My husband was at The Oaks - Limestone and I wouldn't want anyone to even consider putting a loved one there. They are very understaffed. They're not nearly as attentive as they should be. My husband was left wet in his bed for hours and when he pressed the call button, there was never an immediate response. It's just not a good place. The upkeep was good, however, I never got to see the inside of a room of a resident, of someone that is there in the nursing home but the rooms that rehab patients were in were just incredibly dull and bland. Not one picture on the wall. It's just not a pleasant environment. The meals were fair to good. He was supposed to be receiving rehabilitation exercises daily but that didn't happen, maybe once or twice a week. He did not get the necessary care.

The only positive comments I have about The Oaks at Limestone are that two of the physical therapists I worked with were very good, and the nurses and CNA’s were good. The food was absolutely horrible. The facility did not have the equipment needed for my physical therapy the entire first week I was there. They did not have a wheelchair to take me to the physical therapy rooms, did not have a bedside toilet, and did not have a walker for my use during my first week. I did not receive a shower the entire time I was there. When I reviewed my discharge papers, they showed that I was given three Covid tests. I only received one. There were medications listed that I never was prescribed nor received. I am going to contact my insurance company to report the issue with the medication charges I’m sure they received. They sent me home with medications that I did not need. Again, I’m sure my insurance was billed for them. They sent me home with a two month supply of the inhalers that I use, which I took with me and used my own while I was there, evidence of unnecessary charges to my insurance. I have read countless bad reviews about this facility. A nurse at the hospital I was in for 32 days advised me not to go to The Oaks because it was not the same facility that I had used two years ago. I should have taken her advice! I tried calling their 24 hour hotline but no one was there.

The Admin totally drops the ball. The ration of caregiver to patient is very low. Pressing the call button may get you help in am hour, but in some instances that’s too late. The phones ring off the hook and when someone finally answers, the left hand knows nothing about what the right hand is doing. The rehab staff seemed competent, and some caregivers were very nice. However, when a simple request was made to help my father get water in his denture tray at night, even after leaving a big note on the door, this was not done or passed on to others. A simple request he have fresh water daily with ice was not done. Overall they are grossly understaffed, and do not work as a team. I would not recommend this place unless you do as many do and go daily to check on your loved one. As a nursing home I would not leave a rat there, much less a family member. Felt like I had to constantly fight for proper care and follow up plans. Two people never said the same thing and without staying on top of things, lots fell through the cracks.

Being in rehab is no fun for anyone at any age. Some staff really kind but overall the admin dropped the ball. Sent home with no help, no supplies and no info on how to deal with significant medical condition. Not my problem is not a great attitude. Would never recommend a Pruitt home again.

I feel like this place is very unsafe. The CNA to patient ratio is very unsafe. My call light would sometimes not be answered for an hour and one time 1 1/2 hour. During that time I pressed the call button every 20 minutes and the button was turned off at the desk. I am a full code. What if something happed and I urgently needed medical assistance. I felt like in that case they would just find me in the bed dead. The nurse Femmy would disappear and the CNAs could not find him. One day I waited 2 hours for pain meds and was told by CNA “I can’t find the nurse”. This happened on 2 different days. The food is too spicy. I kept having stomach pains because my meds were changed from Nexium back to Prilosec. I had my husband go buy me cans of chicken noodle soup. Sometimes I could get a can warmed, sometimes not. Ice is not distributed every day. So on those days I drank room temperature water. If you can’t get more, qualified help to care for all your patients, then you need to cut down on how many is being admitted. On the day before my discharge, the nurse tried to give me Insulin. I am not diabetic. I asked the nurse why was I to receive insulin. I was told he didn’t know that he was going to be the first one to give it to me because it was a new order. I had him check my blood sugar which was 98. I refused the insulin and told him he needed to call the doctor and find out why the insulin was ordered. When I was discharged, the nurse tried to send me home with insulin which I refused. Things need fixing quickly before someone is harmed.

My dad was at The Oaks - Limestone, and our experience was horrible. They did not follow the doctor's directive. They allowed my dad's wounds that were healed to open completely back up on his foot surgery. We had to go back to a wound bag, then he was back in the bed. It was not a good experience. They yelled at him to go back to his room if he came out of the room in his wheelchair, because he could roll himself into the wheelchair. He was in the rehab part, and they were sterile. Like there's nothing in there, even pictures for them to put up on the wall with little Command stickies, they didn't put them up. Nothing, and he was there months. There were some of the staff that were actually friendly, but not overly. There was one lady that was particularly friendly to me when I would bring in food to him. Several of the ladies didn't even really comment. When I took food in through the window, they could have washed their hands, but they were opening his food, and throwing everything in the bag with it, and that was during COVID. The facility itself was clean though, and inviting with little benches to sit at.

