My sister was at Ascension Living Villa Franciscan Place for two weeks. It was good. The second time we used it, they treated us well. They treated my sister okay. The room was very good. In the beginning, it was supposed to be for two people, but they never got another person, and then at the end, she was alone in the room. They had very good facilities. Really comfortable. All of the staff were very gentle and very passionate with her. They were very good to her. In general, they were clean, so I don't have any complaints about it. Everything looked nice. The food looked good. My sister usually ate very little. They had a very good menu there for people who eat everything, but my sister has trouble swallowing and there has to be some kind of specific food. They offered Ensure.

The care here is excellent. Have had more than one stay this year after surgeries to my leg. The staff is great and caring. Therapists are good at what they do and the reason I'm walking though with a walker. Food is pretty good to. Rooms are comfortable and kept clean.

I went to Ascension Living Villa Franciscan Place to visit a friend. It was very good and they had a very caring staff. It's a clean facility. No odors or anything. There's an ice cream parlor and it's right next to a hospital, which was convenient. The food looked very well prepared. My friend's room was very nice, bright, clean, and a nice size. So, they were getting their money's worth.

Mother in law needed to be moved from this facility to an assisted living facility. My sister in law left THREE voicemails for the social worker to have this arranged. NEVER received a return call. Mother in law passed away at their facility. THAT is when the social worker FINALLY called because they needed to have her body moved as soon as possible. Social worker insisted we give the name of the facility we were going to use and stated otherwise they would move her to the facility of their choice. I called the social worker to discuss this, of course had to leave a voicemail and NEVER RECEIVED A RETURN CALL. Unprofesssional, no compassion -- disgusting behavior.

My sister was in Provena Villa Franciscan before she passed away. It's an old place. I call it the dungeon. If I had my preference, she would not have gone there. For example, they put her in a room by herself, which was fine, but the room looked like a janitor's closet. There were wheelchair parts just sitting in the room, not neatly organized, just kind of on the floor, and they had a dresser in there which was falling apart. They had a whiteboard actually as they do in the hospital, so you could put information on there, and it was all written up with black marker and very messy looking. The floor was filthy. The hallways, it's like walking into a dungeon, it's very dark, closing in on you. If a sick person is going into a place like that, it seems like it would depress them even more than what they already are. When we were there, the staff was very kind and nice when we asked them a question. It was my second experience with that nursing home, and I didn't want my sister to go there when they dismissed her from the hospital, but I didn't have any place else where she could go. There was no other place that would take her. They could brighten the place up so you don't feel like you're walking into a literal dungeon. They could use a paint job. The hallway was not clean. They have a swinging door. You just push it open to enter into the hallway where the rooms are. That wood's all chipped, and it's just all marred up. It depressed me.

The staff is slow to respond to room calls and going to the nurses station is no different and you must address them to get a response . I was told that they would give you a 2 weeks notice prior to discharge What happened was quite different. I was called in on a Thursday evening to have a discussion about what care would be needed and when discharge would be and to my surprise they told me Thursday so that gave me a week to prepare I wasn't happy about that The very next morning I received a call and was asked if a decision had been made on what facility she would be going to when I had not even had the time to see them but I assured them they would know by Thursday .they then told me that discharge had to be monday or pay for everything out of pocket. I informed them the first day of how important this was because I am disabled and need assistance to care for her I trusted them and needed that time to prepare .

