I visited this facility

Evans Health Care was very clean and very welcoming. The staff at the reception was very, very nice and very friendly. I was telling her what I was there for, what I was looking for, and to see what availability they had. She went and got her director and he was knowledgeable. They just couldn't accommodate me, but he gave me a list of all the places to try. He was very helpful. As far as the entrance, it was very clean. A lot of the residents were active, in and out. There were some residents walking, and some were in wheelchairs. They were very nice to them.

My mother-in-law moved into Evans Health Care. We liked the professionalism that was found in the staff. It was a women's-only unit, which was attractive to us, and the location is close to us. It's a nursing home, and they have a memory care unit that's a little bit separate from the others. The head nurse in that unit is trained in memory care but is also extremely compassionate. The food is brought into the unit. They wash clothes. They don't have a gym or a salon, but they have physical therapy. It's fairly near a hospital within a mile and a half or so, and restaurants are around there.

Please seek another facility!!! Do not bring a relative or friend here whom requires medical attention. My mom was bought here to regain her strength after a hospital stay. The second day there they did not yet given my mom the meds she needed. I called the facility on Saturday to speak with a medical person in charge and i was told that the only person available in charge was a maintenance man. From there it got worse for two weeks, my mom told me nobody checked her bp or oxygen and she begged for water. On a Tuesday morning i called her and he answered and stated that she was not well for 2 days and today they told her she has pneumonia. i went right to the facility at about 945am and was told by the patient next to her that she was on the floor the whole prior night sick and nobody helped her, i asked the nurse her condition and was told she has it in both lungs and was getting her first pill at 400pm that day!! 2 days after she got sick. i asked if they could send her to the ER and was told NO shell be ok and theres nothing a hospital could no that they couldnt which i knew was not true, i asked 2 more times and they would not send her out and she looked bad couldnt even talk they told me oh thats normal, Well 10 hours later i got a call she died. Then the nerve of the person calling me says 2 minutes after i find out well call a funeral home now so you can get her out of here REALLY!!!!! I asked for an investigation and the administrator wants to say oh it wasnt the pneumonia it was her heart. My mom deserved better and so does yours!! STAY AWAY

My mother is in Evans Health Care. It's been okay so far. It's been fine. She has no complaints about the food. She seemed to be okay in her room. She received therapy when she first came in, but she's not having it right now, and I felt that she should have it longer. They don't have a lot of activities going, and that's one of my main concerns, she doesn't have enough to keep her occupied. They have an outside courtyard, but she can only go out there if I'm here to visit.

My husband had a stroke and it was recommended by the doctors that he be put in a nursing home for a short period of time for help with rehab. I was told to pick out 5 nursing homes in the area. This was the last choice and the only one available for him. He stayed there 20 days and it got to the point where I feared for his life. Because of covid, we did not visit him but trusted that he would be treated well. WHAT A MISTAKE! I would not send my worst enemy there. I have no idea what the establishment looks like on the inside but I can tell you that if you care and love anyone you will NOT send them here. My husband because of his stroke could not speak very well or hold a phone so even though there was a phone in his room it was next to impossible to talk to him. The few times he did answer the phone, all I could get out of him was him screaming "GET ME OUT OF HERE". I will never know exactly what went on in there but I don't believe it was pleasant. First of all, they don't like to answer the phone. The "receptionist" is only there from 830-530 Monday through Friday and trust me, she answers the phone when she feels like it. The receptionist is rude, curt, condescending and doesn't like to give messages. I would call twice a day to check on my husband and ask to speak to someone and in 20 days I had people call me back twice. After 530 at night, NO ONE answers the phone! There is no answering service, no way to leave a message. The phone just rings and rings and rings. I got someone to admit that "the staff at night doesn't know how to answer the phone...we are working on it." REALLY????? So forget about it if you were like me and couldn't speak to your loved one yourself. You have NO IDEA WHAT IS GOING ON. The case worker Enoele, doesn't like to talk to you or return calls. You can't even leave her a voice mail, it just rings and rings. When you do actually get to talk to her, she is very dismissive and curt bordering on rude. The one nursing supervisor (I guess that's what she is) was very nice and helpful but I only got to speak to her one time and that was after having my insurance company call with me on the line and demand that someone talk to me. IF you are lucky to get a nurse to ask about a patient, they barely speak English and are very vague. You won't learn anything from them. I was lucky enough to twice speak to a speech therapist, Lauren. She seems like a very caring lady and I believe she truly helped my husband. She was very informative and even sent me emails with information about how to help my husband when he came home. The occupational therapist was also kind enough to speak to me once and was also helpful. I would say that the rehab care part is good, but this is not a place that you want to put your loved one. Unless you can go visit every day or your loved one is cognizant to tell you what is going on, you are going to be left in the dark. I had to have my husband's primary care physician step in twice to get information, and once my insurance company...JUST TRYING TO FIND OUT HOW MY HUSBAND WAS!!!!! His stay was 20 days, he came home so thin it was unbelieveable. He told me that they didn't feed him. He was starving all the time. He looked it too. He did come home clean, and shaven but he looked very gaunt. He must have lost at least 20 lbs in 3 weeks. I truly believe if he had stayed longer, he would have died. Scared??? YOU SHOULD BE. DON'T EVER PUT SOMEONE HERE. I will be reporting them to the state.

