My mother spent her last year at this facility. After a serious fall, she was bed-ridden. She needed water to avoid dehydration. However, every day when I visited, I found her water mug filled, but sitting at least 2 feet out of her reach. I would get a portable dining tray and move it close to her bed. When I returned the next day, the table was gone and her water was on the other side of the room again. One afternoon I filled her mug with soda which she loved. I was unable to visit for 3 days, but when I came in, the soda was still in the mug...3 days without water! As for the staff, she had one caregiver who was wonderful. However, the majority of the staff seemed uncaring, insincere, and bothered when I complained. 2 pairs of her eyeglasses were lost. And I frequently found her in a "wet" bed. I strongly discourage people from using this facility. My mother has passed and her last days were made peaceful from an outside hospice organization.

DO NOT SEND YOUR FAMILY MEMBERS HERE!
My grandfather just had a stroke and my grandmother got a information from her Independent Living Center on a nearby rehabilitation center. Garden Valley Nursing and Rehabilitation Center was the closest. This place is HORRIBLE. First of all the beds look like army cots, the staff is RUDE and didn't listen to my grandmothers instructions on what he sleeps in and that he is confused and his right leg was still not completely rehabilitated. So sure enough the next day we were informed that he did try and get up and fell on the floor. We specifically told him that he would do this and that he need to be watched so he didn't try this. My grandfather had not shaved him, bathed him, and his pajamas were still neatly folded in his dresser. The woman occupying the adjacent bed comes everyday and changes her fathers diaper because they don't do that. The next day my grandmother went and she was changing her father and my grandfathers bed sheets because they didn't do that. I read another review that a mother with a low sodium diet was fed CHEETOS for lunch. Also I'm not completely convinced that he got any physical therapy. We were with him during the speech therapist who was there for approximately 10 minutes working with him. The staff was yelling down the hall, cracking jokes, high fiving and being completely disrespectful and unprofessional. This place needs to be shut down - Medicare website gives it a 1 star out of 5, I'm assuming because negative stars wasn't an option.
