I am/was a resident of this facility

My mother is receiving care currently at The Laurels of West Carrollton. I am her P.O.A. After several conversations with both her and the staff caring for her, I can say she is very happy and wants to stay as long as her insurance allows. She was at home previously where she enjoyed activities at a Sr Center, Church, home-cooked meals, her own room, television, etc. Since spending the past month at this facility, She WANTS to stay. She is receiving first-class care for her therapy. She really enjoys the food, activities, fellowship w other residents, but most of all, the staff! She has been Well taken care of! They rearranged her room to accommodate add'l space for her wheelchair and walker. Her physical needs have been addressed immediately. In the past, she was at 2 other very nice facilities. One in Centerville, the other Dayton. She MUCH prefers here. It was her choice to stay or return to my home. She has chosen to stay because of her great stay. Highly reccommend this facility to anyone.

My mom was taken to this facility due to her dementia after a hospital stay. I would not recommend this nursing home to anyone! My mom fell in the Memory Care Unit and laid for hours on the floor. The staff had moved her walker and placed a wheelchair by her bed! Her fall was not reported to Ohio Department of Health. No one took her to the hospital. They called one family member saying "Your mom fell and bumped her head". They bandaged her head and covered her up in bed to hide her fall. No x-rays were run until I asked if she saw a Doctor. She had an orbital fracture and hematoma on her head with severe bruises all along the right side of her body. Mom's room was located at the end of a hallway and nursing care was non-existent. (We were told this by head nurse, Judy) She moved her to a private room out of Memory Care without our permission and Mom's medical records didn't follow her to the new wing of nurses. They didn't realize Mom needed to be fed, couldn't drink from a straw, or that she was a dementia resident. I had to ask daily for water for Mom. Her care suffered as staff didn't understand her needs. I was by her bed the last three days of her life, unable to do much since the damage was already done. I was able to get her food pureed as they didn't understand mom couldn't chew solid food or suck from a straw. I had to ask to get her diaper changed. I also asked for a bed alarm, which Hospice would provide. The Laurels denied my request. She died nine days after her fall, in extreme pain even though Hospice was providing Morphine. To summarize, this experience was a nightmare for my husband and myself. My hope is that no other family has to endure a place like this that doesn't allow bed or chair alarms in Memory Care to assist their residents and prevent catastrophic falls. This would encourage nursing staff to check on patients even if they are located at the end of a hallway! To minimize this situation by telling a family "Oh, your mom looks so much better than last week!" is completely heartless! This comment was told to me by the Head Nurse Judy! I was speechless as mom looked as if she had been in a car accident! When the funeral home received mom's body, they contacted the coroner as they suspected a brain bleed due to mom's bruising and condition of her body. We are waiting for a full autopsy report. With compassion and care, this fall could have been prevented. There was no care or compassion at this facility. If I could give this place zero star as a rating, I would. Avoid The Laurel's as your loved one will be ignored with little to no care. Trust me!

Staff is rude when they actually take the time to answer the call light. Often times come in and turn the light off saying the will be back shortly and never return. If they do come back its normally hours later. Father had to call me after waiting 5 hours to be changed. Would not ever put another family member in this place.

I visited this facility

The Laurels of West Carrollton (formerly The Rehabilitation & Nursing Center at Elm Creek) was too small and sort of depressing. I didn't like it. It was pretty crowded. There were lots of people there, and I got the impression that they were a bunch of zombies. I would not put my husband there. I was interested in their memory care unit because my husband has dementia. The rooms were double occupancy and on the small side. It looked clean and they had a lot of things going on, but I was turned off when right off the bat the girl who was going to show us around said, "You have any questions?" I also saw a lot of people sitting around and not doing anything.

My father had surgery and he had to go into the Laurels (Rehab Nursing Center Elm Creek) for a couple of weeks. The food there was bad, it was not very appetizing. When he was there, he was just sitting in his room, no one would tell him there were activities, and he can't look at the calendar to see. However, they had an excellent rehab center. The facility itself was clean and the staff was really nice. Our sister was also there. She had a massive stroke and had to be in The Laurels for about two months and they were excellent with her, that's why we chose it after Dad's hospital stay. As far as the facility goes, it's clean. They have a dining room, they have activities, and the staff is very friendly and excellent. It was just the food he was getting was bad and he was just sitting in his room. We later found out he wasn't put in the memory care section. He was in put in the regular nursing care section, so that's why he wasn't being told when there were activities, because normally people don't need to be told. It wasn't totally their fault.
