I was a rehab patient at the Onondaga Center for 8 weeks and agree with all the negative reviews submitted before me. My room was filthy. There was a pile of dead bugs in a corner that was there the whole time I was. The food was terrible and always cold because the trays would sit in the cart for easily 2 hours while the staff would be talking to each other or on their cell phones. I know this because my room was across from what they called the nurse's station. I asked repeatedly to speak to the social worker who didn't even see me when I got the CEO(?) Involved. The only way I got transferred to another facility was by calling their Social Service department and got a social worker that was very caring and helped me get into their facility. By far the worst thing was the staff, including attendants and nurses. Call lights were rarely answered, if you were given one in reach instead of hanging them out of reach. I would watch the staff sitting around the station totally ignoring the lights. When they did come in to do something they were very rough and curt. I would wait hours for pain medication, watching the med nurse talking on her cell phone or with other staff or just sitting. Remember I could personally see this due to my room location. I don't cry easily, but I cried every day that I was there. Don't ever go there, and if you have a friend or family member there do everything you can to get them out.

I cannot stress enough how much you and your loved one should avoid this place. It has two police officers who call it their beat due to the sheer amount of times they are called out there. Patients are neglected on a routine basis, my elderly father was carted off by ambulance and the EMTs who came said it was a regular occurrence to see patients getting worse rather than better after being there and they called the cops who then filed a complaint. Lack of nutrition, lack of patient safety measures, staff who will not admit to lapses in their duty of care even when confronted with their own voicemail messages. AVOID AT ALL COSTS!!!! I am beyond appalled that St Joseph's Hospital would call this a partner site to discharge their patients. They should be ashamed. Of course they also offer Van Duyn so I guess our expectations needed to be low. I feel badly for the patients we left there crying in their rooms and halls as we at least got my dad out. Not before he sustained a fall, not before his body started to fail due to lack of basic necessities like water for hydration, but at least in time for him to have a chance of living.

My loved one was there for two weeks. They never set up a plan of action for them. They didn’t return phone calls. I finally had to leave a message threatening to report them to the state in order to get a phone call back. One day, my loved one was left in a wheelchair for 7+ hours, which meant they didn’t change the depends or check on my loved one in that time. The food is terrible & I wouldn’t feed it to my dog. My loved one didn’t have a working call bell, and they left a bell you would ring at the front desk of a hotel. Trying to find staff to assist in the evening was nearly impossible and sometimes had to wait 1/2 hour to an hour for assistance. During the two week stay, the social worker never called to discuss our loved one. The cleanliness of the facility was subpar. The vent by the patient’s bed was filthy, and a few times, my feet stuck to the floor. I would not recommend this place for someone you love. It will break your heart to see them there or to leave them.

No communication with family No medical assistance being taken care of. Nursing are never available for patients. Social worker never available for family to addresses issues with love ones. Never would recommend this facility to anyone.

My husband was at this facility for 3 days & ended up being sent to the hospital! The nurse who passed out his medicine never bothered to give him water. Result ended up vomiting blood! They say they will investigate well that hasn't happened! No return phone calls! Place needs to be investigated to the fullest extent!

My friend was in Onondaga Center for Rehabilitation and Nursing. It's understaffed. The good staff members were overloaded because the rest of the staff was not good. They walk around holding their smartphones constantly or they sit at the nurses' station and do nothing. The director left, a great nurse left, and the decent staff is going out the window. It's a terrible place. He should have been eating solid food all this time, but at some point they determined he was a choking risk, which he is not. We've dined together a lot of times and he's never choked. As a result, they gave him pureed food chopped up, a lot of sugar, yogurt, pudding, pies, and things that he could easily swallow, but they did not need to have him on a restricted diet. His room was like a prison. The bathroom was shared with a room on the other side and it smelled like urine all the time. He had a roommate with an ostomy bag, and the room would smell like feces because nobody was taking care of it. It was degrading. They had a nice floor-to-ceiling sliding glass door, but it was never open. They had an awful looking American flag, which was not displayed properly. There's no care for the life of these people. It's so depressing and it's very expensive. He even fell getting out of bed one night to try to go to the bathroom because nobody answers the call bells.
