I am a friend or relative of a current/past resident

My father was left naked and in soiled sheets for hours. They did zero PT for him because he had Covid. He developed a blood clot and his foot started turning purple. I brought it to his nurse’s attention, but she assured me that when he did PT it got better. Well, a few days past and we had to take him to the ER and his leg had to be amputated as his foot died due to lack of circulation. We feel this facility neglected my father to a point where his life was in danger. Stay away from this awful location!

I visit a friend at Hollymeade regularly. The reception desk is supposed to remain open until 8:00 p.m., but often it closes before 8:00 and there is no access from outside. Last night I was there at 7:50 p.m., the receptionist was gone, the door was locked, and no one would answer the phone to let me in, even though I had pulled on the door and the alarm was set off. Recently when I was visiting my friend turned on her signal light to get help because she needed a sanitary change a little after 8:00 p.m. Twice during the next 2 hours I went to find an orderly to them to help her, and both times I was told someone would come "soon". At 10:00 p.m. I went to the central nurse station where there were 6 nurses, not orderlies, standing around laughing and joking. I again asked for help, but was told the shifts were changing. Around this time I watched 3 orderlies, including those I had spoken to earlier, leave. About half an hour later a kind orderly came to help my friend, who had been lying in excrement for the past 2 and a half hours since she originally signaled for assistance.

Please keep your loved ones away from this place if you have any love for them. I was admitted here and only last a day! The treatment is inhumane. The staff some of them are rude and nasty,,arrogant and unskilled! The first person you greet at the door can’t be bothered to greet anyone! This place has no business accepting patients until they meet state standards! Please stay away! And families who have loved ones there, please check on them!

Transported by Hollymead required transport. Dumped in the bed no one showed up to greet or advise. Left for hours without management or nursing staff visit, Phone to desk unanswered, call light likewise ignored. Entire phone system blocked when I tried to contact outside family. When challenged on our departure we were advised we were leaving against physician's advise. (they clamed a staff physician is on site) when administrator tried to find him or her were were advised they could not find the staff physician. when arrived there was a (cold) breakfast plate waiting for me (arrived after lunch) with all foods counter to my (very private) physicians' directions. We were told that using the facility's transport we would not have a copay or and expense. Recent mail puts the lie to that claim. Billing on the order of $2K for a few miles. (Seems to be an inside deal) Simplest advice DON'T GO ANYWHERE NEAR THIS FACILITY!

This facility is understaffed and the staff is either the unqualified to perform their duties or just don’t care. They ignore the patients needs. Patients are left in their beds with wet sheets, wet clothes. Bathrooms are left dirty. Phones are not answered. There is a lack of communication from anyone in charge. Quality of care is a huge issue. Do not put your loved ones in this facility. They will be placed in a room and basically ignored. Expect your loved one to decline or suffer from neglect.

Buyer Beware – Don’t be fooled by pretty picture We placed our mom in here with the high hopes she would be well cared for. The admissions department says all the right things when you are blinded by the emotions of an ill parent. We were overwhelmed how our mother took a real turn for the worse, and the hospital was basically throwing us out and had less than 24 hours to find a place to bring her. Hollymead provided us with a very manicured room that she would move into (Our mothers room looked nothing like what they showed us), We were warned upfront about lack of staff and that proved to be a very sad true statement, but at no point did we think our mom would be ignored. Twice she got herself into a pickle and stayed in the same position until we called the facility. (We had a camera in her room – thank God we had the sense to do that, or she would have possibly been in a very precarious position for hours) There is no after-hours phone number to call in case of a situation like this – its hit or miss if you will reach anyone. They left her in the same clothes for days and it wasn’t until a family member changed her garments. The PT and CNAs were for the most part nice, but PT should have done bed exercises with her when she refused to get up. Not just walk away and give up on her. One night she was very confused and the care worker walked in and all she wanted was to talk to someone and she walked out in a huff and sarcastically told our mother – there is no one in the room with you. If you are going to care for the elderly or sick people, please have some compassion. The food was unappealing and not fit for anyone. Looks like mush on a plate. Hollymead is a total money grab and has one thing in mind – make money. Our mother is now in a real facility that cares about her dignity and care. They make her feel like a person not a thing. They place a tray of food in front of her that looks like something you want to eat, change her clothes every morning and into her nightgown at night. This is what Hollymead should provide for their overpriced care.

