I am/was a resident of this facility

I'm going to stay here at Creekside Transitional Care & Rehabilitation because they've given me a big private room. We have 24-hour nurses, and we have very good food here. They take excellent care of us. It's not like an assisted living facility with fireplaces and all those fancy things. Every hallway has its own RN, so they don't have to run everywhere. You have them available to you immediately. And we have all CNAs, we don't have any med techs. They have three or four doctors that come in once a week and go through the whole place, and they're on call anytime they're needed. If they cook something and we don't like it, we just send it back to the kitchen and tell them what we want. There is a separate menu that you can get most anything on, and they will fix it for you and send it to you immediately. We have several clubs. There's a men's club, a women's club, and we have arts and crafts. We have a large TV in a TV room, and the arts and crafts department has all kinds of special things in there where they bring refreshments in. Their therapy department is excellent, too.

I am a friend or relative of a current/past resident

My mom went to Creekside Transitional Care & Rehabilitation for rehab. The staff there was just amazing. They all communicated and stayed in touch with the family on a pretty much daily basis. When we have any questions they're prompt to answer, and if they don't, they find somebody who knows the answer. They treated my mom with dignity and respect. I would recommend it to anybody because it's just a wonderful place. We were able to have meals with my mom and it didn't cost us anything. That was really nice. At the hospital, we had to pay for our meals, but at Creekside, they offered us meals to have with her. The food was really good too. They had a wonderful layout for physical therapy rooms with multiple options to work with depending on the patient and what their needs are. They have everything in there to meet multiple needs. They have a transitional part, which is where my mom was, and then they had the long-term care as well. What I saw in the long-term care part was that the staff was really prompt with them also, and they do have different activities for them to do throughout the day. They keep those posted, on what's available to do, and they have a chart on the wall that tells you your choices of meals. It was really nice for the long-term and for the transitional. We were fortunate.

The people at Meridian Care and Rehabilitation Center were very nice, but my dad was not happy there. He said the nursing staff was excellent, but he did not like the executive staff. He didn't like the food. He pretty much stopped eating. He lost 37 pounds while he was there. The staff tried and they did everything they could and I don't blame them, but he was not happy there. There wasn't anything for him. They had a TV room type thing. I didn't see any other activities that they did. They offered to wheel him down there occasionally, but for the most part, they didn't get him out of bed. I would not recommend it. It was old, so the upkeep was not very exceptional, but it was clean. They need to improve the quality of their food. It was extremely high priced compared to any of the other places we looked at. We paid on average a couple of hundred dollars more a month. The people that I talked to were all exceptional, but they need to improve the quality of the facility and the food.

If I could leave a 0 star rating I would. We check my grandfather Jimmy into this skilled nursing facility about 3 years ago after his Multiple Sclerosis progressed into 24 hour, advanced care. It started out organized and friendly. They sent a shuttle to pick my grandfather up at our house and take him to Genesis Meridian Care. They did not do an initial inventory sheet, we found out later, and WE had to fill out an inventory sheet. The staff started out by taking great care of him; by giving him his medication, assisting him with eating, bathing regularly, and shaving. The basic-advanced help he required (ADL). It went down hill from there!!!! The staff lost Jimmys dentures 2x, and broke the front tooth out the 3rd set. The staff all together quite putting his dentures in and they no longer fit in his mouth, requiring him to not longer be able to chew harder foods. The staff also lost MULTIPLE sets of hearing aids. The staff that does laundry switch 90% of Jimmys clothes with another client clothes OR just gave him the grungy lost/left over clothes, with holes in them, that were 3XL. Yes, we put his name on the inside of his clothes. The staff could not find his "nice/ matching/ sweat suits" we bought for my grandfather before going to Genesis Home Care. Within a year Genesis Meridian Center had over 15 different staff member that worked with my grandfather. No staff was ever consistent with the care Jimmy. I would randomly pop in and he would be SOILED, NOT SHAVEN W/ LONG DIRTY FINGERNAILS, and LONG SCRAGGLY HAIR and just left him in his room uncomfortable in his bed. After Jimmy got bed sores, the staff brought a "blow-up" mattress in. On 2 different occasions I saw "BLUE/GREEN bruises" on his temple eye!!! I talked CONSTANTLY TO THE STAFF ABOUT THESE ISSUE!!!!! THEY NEVER GOT FULLY RESOLVED. I DO NOT recommend you leaving your loved ones here at this location. The sad thing is that my grandfather lost the ability to speak. I would talk to the clients that could speak and they said the care is ummmmmm fair, to not good. When my grandfather past a little bit ago I was give his belongings from the center. We went in later to see about his BSU blanket that was taken and not given to us; Also, his expensive wheelchair, the staff said they did not have Bc it was not on his inventory. Again I do not recommend this Center at all.

I am a friend or relative of a current/past resident

My significant other was in Meridian Center Genesis Healthcare. He lived there for several months and the biggest problem was it was just way too far away from me and my home because we live in the northeast part of Boise in the highlands area. It was liveable, but they weren't personal about things. Everything over there seemed to be connected somehow even though it may not be in writing with Saint Al's Hospital and living in the north end of Boise (I'm a St. Luke's person) and we live almost a stone's throw from St. Luke's. The other problem was the noise level. They weren't even all old people. They were people within infirmities of some sort who were there and I thought maybe a lot of them were hard of hearing. It wasn't cold, but there wasn't the warmth and all the smiles like where he's at now. The food was so much better than it was in the hospital. They brought snacks of fresh fruit. He had physical therapy there. They had an activity room and a physical therapist that came every day. They had big activity rooms with physical therapy type equipment that they could use with or without somebody directing them. They walked up and down the halls. It was terribly noisy. Every time a cart rolled out in the hall, it just rumbled. He had a television in his room and a good part of the time, I couldn't even understand the television because the noise was so loud from the hall.
