The aides are very understaffed and the nurses display pure arrogance speaking disrespectfully to the aides and refusing to help them. The supervisors are very nonchalant and and very inconsiderate of the families feelings. The food is terrible and on top of that never on time .There is always COVID in the facility and the proper measures aren’t taken to assure the safety of anyone else . I would not recommend this place at all

Rooms are clean.nice atmosphere.staff is very busy on all shifts,extra staff is needed.staff always very helpful to me when I am need of care I am bedridden so I am very appreciative for their care

This is the worst facility ever I would not send my dog do not send your family members if you love them nobody ever answers calls patients are left in the room alone continuously with nobody answering them for help the food and the facility suck

My brother was a resident at Transitions for approximately five years. I am his sister and served as his POA for 30 years. I attended in person or by phone his quarterly care sessions which were frequently cancelled by the Director of nursing (who is not longer employed at Transitions). Although I live out-of-state, I visited every few months for three days at a time; always ready to meet with staff and to assist with extra care as needed. I spoke with his hired caretakers daily and would infrequently call the staff (knowing how busy they were) but I would always be available when they phoned me. My brother could not advocate for himself and the care he received was at times dismal, sometimes adequate. I hired two loving caretakers to visit him almost everyday: one who was with Ralph from the day he entered Transitions) and one who was hired in the last year of his life. Typically one came at lunch time and one at dinner time. My brother was blind, a diabetic, and a double amputee who sometimes sat in a wheelchair but for the last 6 months of his life, lay in bed 24/7. He needed help with eating, etc. He had a WONDERFUL sense of appreciation and was thankful for whatever quality of life he still had. A TRULY REMARKABLE HUMAN BEING. Transitions struggles to keep nurses or aides; many of the aides complain about each other or the resident quite openly. I could present many examples, but will suffice with this one: Aides would come into my brother's room with the assignment to feed him (he needed assistance the last few months of his life); when a hired caretaker arrived the aides would have the door closed, the food untouched and not offered, and they would be either sitting using their cell phones or using his bathroom. If your loved one cannot advocate for themselves or you are not with them 12 hours a day, I suggest you either look elsewhere or hire advocates. My brother died at the end of October and I have yet to receive a call or a note. The staff notified one of the caretakers of his passing but never did they reach out to me. During the last few weeks of my brother's life, I tried to speak with the then, Director of Nursing, to seek input on a decision whether to place my brother on hospice. I made three attempts over a week. She NEVER returned my call. She has since left after receiving a "Certificate or Caring Recognition" (from my point of view, a very undeserved recognition). I even phoned the Executive Director, to plead for a return call. I was assured my call would be promptly returned, IT NEVER WAS. She has since left after receiving a "Certificate of Caring Recognition" (from my point of view, a very UNdeserved recognition).

My sister is in Transitions Healthcare of North Huntingdon right now, and I like the social workers and all the help. They seem to be very accommodating with her. She has trouble with her eyes, and they accommodate that; they also communicate well with me. She likes the food; that is a big deal because she is all about food, and she is very picky. They give really good meals. As far as appearance, when you walk down the halls, it is kind of more like a hospital. It is nice, clean, and everything, but it is busy. There are a lot of people working there and a lot of people who are in the halls. It kind of gives me the feel of a hospital for some reason. The rooms are OK. They seem clean, and they are semi-private. It is nice, it is not very luxurious, but it is comparable to all the skilled nursing places.