When I first looked at Evans the reviews were horrible. They got hit pretty hard by coronavirus and after that happened they cleaned house. My mom was in there with the new staff and she loved it. I don't know what the food was like and I was unable to visit durring quarenteen. I expect it's about the same as any other nursing home. But mom loved it and enjoyed her time there. I was really impressed. It's hard to get a good nursing home on Social Security and I don't know what this place is going to be like down the road but right now it's a good choice.

Had a dementia client die 4 hrs after being sent by ambulance to ER. Two days earlier was told she was fine. ER lab test showed COVID-19. This was 4 days ago. I have yet to hear from facility and my calls/messages go unanswered/not returned. Don’t send anyone you care about here. I am calling the State DOH next.

This review is very recent and things continue to go downhill. The cockroaches are running up & down the walls even when I left my night on all night! Therapists are good. Food is poor but hot. Nurses are wonderful but cannot make up for some of the CNA's. Some CNA's have been there for years and can be quite rude and the newer ones follow what they say. There are a few that smile and try to make up for the rest! The call lights were answered within a reasonable time. Several activities are available. A big problem, like most places is staffing. No problem with parking spaces and the lot is well lit. Overall, they need to "clean house" (literally) and hire new CNA's and this place would be great. Social worker was super overworked and little help to my discharge! I was transfered there from the hospital, I should have checked out the reviews here. Because of the location (which is great) I chose Evans. There are 3 wings, short term, long term and dementia. People that wrote and said it was great must have been in the locked dementia wing or lond term. I am commenting on short term only. Each room has a private bathroom (filthy) and you go down the hall to get a shower (hot).

I visited this facility

I visited a family member there and I’ve seen dog pounds cleaner than this place.House keeping and cleaning staff should all be fired and replaced.l actually took photos of the floor,walls and the bed frame.Absolutly filthy and unacceptable .Someone must be getting paid off.

The rooms were not clean and they do have problems with cockroaches. Many of the staff were nice and helpful but there were a few who have attitudes and barely do anything. The nurses were great, just not enough of them. The food was below average but it was hot. There were activities but because of therapy, most activities were attended by long term residents. The social workers were helpful and pleasant. The therapy during the week was good but minimal on weekends, which was ok because we needed a break. The phone system was poor. Sometimes we could not call out or family could not call in.

Dad went to Evans Health Care from the hospital, and it was terrible. They weren't keeping his room clean, mom had kept track of some stains on the floor that hadn't been cleaned up, it was not cleaned for more than a week, and she told somebody at the nurses' station. They actually told my mom to go find the custodian to make him clean the room. They never said they were sorry and that should not have happened. They regularly take a long time to get to him when he pulls his call button, and it took an hour and 10 mins for them to get his medicine to him after he asked for it. He was practically in tears when they gave it to him. My dad told me it happens pretty much every night. There is a woman who brings 2 of her dogs with her, and you hear dogs barking in that whole section of the building. A lot of times those dogs are running up and down the hall, it's like a zoo, and it's just chaos. You wonder how they haven't been shut down until now. It's one of the cheaper places to keep him, because we pay out of pocket, but do not put your love one here.

This is a great place. My Mom just died. For over a year in the Dementia wing, she was given the best care. She was a very difficult patient but the nurses said no problem. The place is always imaculate. We live far away and have had multiple unannounced visits. Everything was just as it should be,when we arrived. My aunt pays $14,500 a month at her nursing home. The room is a mess. Staffing is no different on a per patient basis. I don't understand the motive of the people criticizing this place, but it is either based on a perceived single slight, or they are simply dishonest.

This place is terrible. I was there for rehab from hip surgery very recently and found conditions to be very bad. The food is barely edible and it was the same thing every morning. Press the call button and you may well wait a very long time for a response from staff. It averaged between 5 minutes and 30 minutes on more on several occasions which is really unacceptable. Had I been suffering a medical emergency, I would have died waiting. I had physical therapy but it was not intensive nor long enough. Averaged less than 30 minutes a day and the same therapists attended to 3 or 4 others at the same time. Therapists spent too much time looking at their cell phones and IPads while I waited. As soon as I could put one foot in front of the other, they pushed me out the door. I am home now but I still can't walk except to go to the bathroom and kitchen of my apartment. I can't walk well enough to leave this building so now I am stuck indoors. Am looking at Page for the therapy I didn't get here. Their therapists are wonderful.

Evans is a nightmare! It is a filthy, roach infested, understaffed, horrific place. The majority of the nursing staff was kind and responsive with the exception of a few, but they are grossly understaffed and the nurses are stretched thin. THE PT STAFF IS FANTASTIC! I documented everything, roaches, clogged toilets with feces floating in it, 3 hour wait time for my roommate to have someone respond to her. All of it. The food is deplorable. If it wasn't for the kindness of my neighbor and visitors bringing me food, there were days I don't know what I would have eaten. They are not 5 star rated, they have dropped to 4 and on their way to 3. Please do not put anyone you love or care about in this roach infested, filthy holding pen.

I visited this facility

It looked like Evans Home Health Care was clean. There were people in the hallways screaming and people curled up in their wheelchair. It wasn't a good environment. We couldn't wait to get out of there.
