I am/was a resident of this facility

Absolutely HORRIFIC experience! In early May, I broke my femur in two places and had a rod put in. A few days later, before the hospital discharged me to Bellevue, my patient advocate at the hospital spoke with the female administrator (I'm withholding the name intentionally) about my situation. I am a multiple assault survivor, with PTSD as a result, and being in a facility with male employees, especially when I had no way of defending myself with a broken leg, was very triggering. The administrator assured my advocate that my needs could be met and that no male employees would be allowed in my room. This conversation took place several hours BEFORE I was transferred to Bellevue. I had been at Bellevue for less than an hour before my first encounter with a male employee. The PT team had left me in the bathroom, and I told them I would use the call button for assistance once I was done. When I did, a male employee entered my room. I called out from the restroom that there weren't to be any men in my room. That this had been arranged with the administrator in advance. He said there was no one else available. I reiterated what had been agreed upon before I arrived and that I needed him to go and find someone. He said there wasn't anyone. I told him he could not come in. He didn't get anyone and I was left sitting on the toilet. After 15 minutes (thinking he'd gone to get someone), I did what I had no choice but to do. I unsteadily made my way out using my walker. By this time, my pain meds were wearing off and I needed more. I had been at Bellevue multiple hours by this point. It took a couple of more hours for Bellevue to figure out there had been a mistake, get it figured out, and get me pain meds. (A femur break ranks up there with the most painful things a person can experience). During this time of waiting for my medication, who should come back into my room... the same male employee I had already told that men weren't allowed in my room! By this point, I was an emotional and physical wreck, and told my spouse that he had to get me out of there. Texts regarding the situation went back and forth between me and the original administrator who had assured us that meeting my need of not having male employees in my room wouldn't be a problem. My spouse was going back and forth with the head nurse regarding why I hadn't received any pain meds. Given the nature of my injuries, my spouse was concerned that he couldn't manage my needs, and I agreed to stay overnight and the administrator said there would be a note on my door stating that no one was allowed to enter my room without approval from the nurse in charge. I was so far into trauma response at the time, that I couldn't sleep, despite finally having heavy narcotics in my system. While the first shift during the night adhered to the sign on my door, by morning, there was an employee who had come into my room twice before noticing the sign. By morning, I had been up over 24 hours and was sobbing when my spouse called to check on me. He was FURIOUS that things had continued to go sideways and rushed over to check me out immediately. The gentleman working that morning got belligerent with my spouse saying that I couldn't check out. That if I did, my order for physical therapy would be considered completed and that I wouldn't be able to get PT anywhere else. More phone calls happened, and eventually he got me released. It's taken me seven months to be able to leave this review. It took several sessions with a trauma therapist to work through what happened at Bellevue. And yet, months after leaving Bellevue, they had the gall to call my spouse and demand payment for something. He told them he was more than happy to pay it once Bellevue reimbursed us for the trauma therapist. Bellevue immediately waived the charges. To this day, I've kept the administrator's texts about the entire situation on my phone as I still can't believe their incredible negligence. Don't promise what you can't deliver. People's lives are at stake.

I am a friend or relative of a current/past resident

My wife was a patient in the Rehab section of Bellevue for five weeks after surgery. The owners and employees are very professional and patient oriented. They arranged transportation from the hospital to Bellevue. We were briefed on the day we arrived regarding costs, amenities, food, rehab procedures etc. My wife's room was very clean and cleaned every day, food was excellent with many choices for every meal. Laundry was done daily, no shortage of staff and supplies, very good covid protocols. The place is family run and there are employees that have been there for over forty years and their offspring also work there. My wife was non weight bearing for four weeks, so she required help for everything and the staff were right there to help. I recommend this facility to everyone.

I am/was a resident of this facility

I have been a resident of this place for 15 months now. I am younger than most the other residents and the activities available are wholly geared for older diminished capacity residents, leaving me with absolutely nothing to do everyday except stare out a window. This also leaves me with nearly no socialization with other residents. The "food" here does not merit the name. It is all precooked, processed, frozen, canned or otherwise requiring no actual cooking skills on the part of the kitchen staff. This is just as well as said staff possesses no such skills. Even with most of the work done for them they routinely overcook things and seemingly have no concept of seasonings. They are, unbelievably, incapable of making toast, 15 months here and I have yet to see them do it correctly. I can teach a five year old how to make toast, but the staff here seemingly lacks equivalent intelligence. Personally I would be embarrassed and ashamed to serve food this bad to anyone but especially to disabled and elderly people. The nursing aids are generally good, there are exceptions of course but that's true anywhere. The rooms are clean and mostly comfortable, except they think 75-80 degrees is a good thermostat setting. The brightest part of the facility though is the excellent therapy department. To summarize, excellent therapy, good rooms, if you're under 75 and/or have full mental capacity they have little to no activities to keep you mentally engaged, and the food is absolutely atrocious.

