I am a friend or relative of a current/past resident

Pretty from the outside but looks are deceiving. Staff is very inattentive. My mother has waited up to an hour for someone to respond to calls for help. Food is horrible, and trays are often incomplete. Therapy used to be good but has gone down hill. Administrators are less than honest. Unless you have time to sit with your loved one all day I would not go to this facility.

I am/was a resident of this facility

I was transferred to this facility from Texas Health hospital.i need care of my zipp line. I was there 20+ hours. I was left in a from and no One even came in. I learned later they had a fire. I was never told of this...i was totally ignored. Lunch came and went...so did dinner. No food. I rang and rang the call button both by my bed and in the bathroom. Finally someone came in and was totally rude. Came back and all but threw the tray down...Worst good I had ever seen. 2 smashed cold grilled cheese sandwiches that looked like they had been sat on....and half cup of tomato soup looked like it was used. No one ever answered the call light. I was awake all night and no one ever came in. No one checked my zipp line or the shoulder I had surgery on 4 days previous.the nurse aides do rn duties....my husband came to get me...oh and the sheets did not smell clean and bed bugs were found. Took photos to show them and killed 2 that I kept. This facility is the most deplorable place ever. They haven't heard the end of this !!!!!

My mother is currently there for rehab following surgery. She's only.been there 3 days and my family and I are working on getting her discharged. The care has been inadequate. The weekend nurses have been more caring and attentive than the ones who are there during the week. There are so many examples I could give right now but not enough space. I was warned by others about this place and we should have listened. To sum it up- horrible food, lazy nurses. We bring bottled water, gripper socks, and ice packs from home because that's the only way my mom will get those items. Just so unhappy with the whole experience and atmosphere.

Mom is at Fort Worth Center of Rehabilitation. The place is great and clean, and the environment inside is just really nice. The staff seems to be OK so far. They offer activities for their residents, and Mom played cards one time. The food looks really good, from what I've seen. This was highly recommended to us, and it is close to a hospital; that’s why we chose it for Mom.

Only a few nurses know their job and will do it the best they can....they are ALWAYS short of workers.My mom was there..she passed away...ALWAYS had to ask for water thats when we knew she wasn't being offered water she was so thirsty...She was not capable of asking for It herself..The place was not clean...we would tidy up mom's room...to make sure she got a nap and didn't sit up ALL day my family would take turns being there when it was time for her to lay down otherwise they would let her sit in a wheelchair from morning to bedtime..They were not a compassionate staff as a whole....Only one weekend nurse really CARED for the patients., It all comes down from the lack of integrity of the administration....They could care less if they were understaff or had incapable workers....When ask the nurse or aid a medical question about mom...they seems ask if they don't know and will not try to look up her record on the computer to fine out....Why did we stay....the facility was in a convenient area for the family so we chose to be there everyday to make sure she was getting the care that she needed. We rotated our time with family and friends making sure that Mom would be okay. We knewwith our presence there daily they would have to do what they needed to do and what they were supposed to do as a medical staff.Family members need to be with their love ones when in nursing homes to make sure they get the proper care...we saw so many who were left alone without a voice to protect them.....we participated in care plans and activities....My mom knew we loved her and they knew it.

Had my mother here for a week and she left this facility in far worse condition than when she entered. The building is new and modern, You think you are checking into a expensive hotel. The first floor is impressive. The Physical Therapy Center is well staffed and equipped. Thats about where the GOOD ends. The nursing care here is very LACKING. Long waits for call light responses, not unusual to see multi call lights on at the same time. Erratic delivery of meds./ skipped meds. and in one case that I saw with my own eyes a patient being given the WRONG IV meds, Food is VERY lacking.. (what little the patients get. ( dinner on our last night there was a tiny cup of soup and a room temperture grilled cheese sandwich), a light lunch at best. STAFFING seems to be the biggest issue. Saw what I would consider a very large number of 911 calls, and patients being removed via ambulance. Go looking for your nurse? he/she is on break be back in 30-45 mins.. Need for assistance during meal time ?? good luck, almost everyone on the floor is used to serve meals. Weekends ?? less staff and not a RN in sight all LVN's I will say that for the most part the people that work there try, just not enough of them. Simply put I would NEVER recommend this facility to ANYONE.

My husband is currently in this facility for short term rehabilitation. It took 12 hours for the pharmacy vendor to supply the facility his prescribed medications. The food is inedible. He was told they are on a limited budget, and that is all they could o. He has not had a single vegetable in the last 48 hours. It is worse than prison food. Last night he was denied pain medication by one person who stated he had all the pain medication he could have for the day. The worker argued with him. He is not confused and he knows what is going on.

Our family had a very negative experience here, including neglect bordering on abuse. When we finally relocated our father to a nursing home, he flourished. There was a perceived hostility interacting with the staff. We were so glad when he moved to a nursing home. My family would join me in giving this facility a firm "NO!" when considering this facility.

I checked with my lawyer and he said it is not libel if it is true. This is true. My wife came here the day after surgery for wound care. It took 24 hours after admission before all my wife's medications were available, including her pain medicines. I had to medicate her from the medicines we brought with us. I once found her bandages soaked - they had not been changed for 2 1/2 days despite a high infection risk and a doctor's order that they be changed daily. One Saturday evening, they ran out of her primary pain medicine. Ran out! When I went to get our own meds out of her shoulder bag, they had been stolen. It then took them 24 hours to get the medicine to the hospital and get her a dose because of their "protocols", per the hospital director. Common RX available at any neighborhood pharmacy!!! The Chief Nurse later told me running out was the fault of the wholesale pharmacy that serves their account, even though her people gave the last remaining pills and took no action! The very next day, my wife again did not get this very same pain medicine and was given a substitute because the nurse "couldn't find it". (They were at the other end of the hall in the wrong medicine cart.) They served fish to a patient on strong medicine that had bones in it. I provided all her meals from outside after that. Another patient's medical files were left on a table in the family room on a Friday afternoon, laid there all day Saturday and Sunday, and were not removed until mid-morning Monday. When I took that info to the hospital director Monday afternoon, he said that was an isolated incident and he would check on it. I told him if he wanted he could go right now and find unattended medical records on a table in the communal eating area, another isolated incident I'm sure. I have photo's to prove this. Why did we stay? The wound care nurse and the PT people are top notch. They get one star only because of this. We also stayed because I was able to be present most of the day, everyday. Do not leave anyone you love in this facility unattended.

I am a friend or relative of resident

The RN's for the Nursing Home are very nice and do not do what they say they will do for extra care. Both shifts didn't call the doctor when I requested but the weekend LVN did!! This was not an emergency case. However, during a qtrly review I requested this help from ALL in the meeting and Not one of them followed through on this request from the Director of nursing to the Administrator to the social worker. Not Good!