I would not recommend this facility for anyone. The staff was either unresponsive or condescending. They sent my frail, elderly mother to a doctor’s appt in nothing but a hospital gown and socks. They didn’t even brush her hair. It was a totally degrading experience for my mom. We finally got her out of there and into a caring facility. Don’t rely on Medicare ratings alone. Talk to people in the area. We’ve since heard from several professionals that Hollymead consistently gets complaints.

I visited this facility

My husband has lived at Hollymead since DEC 2020 and I have found the staff all very nice and helpful. They take great care of him they really seem to care for him. I have gotten to know quite a few of the staff, aids and nurses and very are very nice. It was a rough year for everyone with COVID but they kept me well informed on what was going on with him. I get very good and fast responses to any questions. It took some getting used to the food but I understand it has to be bland for their diets. He keeps gaining weight so cannot be all that bad, he used to be a very picky eater. I thank them all very much to care for my loved one as I was unable to. Yes I would recommend Hollymead. I used to able to talk to residents and they all seemed to like it also. Of course there will always be those disgruntled but no one wants to be there in a home but they would not be if they could take care of themselves. I had my husband at home for 18 months taking care of him until I just could not physically do it anymore, it is not an easy job at all. They are angels to me. The rooms are good sized with fridge, microwave, TV and even a recliner. I do see actvities going on but he is bed ridden so does not participate.

I made the mistake of not researching enough. My 91 year old mother had a stroke and was released from Denton Medical City on November 10th. From the moment my mother arrived, her care was beyond awful. as covid rules only allow sparse visits, I visited on November 11th. I arrived at 1130 and her breakfast was in a container sitting across the room. My mother is paralyzed on one side, blind in one eye, and cannot speak due to the stroke. I was there when lunch was served and she was starving. her medicines were not crushed by the nurse and my mother choked on them, she was also given clear instead of thickened liquid. I installed a RING camera and witnessed her slumped over for hours, almost falling on the floor, ignored for hours, and her hygiene needs were pretty much ignored. The PT and OT worked with my mother, but the speech therapist did not even have a consult until the was there for 1 week. The speech is the most important for a stroke as her swallowing and voice are of supreme importance. There is zero communication between staff, management and family. Schedules are only promised verbally, there is no recording of bathing, changing or any other need besides food/medicine and PT. Please do not put your loved one here.

If I could give them zero stars, I would. My 89yo dad was here for rehab in January/February 2020. He had Parkinson's and a brain bleed from a fall, and either I or an independent paid caregiver was with him at all times, 24/7. I was with him 4-5 hours every morning, so I was there a lot. We had been told the nursing was bad but the PT was good, so we took a chance. the PT was good...the nursing was horrific and dangerous. It took three days to get his medication straight when he transferred in...the receiving nurse forgot to add one and then they forgot to order 2 other meds. I am the one who caught it. They consistently ran out of clean bedding...I finally had to raise a ruckus at the front desk to get some...but the other patients were SOL. I nearly always just went into the linen closet myself to get clean bedding if it was there. They would also skip shower days for him. If he was in PT when they came to do him, they'd just skip him. Horrible. They didn't always administer all of his meds...they left his inhaler in his room unsupervised twice...and my caregiver reported that one time a nurse tried to force Dad to take all his pills at once with his pudding rather than one at a time, even though he was on puree food/swallowing restrictions. He choked them back up. So dangerous!!!!! The director of nursing always talked a good game, but nothing ever improved. They consistently tried to move him without a gait belt and both times when he had one of his seizures while an aide was in the room, the aides had no idea what was going on or what to do. Again, dangerous! This place was a horror show. His 1st Dr., (Dr. Higginbotham) only came late at night and never called me even though my phone number was everywhere in the room with signs asking him to call me 24/7 and instructions to my caregivers to tell him. He never made any chart notations after the initial one (my Dr. brother was HORRIFIED) and waited days to order a urine analysis when Dad had a UTI. He was impossible to get hold of. I finally got them to switch me to Dr. Munis who was MUCH better and more available. They consistently brought him the wrong diet and had no good option for a ground diet. They would grind the meat, but give whole veggies and forbidden foods, like beans. We had to BEG for extra gravy to help him swallow when he wasn't on puree. My caregivers and I were there to cut it all up and keep him away from beans, peas, corn, etc. but on his own, he could not have managed. The staff had terrible attitudes and would stand around the hallways complaining about mgmt, their hrs, and how many patients they had. They'd keep chatting even with room lights lit. Even emergency bathroom pull cords. My dad had a seizure in the bathroom and I pulled the emergency cord and it took 10 minutes of me literally screaming for help to get anyone. The owners/bigwigs toured while a couple of times and my dad had a seizure during one tour. there must have been ten people in the room helping us, including the director of nursing, but any other time he seized, I could barely get a nurse to show up. His new chair was in the hallway for a few minutes one day while we were in the room and the housekeeping mgr just took off with it without even knocking and asking. she dumped our $300 roho cushion and his shirt onto the floor outside our door. I had to chase down his new chair they had just ordered for him to take home. She was anxious to clear the hallway because the bigwigs were coming. Those people are the only ones the mgmt there cares about. The aides consistently took 30 min to and hour to answer call lights and more than once my paid caregivers reported rough handling of my dad by the aides. Two of my caregivers, separately and unprovoked by me, told me this was the worst place they had every worked in, by far. My caregivers and I kept a spiral notebook record 24/7 from 1/1/20 to 2/14/20 of all of this. It reads like a horror show. DO NOT SEND ANYONE YOU LOVE HERE.