Our mom is a long-term resident of this facility and has been receiving WONDERFUL care! The owners and staff are truly dedicated to their residents and any concerns or questions about our mom's care have always been answered and addressed immediately and with the highest respect for her and our family. We would recommend them over and over again!

I am/was a resident of this facility

I was a former patient of Bellevue Health and Rehab of okc. I have been a resident at this institution a total of 5 times over a two year period. The nursing staff was so caring for me, the last time I was there it was about seven weeks. The nursing staff and aides there were always improving and were so caring and wonderful. I was so impressed every time I had to do any rehabilitation there. The way the therapist treat the patients is truly outstanding and the therapy I received while being there every time hands down beAt any of the other places I have been in town. This is a true A plus facility. All of the staff including the office staff and owners alwYs treated me and my family that came to visit like we were part of there family. Thank you Bellevue for always taking care of me.

Mom was in pretty bad shape upon arrival. She could not stand alone due to inadequate care at a prior rehabilitation facility. We were told at Bellevue "We will get her back to her condition prior to her surgery". The building is an older building, but the staff inside was golden. As soon as mom pressed the call light, they were there within 30 seconds. (they have what I call "hall monitors" sitting in the hallway, just watching call lights and they immediately jumped when she called). Mom had incontinence the first few days. It can be humiliating for the patient, but the staff was kind, and assured her 'it's ok! We are here to help you! Call on us anytime!' The staff was encouraging, but not pushing beyond her limits. The administrator, Heather, made rounds every morning to check on the patients. Mom became stronger and stronger, emotionally and physically, under the care at Bellevue. Mom did not participate in the structured activities, as therapy wore her out! She was aware of activities offered, but chose to rest when she could. The food was ok. The portions were large, and nutritious. Her main complaint was "so much turkey is served, and no fresh fruit". But, that was minor compared to the care she received. (We brought her food from outside on occasion and the staff welcomed the fact she had an appetite!) After 6 weeks, she was able to walk with a walker, quite a distance, and completed all the goals the therapists set for her. Laundry service is part of the package, which I gladly accepted, so I didn't have to do it 2-3 times a week. After returning to her home, she is continuing to do the exercises she learned at Bellevue, to keep getting stronger. After so many horrific stories concerning nursing facilities, this was a breath of fresh air. I could rest assured mom was getting top notch care. I didn't have to be there every day to check on her. I am so thankful for Bellevue, their administrative staff, their nurses, their therapists, their support staff, everyone you come in contact with has a smile. You can tell they truly enjoy serving others.

My Mother-in Law and my Brother-in-Law were both residents at Bellevue Health & Rehabilitation. My Mother-in-Law was a resident for over five years due to a massive stroke and my Brother-in-Law for over 2 years due to advanced COPD. In all categories, Bellevue was excellent. The staff was attentive, They handled all medication issues from ordering to administering. The food was exceptional. The food staff was available to prepare special meals when needed. The staff at Bellevue was outstanding. In fact, my wife pays frequent visits to Bellevue to visit with the personnel who cared for both members of our family.

I went to visit my mom, the food was the first issue. Everything on her plate was over cooked, and cold. The kitchen staff didn't speak a lick of english. And all had terrible attitudes when asked anything. The nurse on duty at the time was inexperienced. She couldn't tell me what the meditation she was giving my mom was used for. The Cna's we're very nice, she adored them. The facility is average. If the food was better, and the charge nurse was helpful ratings would go up.

I visited Bellevue Long-Term Care. The layout and the appearance were very nice. The place was friendly and warm. The person who helped me was very nice. The residents had a nice lunch when I was there. The demeanor of the residents seemed happy.

My loved one kept getting urinary tract infections at Bellevue and had to be on one antibiotic after another. He stayed there for two or three weeks. I complained about a loose tile on the floor, and they fixed it.

My father-in-law was in Bellevue Health & Rehab Center. It was wonderful and helped him with his needs. The caregiving was good and came highly recommended from other people. The grounds were very nice and the location was very easy to access. The pricing was reasonable. We had a great experience there.

My grandmother had stayed at Bellevue after a surgery and had a great experience. We would definitely recommend! The was beautiful and clean. When ever she was in need of anything she said that she never felt that she really had to wait long at all! The staff was always kind and warm and would come any check to see if there was anything extra that she needed. I think she felt a little waited on!

My mother had a wonderful experience at Bellevue. We chose this facility after our mother had an illness that resulted in a surgery. She didn't bounce back from that surgery very quickly and ended up needing physical therapy and skilled care which was a whole new world for us! We were nervous but the minute my mom entered the doors she was greeted, shown her (very nice) room, and then quickly asked about dietary needs and preferences. She ended up spending about a month here, which included Thanksgiving day. The facility had a nice dinner for the residents but we chose to also bring her some food from our meal---all in all, the staff helped my mother have a good holiday despite the circumstances. I would highly recommend this Rehab center and my mother certainly does! She hasn't stopped talking about it yet.

