Worst place ever all the staff are lazy including administration. The best person there overall iCassandra second wing nurse in charge. I wouldn't leave a cat in that facilities care. The facility is very dirty. My mother got a stage 4 bedsore due to lazy staff and administration lack of enforcement .

This facility has all new management, and they are excellent! The Executive Director Rachelle is amazing with a heart of gold and her love for her residents’ shines! The Director of Nursing has great communication and is very compassionate! She took the time to speak to me several times with updates on my loved one! The therapy Director Jordan is very knowledgeable and gave me updates when I asked. When visiting everyone was very nice and full of smiles from the front desk to the CNA's working the floor. Everyone also seemed to know the names of all the patients! Very impressed and nothing bad to say! Also, the Dietary manager is new with obvious experience, my loved one had no complaints about the food!

I have personally seen food served many times. It is inappropriate for seniors with no teeth, or swallowing issues. Things like potatoes with skins, sausage with casings, tough meat or meat that is served with heavy gravy. Most of the time it looks like prison food

My dad is being cared for here by a group of very caring nurses, therapists, administrators and staff. He has significant medical needs and they are very attentive to his care. The facility is very clean and the residents are friendly and seem happy to be here. The activities director plans wonderful events!

This is the worse place ever!! My father was their for maybe 2 days and I was with him I was told he didn't eat breakfast or lunch and for dinner the gave him tuna with mayo which he can't eat then hot dog with mustard witch he is deadly allergic to. They did not come in for over 6 hour I cleaned my dad , help sucked period they don't care. I seen my dads breathing was faint so I asked for nurse she comes in 10 min later and says his chest is moving denied a anbulance till another nurse started to take his vitals and said he needs one ... If you send a loved one here they will die.. that's how I feel.

Avante at Leesburg was good, but I'm not too impressed with any nursing home. It's just depressing seeing all those people in wheelchairs, but it was adequate.