My father-in-law was here after back surgery caused a heart attack so that he then had open heart surgery. He was there for 97 days. He was unable to do much for himself. The best thing was the physical therapy, but there was a lot of turnover so whenever my father in law got accustomed to someone (about 2 weeks of nearly daily therapy), the therapist without much explanation would simply be gone and replaced by some random other therapist. The room was okay. The reception desk was okay. The staff on the floor were very very slow to respond to resident's calling for them. If called, they would typically come 45 minutes or more later, meaning that my father in law would have had to go to the bathroom in his diaper pants which was demoralizing at best. Or, importantly, that he could have been lying there in active pain or having fallen, and it wouldn't have been noticed for over 45 minutes. Unsafe, in my estimation. They were even unresponsive when family members would walk up to the nurses desk to ask for specific things. They'd barely acknowledge the request and then would take over a half hour to respond most of the time, even to a fairly simple request. And often I'd wait standing at the nurses desk for 5-15 minutes before any nurse from the glassed-in full nurses station would come out to find out what I wanted. They didn't even meet my eyes and indicate I would wait. It appeared they were talking about personal business, not in the middle of work responsibilities. I eventually began sticking my head in the door to give them the room number and ask if someone could help me. They really resented having visitors impinge on what they clearly felt was their private area, but when the resident needs something we are unable to provide ourselves... Anyway, they gave the appearance of not caring at all about the resident or the visitors. The worst part was discharge. They scheduled us for a Sunday discharge. We were given little guidance about what they thought was necessary to allow him to live at home - or even if he was actually ready to go. But we okayed much of what were options during the prior week and were told he could take him home Sunday (and if we didn't, Medicare would no longer pay). When we drove there to get him Sunday, not all of the supplies had arrived at his home; in particular not his commode needed for bathroom use. When I got a nurse's attention and asked her about that, she said that the department that handled discharge was not in on the weekends; only when I pressed the issue asking how he was supposed to be home without being able to use bathroom safely, did she 'page' the right department. Which department never responded! We bought a commode ourselves from Home Depot just to take him through that first night. and that we had not come at a time when they expected to handle discharges. So the nurse was unprepared and put out. Said we were there during medication rounds. But no one had given us any time guidelines! In the end, the nurse got something printed out that somewhat passed for being discharge instructions and then handed us some medications from a cart in the hall with rapid fire verbal instructions. They gave us no training in how to load father in law into or out of any vehicle. When we got to the pharmacy, the prescriptions were incomplete. Then, when I hired an agency to come to the home, and the agency ordered a hospital bed, the nursing home cancelled the order. It took us several days to figure out why it didn't get delivered as promised. These repeating errors would have been funny if we weren't trying so hard to try to avoid another 911 call from home. We only made it one night; the second night the EMTs came out and he had to spend the night in the hospital ER.

I visited this facility

Presence Villa Franciscan looked a little newer than the other community I visited. From what I've seen in the rooms, everything seemed well kept. It looked clean like a hospital with beds, paint, carpet, everything pretty much up to date. It didn't look ragged or that there's lack of care. The rooms are on the small side, but there's still plenty of room. It is a skilled nursing community. Usually you see a lot of wheelchair scrapings on the walls, and door and floor, but you don't see such damage on this place. The air conditioner and heat worked a lot better than the other community I've been to. The patients look happy and content. The patients said the food is good, and they had a good cook. Everybody in the therapy department was wonderful. It's a definitely a place that I want to put my father in, but they have a long waiting list. The calendar they showed me has plenty of activities. They have about two to three activities in a day depending on the capacity of the resident, so that's quite encouraging. It's setup for people who are older with handicapped sinks and bathrooms.

My mom was at Presence Villa Franciscan. The staff was great, but they just had a skeleton crew. It was not the staff's fault. They would come in the morning and they would wash my mom up, but nobody would show up again until I got there. So imagine, my mom cannot move, she's sitting in her chair from the morning until 12:00 to 3:00 p.m. before somebody could come back to her. Every time I visited, my mom would be soaking wet or full of poop. I hope they change their management because it was not good. In the front when you walk in, it smelled fine, but when you go to the back, it smelled bad. A few things were broken in the room, but they would just try to go around it if they could. My mom had a roommate, they tried to keep the room decent, but it was not as satisfactory. They would put the food in front of her, but she had dementia, so she didn't know what to do with it. They did have activities. However, they would just put the people in the room and turn on some music or turn on the TV. The residents also would sit in the hallways in front of the nurses station so the nurses could watch them. They did have a beauty shop and a barbershop, but they didn't have a lot of staff members to engage with the residents.

My grandmother was here after a bad fall. They got her back up and walking and now she's almost back to where she was before it happened. Some things weren't perfect, but the staff and management would make sure things got fixed ASAP! Their therapy is a 10/10!

My mother was here and she had a great stay! The physical therapy after she had her knee done made all the difference! When I asked questions about her care I was always given answers and not given the run around. Her private room was great and big enough for the whole family to come and visit. Housekeepers were always cleaning and I rarely ever smelled anything bad. I'll have to copy my mom when I get my knee done and come here too!

My mom stayed at the Villas for 6 weeks. She was there for daily rehab. The staff ( nurses, CNA'S, PT,OT, dining room staff, activity director, housekeeping, etc,) were all wonderful. They got to know my mom well during her short stay. Rehab was excellent. Activities offered were great. Her room was cleaned daily. It was a nice place for my mom while she got stronger after her surgery. Thanks for your hard work and dedication in helping the elderly.

My dad was at Presence Villa Franciscan. The facility needed to be updated. It was certainly old. The care was good and the staff was very good, but the facility was very old and not that clean. My dad had a private room at one point and then he had a shared room. The rooms were very nice, but everything needed to be upgraded. The aides were very good. They were very attentive. At meal times, they were very good about working individually with each of the residents making sure they have what they wanted to eat and it was almost like a restaurant setting. The dining area was nice.