This facility is not the best care or safety for our seniors. They have the least amount of compassion for seniors. There is not enough coverage in the evenings and a few CNAs that should not be there helping our seniors at all. A couple of CNAs that we're on my relative's hall allowed my Dad to fall more than 4x's in a week and a half. 3 of those times.were on the same CNAs watch... Unacceptable! My Has suffered with anxiety, leg injury-blood clot, as well as Congestive Heart Failure while in this God Forsaken Place . Others CNA's like April, Kirsten, Sylvenia, Destiny, Chaquille as well as a few others were phenomenal and very attentive to my Dad and other patients when assigned to his hall and when not. We brought it to the head staffs attention a few times but nothing was done. The CNA is still there and other Patients still suffer from her disappearance, lack of will power to do what she is supposed to do, laziness and fakeness. She pretends to be attentive when patients family is there but disappears shortly afterwards. I am not pleased and my Dad's health went down hill.as of the last week with them after the falls. He was walking and eating himself the 1st 2.5 weeks then all went downhill after her fell numerous times. Don't send your loved one there if you can help it. They don't even lock the doors at night after visiting hours. Many nights after 10pm the doors.to the facility were unlocked daily unless someone was told to lock it by a family member of patients. Pruitt Health Oaks at Limestone need help and need to step up their game with caring for our loved ones. Dismayed and Not Pleased! Ms. Indisbelief Jan. 2019

The Physical Training (PT) was good, the regular care not so much. Daytime staff was courteous, the nighttime noisy. Food was over-cooked, tasteless & often cold. I never saw my room sanitized, nor was I roomed on the PT wing as told (there appeared to be beds available). After being told home PT would start within 3 days after discharge, the Amedisys Agency was not contacted until 7 days later after multiple phone calls from us. I would not go back.

I want to thank the staff at the Oaks for helping my family member in seeking services in the community. The many times I visited the facility I found it to be clean and pleasant. Thanks to all of you for your effort and assistance.

I can't count how many things that went wrong while my Mom was here. The nurses and Aides don't want to really do anything for the patient, just going through the motions to collect a paycheck. When we complain the Nursing director & patient care manager did nothing. No accountability for their lack of care or laziness. My Mom went taking about 6 pills a day, By the time she left, she was taking over 25 pills a day and we had never been advised of it. It was just another way to line their pockets. I would never recommend this place. Patient care is not their goal, especially for an elderly patient. The rooms ate generic, little to no activities, parking is horrible, no place to visit. Go somewhere else.

I would advise that you take your loved ones somewhere other than The Oaks. My mom was there for rehab. Terrible experience. She went 3 weeks with an untreated UTI. We asked many times to do a Urine test with culture. After a week they did a dipstick test. It was negative. I asked them , did you do a culture as we requested? They said no, 2 weeks past, finally they did a culture. They came back and said. Your mom has a UTI. Three weeks untreated. On top of that the therapist came in insisting she do rehab even with an IV and again when she had the flu. It is understaffed. I could go on and on. On 3 occasions the nurses were rude. Physical Therapist want you to work out like Rocky Balboa regardless to how you feel even with the flu.. Stay away from this place.

The worst care! They are short staffed and don’t have time to properly care for the patients. I called for a nurse, hadn’t come 1.5 and she still hadn’t come by. It’s noisy, disruptive, no privacy! Do NOT bring someone you love here.

I wish I could give 0 stars for The Oaks. This is absolutely the WORST place for your loved ones. The staff is uncaring, inattentive, and unprofessional. I should not have to give advice on the steps that should be made to give my father the care that he needs. I have no medical education. I could list a thousand examples of why this is the worst health care organization, but I will only list the most discouraging one. My mother died unexpectedly while my father was at the Oaks getting therapy for a recent stroke. We called The Oaks making them aware of the situation. We told them that we would be at The Oaks in about 30 - 45 minutes to tell dad about mom's passing. We asked for advice on how to handle telling a recent stroke victim about the death of the love of his life. They gave us no advice. Ten minutes in to telling him about mom, house keeping came in and asked us how much longer we were going to need because they needed to clean the other side of the room for a new patient coming in. We told house keeping what had happened and they were very apologetic and left. 2 minutes later, a nurse came in and explained they needed us to move to another room so they could clean the other side of the room. 12 minutes for us to tell him about mom, the love of his life, dying unexpectedly! The sad thing is there are no good places for sub-acute care in Gainesville. It's a shame that this was the "best" option. Totally unbelievable.

