being sick.

I visited this facility

Do Not take friends/family to this place. I'm assisting a Resident here, and service is LOUSY. Resident was having an issue and signaled for ths nurse at 8pm. 3 Nurses Aids came to see she wanted...here it is, 9:05, and the Nurse Still has not come. One Resident waits untill late night/early morning to play TV loud. Another hollers, a good portion of the time. The Dietary Aide served food in her street clothes. The Nurse, finally, showed up...and placed a gauze on wound that touched her pants and was insulted that I asked her to take away this unsanitized one. Not all the workers are unfit, there were a few GOOD ones..God bless them for tolerating this environment!!

I visited this facility

Very poor quality care for patients . Rooms are small ,among nursing staff no communication. I have nurse Dan told that no body knew any situation going with patient. My uncle soaked wet in Urine ,it looks more than one days,talked the DON,Debbie said,we take care of it,but who knows.when ask any questions,they gets upset ,your loved ones has to suffer. Horrible place to be. Speech therapy poor quality. No STARS

The nurses were very nice. My helper got my prescription for me and dropped it off at Walgreens. The nurses were great, but the food was absolutely terrible. They have movies but nobody sees any, and they had a patio to smoke on, but that went down the tubes, so I'm glad I'm out of there. The staff was great. The place was very clean.

I visited this facility

We had my aunt here. I would highly rate them as they are very good. I didn't have any issues or problems with them at all. The staff is very friendly and helpful. The only thing I would say is that they need to have someone answer their phone at the nurses' station. Their dining area is very clean, and the food looked appetizing.

It looks very nice from the outside as well as inside. My brother's room is spacious and very clean. The closet space and the bathroom is clean. I like the dining room area.

Pros : Excellent rehab. If you have been injured and need to have physical and/or occupational therapy this is the place to be. My mother received much better rehab here than in a similar place in another state after a fall. the food - plenty of selections and portions. My mother loved the dessert cart especially. nursing/medical care - very good medical staff. Much more responsive to problems than was experienced at the other facility. The other facility let mom leave with a deep bed sore; here they healed her completely before release. Cons: older building, rooms could really use some modernization. not enough CNA's. There were times when the wait for help to the bathroom was just too long. One night mom waited an hour for assistance. This place is fine for short term stay; would not let mom live here on a long term basis however.