The Hollymead experience was a nightmare to say the least. The facility is nice but do not let that fact fool you. In general the limited and inadequate staff is horrible. My 92 year old father was there after a hospital stay for blood clots but Never again would I go there or suggest anyone to go there. Horrible, unclean, unskilled, understaffed. Think One Flew Over The Cuckoos Nest.

It has taken me 4 months to write this review and I am sorry for anyone who has placed a loved one in this facility. My husband was at Holly Mead for 4 days for rehab after a hospital stay. His care was non existent. The staff disrespectful. It is too painful for me to go in to details but I will give you a few example. My husband was never alone either me or my son was with him 24 hours a day for the 4 days he was there. The night before my husband was rushed to the hospital I went to get his nurse Mai (sp) since his light had been on for 20 minutes. My husband had been diagnosed with pneumonia that day and he was in pain from blood clots he had been in the hospital for. I asked her to come to his room and give him some medication. She informed me she was on her break and had another 20 minutes left on her break and she would get to him after her break. I said he needed something now. She grabbed her food and turned out walked away without answering me. When I approached to tell her that was unacceptable one of the CNAs (Ray or Roy) who was not caring for my husband informed me that they did not have to even talk to me when they are on break.. I called the Director of Nursing who was polite and assured me she would address the matter in the morning and called the desk that night. While meeting with Director Of Nursing on Monday we were interrupted by the PA saying she was having my husband sent to the hospital so we never finished our meeting. My husband passed 3 days later (December 26th). Beside the horrific treatment by staff other issues were – his light was never answered in 10 minutes it usually took anywhere from 30 – 60 minutes and usually we had to go to the nurses station. His diabetic medication was not given even though it was due the day we got there. I addressed this issue at least once a day with no results. There many other issues but the treatment by the staff was unforgivable and I’m writing this through tears so I will have to stop now before I get bitter. But PLEASE consider any option but Holly Mead if possible for your loved one.

My husband was a patient in this facility, some of the staff go the extra mile but some are disrespectful. They snoop around your room when you are not there violating your privacy right and removing things without your knowledge. They are understaff and care is not rendered to patient on time, medications not giving on time, when you press the call button/bell it takes hours before anyone will come to your aid. You really need to stay with your family to help out if not care will not be administered. This is a terrible place to send your loved ones for rehab treatment if you will not be staying with them since staff are not available .

