I am/was a resident of this facility

I was in Bella Terra for a month. The food was terrible. I will give a big shout out to three people in the facility - Crystal and Deloris in therapy and Deana the wound care nurse. Susannah the social worker was okay. Most of the rest of the staff don't seem to give a damn. I wouldn't recommend this facility to anyone.

I am a friend or relative of a current/past resident

My mother is in Bella Terra Lombard. My biggest complaint is that I see residents late at night hitting their call buttons, and it's taken quite a while for nurses to respond to it. I know they're short-staffed, so they get to them when they can get to them. That sometimes has been an issue. I noticed yesterday visiting my mom's, that her fingernails were dirty and that should be attended to. When I come around, I see a little more urgency because a family member is there, so there's some care there. For the most part, they're changing her and they're keeping her bed linens clean. They've taken her to the doctor a few times. They get some things checked out with their arms and legs. Medically, they seem to be keeping up with things nicely. One time, they had all the residents get out of bed and had them all in the cafeteria, and there had to be three or four residents in their wheelchairs, sitting in their own urine that was dripping on the floor. I've only seen it once like that, and I was not happy about that. Her room is clean, and the bed linens, pillows, blankets, and all that are changed. The hallways are clean, the restrooms are clean, and the facility is very clean. The staff and the nurses are friendly. I spoke to a doctor once and he was good. The CNAs were friendly and courteous. They have a couple of hospitals close by and there are a lot of shopping and restaurants nearby that they can order from. I see some of the residents doing DoorDash and having things delivered. It could use more public bathrooms, but the rooms are adequate. It is an older building and it's a little outdated in some areas, but it's adequate.

I am a friend or relative of a current/past resident

My dad is currently in the skilled nursing building of Lexington Health Care Center of Lombard. It's across the parking lot of the independent and assisted living community. He's been there a while, and he has received very good care. I think they're nice people though I think my dad's kind of a pain to deal with. They've done a good job. He started out there in nursing rehab and technically he's still there. With insurance, they wanna kick you out after a certain number of days, but he's now accepted on Medicaid. He was being provided physical therapy to help him recover after a septic infection that diminished his ability to walk. He's kinda getting help with daily living aspects. I think he's generally pretty happy there. From what I've seen, it's clean and organized, and they have a little reception room near the front door where you can have window visits on a pre-scheduled basis. My dad does not like the food there; it's pretty much all the same with other places.

Lexington Health Care Center was very nice. The staff was very pleasant. It appeared to be very clean and also smelled better than other place, but I noticed quite a few weird practices when it came to handling transmittable diseases.

I visited this facility

In my opinion this facility was an extremely unacceptable place for a person to live let alone someone that needs assistance with their daily activities. The living quarters themselves were of an acceptable standard, but the staff was completely unacceptable. Most of the staff appeared very unqualified for their position, as shown by their poor decision making skills, and some of them couldn't even speak English very well. The staff was very slow to respond to requests and on occasion forgot about residents requests altogether. On multiple occasions they changed my grandmothers medication which I had been keeping track of without contacting me or any other family member and put her at extreme risk. I was forced to monitor every move that was made in regards to my grandmas care to make sure her health and well being was protected. On multiple occasions they almost gave her medication that she was allergic to. I was extremely disappointed with the care that was provided and believe that some of the practices preformed here were not only morally lacking, but also problems that could be prosecuted by law.