This place is wonderful. It is family owned and there is always an owner on site, 24 hours a day. The nursing and therapy staff are very knowledgeable and show genuine compassion to their residents. The building is always clean and smells nice. I would strongly recommend this place to anyone looking for a place for their loved one.

My friend was a patient and she had a good experience. She had her own room and we could visit her anytime. We had supper with her one night, the staff checked on her while we were there. The place smelled good and we felt comfortable that she was there. She got therapy until she could walk and could go home. I would go to Bellevue if I needed to and recommend them. I also liked that they have are family owned, and they checked on my friend when she was there.

I transferred my mother out of this facility after 48 hours. To say the staff is rude and incompetent would be the understatement of this century. Medication errors, (ordered a blood pressure pill in error then screamed in her face to "calm down" when she refused it) lazy nurses and aids, (I challenge you to catch one not sitting and eating or sitting and playing on their cell phone) and the smell (not sure which is worse, the feces or the mildew) are just the tip of the iceberg. Avoid this place like the plague!

My mom got there September 8th 2015. with a fracture in her ankle. I had to make funeral arrangements today. They did not turn my mother and she developed a red spot they called it. Stage 4 Mrsa was what it was. They failed to mention she had been starving, malnutrition and has been aspirating for over two weeks and her lungs where completely for of pneumonia. I meet with the so called updating team on Thursday got a report that she was making improvements. My mother had never been sick a day in her life, never smoked or drank one drop. They even called me bc mom wouldn't drink caffin. Why they asked I responded the doctor told her it wasn't good for her 20 years ago.t They accidentally gave her a shot of hallidol that made her so lethargic. When I questioned them the answer was it won't hurt her it will where off in three days. The staff seems more interested in caring a TV remote to each person's room so they can change the channel and keep up with the football games. If given a chance don't use the facility and if so bring in your own doctor. For the record I asked to speak and meet Doctor Frank . I truly don't know if he really ever exsisted. I hope she comes back as a guardian angel and spends time there helping those that cannot help themself. I sent my mom to the chamber's if he'll. I hope this helps save someone's life. Thank you, SB

I'm in Bellvue right now. My 1st night here I was given Stewed Tomatoes for supper. I told the nurse and kitchen I was Allergic to tomatoes. They said they didn't get a list of allergies. Ordered Chef Salad no tomatoes day 3. Arrived with tomatoes. Brought it to Dieticians attention. She said "made in advance". I said"don't serve". Day 8: ordered Chef Salad, No Tomatoes, before making ahead of time. SALAD ARRIVED WITH TOMATOES AND TOMATO SEEDS. Charge nurse attempted to tell me "it's not tomatoes it's cabbage", (oh okay). I told her to get dietician now. 3 times tomatoes, still deathly allergic! Nurse came back to take the tomatoes to Dietician. I told her No. Not leaving my room. Wonder when they'll ask me to leave? Calling my surgeon tomorrow to get the hell out of here.

I am fairly happy with Bellevue at the current moment, mom has only been there a little over a month, and so far so good. She is bedridden and needed 24hr care. They had long term care, and it would work into the budget so that’s where she stayed.

I have never in my life seen such cold heartless staff. Storm caused lights to go out call lights did not work no flash lights offered to residents, staff did not come around to room more often due to power outage. Poor attitude aide slammed door when asked a question very unproffesional. HATE THIS PLACE!!

The location of Bellevue Health & Rehabilitation Center was near our house. The rooms were not the best and the maintainance was not very good. There was an electrical problem and it was not repaired until we were about to leave. With the exception fo the therapists, everyone was good. They had crafts, church service and entertainment came in, etc. They needed to update the rooms and made sure everything worked properly as well as there was enough space for two people. The room was just too small that there was not enough room for the equipments of two residents.

Bellevue was an amazing facility overall. I was treated incredibly well by the majority of the staff. I lived in a private room for four months during my recovery from a major climbing accident. Although I was not the normal patient because I was the youngest patient by 30 years, I required the same types of care. I was administered medications, aided in bathing and most personal care actions, fed, and retaught to walk by their physical therapy staff. The facility was kept very clean, the. Food was actually pretty good and they made activities available to keep the patients entertained. There were a few instances where I had issues with specific personnel or equipment. The administration in charge was very accessible and extremely helpful. They rectified any and all issues I had in a timely manner. The bed was very comfortable and a second bed was provided for my family that wanted to stay there with me. There were several lovely interior courtyards easily accessible to the patients. Meals could be taken in room or in the cafeteria. Snacks were provided throughout the day by request and a nutritionist checked with each patient to ensure their nutritional needs were taken care of.

On the first day my mother was there, they left her on the toilet and didn't bother to come back and get her, so having had a stroke and paralyzed on one side she tried to reach her wheelchair and fell. They wouldn't call 911 they were too busy covering up their mistake!