First off... Kristin and Sabrina are wonderful. The rehab there is amazing! They work hard to get these people back on their feet! They really do a wonderful job! BUT>>>> Staffing is sorely insufficient. I witnessed alarms going off for long periods of time (1/2 hour or more) and staff ( Green uniform male rehab side) sitting at the nurses station going through social media on his phone the entire time! Really??? (this is not just once but day after day) Have you no compassion? We (the family that visit) are mostly older people trying to help care for the seriously elderly. I have a bad back and cannot transfer my mother by myself. The average wait when using a call bell to get some help is 1/2 hour to 45 minutes. The food is TERRIBLE! Not just mediocre it's terrible! If there is one place that this facility should strive to improve.... it the food they serve these unfortunate folks, AND fire the deadwood that is unwilling to work and has no compassion!

Rehab team is absolutely amazing. Friendly, caring, informational, and fun. Food is ok. Rooms are clean. They do have private rooms available upon request/availability.

The Oaks at Limestone was nice and clean. The room was of a nice size with two people in the room. All of the workers were polite and seemed to take good care of the people. They had my friend do physical therapy. I would recommend them if you’re in Gainesville because it’s convenient. The food looked good, and the people were pleased with the food. It was a lot better than hospital food. I have several friends who stay there, so I visit a lot.

I stayed at the Oaks for about a month. It was clean, the people were friendly, and they prompted me to do things that would strengthen my body. I had another companion in the room with me since it was a two-bedroom. I had the food mostly brought to my room, and it was edible, so it was good. They did bingo and some other things, too, like movies and religious services on Sunday.

All the reviews on this facility appear to be from the Rehab Facility, and none from the "skilled" nursing halls. My mom started off in the Rehab section and the care was acceptable. She has now been on the Nursing floor for 2+ years. It's horrible. The staff is so insufficient, there's no way anyone gets the care needed. Very few of the staff are worthy of being called "skilled", and the few that were qualified have left. The CNA's are not only without a clue, but most simply don't care. Again, the ones worthy of the title "Certified" have left telling me they are both underpaid and overworked. The only reason I keep my mom there is that she's close enough for me to check on her constantly and I don't think moving her would benefit her emotionally due to the change to something new. It's so sad to get to this age in life, pay an enormous price to stay in a place is is for the most part a whitewashed joke. I would not recommend this facility to anyone.

My husband was a resident. The staff was great, physical therapy was wonderful and overall a nice place. The food was not the greatest but home food wouldn't be either cooked healthy and the amounts etc. that they cook there. It is a nice place. Barb

I visited this facility

WORST FAUCILITY staff is friendly , however, many are not trained as they should be. Many did not even know how to do a board transfer from wheelchair to bed. Pretty badly patient is training staff. Food was absolutely horrible. If you don't have someone to bring you food. This is definitely not the place to go. Had 2 decent meals and was at facility over 2 weeks. Strongly not recommended.

My father is currently in a rehab facility at The Oaks at Limestone. This nursing home has been really clean as far as I can tell. I would probably recommend this place. Dad has been there for a couple of months now. They do offer services that you could expect in any nursing home. They have physical therapy, speech therapy, occupational therapy, and medication management. I would say though that whenever I call to talk with somebody there, say their physical therapists, the medication people, or the director, they all spend the time on the phone with me. Their physical therapists, I feel, actually really have their hearts into what they do. As a whole, they really are trying to help. Regarding accommodations, he has enough space in his room even though he shares it with another person. However, he hates the food they serve for him there. It's scrambled egg everyday and they're cold. So food wise, he hasn't said that he liked anything but then again, he is on mechanical diet, which is no fun for anybody.

My mother was in The Oaks at Limestone for a rehab. I admired their staff and the cleanliness of the place. Also the facility looks nice. The best thing about this facility is the staff. Everybody is very nice and informative. They are well dressed and they are willing to help you with your questions. The rooms are nice and well-decorated. It is a lot like home. I wasn't there all the time so I cannot say what type of activities they had there, but I did like the way they offered mother to do stuff. If I have to put her in a nursing home, that would be the one that I would suggest.