Hollymead is a good facility.my friend enjoyed her stayed in this facily.very clean and neat.My friend was welcome by a unit manager max,he did a great job by explaining everything in a nice way,at the same time assisting my friend with his needs.such a wonderful nurse,respectful,polite and very considerate of other people,flexible,attention to detail and demonstrate a lot of solving skills. I will recommend hollymead to anyone looking for a good facility for his/her love ones.Great staffs from therapist,administration to nurses.Rooms very clean,hallways and doorways litted and clear of obstacles.

Let me be clear; everyone was nice. However the care my mother received I believe contributed to her complications. She went there for a fall/fracture hip. She had dementia, I wish now in hind sight I would have gotten a sitter with her during the day. I found her wet on more than one occasion, found her crying for help due to pain, found her in the bed most of the time in her clothes and she developed a pressure wound stage 4 which was extremely painful. My 85 year old mother passed away during her stay at Hollymead

This is by far the WORST facility in the greater DFW area. My mother was admitted after a fall and shoulder replacement. They nurses barely stopped by to take care of her. The room was full of trash consistently. Then to top it off, they left her in the shower alone for over an hour. She was removed immediately after she was sent BACK to the hospital due to the lack of care. Then they have the nerve to send me a bill for a "bed fee." If you want your parents or loved one to receive decent care, STAY AWAY from this place.

Hollymead is an excellent facility.The staffs are so nice.700 hall is a very good hall.Nurse max is such a caring and nice guys.my grandma told me he was the best.my grandma enjoyed her stayed in this facility.my grandma was treated with respect and dignity.staffs are very friendly.physical therapist and occupational therapist are also a nice team.when we got discharged this nurse max assisted us to the car with our belongs and helped transferred my grandma into the car so nice of him.i strongly recommend hollymead to anyone looking for a place for their love ones.Good facility and good services.

This facility is a good place to be.my dad enjoy the care there.my dad was on 700 hall.This little nurse name(MAX) is so awesome.max leaves the values of loyalty,honor,respect,integrity ,very caring and treat residents with dignity.when he is around I am very confirm my dad will have everything done right.every residents talk about max.he dances on the hall and makes them laugh.always look for a solution when there is a problem.excellent care at this place.the therapist are also very nice to patients.thumb up to staffs at hollymead.if you are looking for a nice place for your love ones.hollymead is the place for them.

I visited this facility

I have a friend in this facility, and I am distressed at the care (?) she is receiving. They wake her at 3 to 4 a.m. to change sheets on her bed. I have tried to reach her repeatedly today, and they don't even answer their own telephone. This is appalling.

If I could rate Hollymead any lower, I would. When we had our mother transferred to Hollymead of Flower Mound from the hospital for physical therapy, we were told by the Administrator that the staff ran "lean". Little did we know that meant that our mother would be left unattended for basic needs such as going to the potty for hours. The PA/doctor notes were incomplete for up to 12 days, and therefore the doctor group had no clue of her physical status. Friends and family visited her daily, and tried to inform the staff and doctors thru email, phone and personal conversations about her conditions. All to no avail or even the courtesy of a response. They even refused to give her basic medication such as lasix that she had been on for years, never reaching out to her PCP. They insisted on running more tests, instead of doing what was best for her, and proven through years of experience. Unfortunately, due to the lack of proper basic healthcare, she went into congestive heart failure, and they refused to release her back to the hospital, family or hospice care until it was too late for her to be able to recover.

This facility is very nice. My mother was here for a short time. The nurses were very friendly and helpful for us. The admissions team was a great help getting mother situated. The administrator was friendly and made himself available to us and mother for any questions or concerns. She enjoyed the food and said that someone would come by to take her order, so it felt more like room service at a hotel than what you might expect from a nursing home.

My mom was in Hollymead before she passed. The service was very good, and it was very clean and well-maintained. The only problem we had was that we were very disappointed regarding the assistance of the doctors at the end. The resident doctor or the one who was supposed to sign her death certificate, used an abbreviation. It was already a week after mom was in the funeral home, and they didn’t cremate her because of that. They called several times regarding the death certificate, and that to us was stressful and something we will never forget. We had to call Hollymead about four times on the Thursday before the service, which was on a Saturday, telling them that I needed answers to why my mom wasn’t cremated and why the doctor didn’t go there. They said that the doctor was on a personal appointment and that she can go in to take care of that on the next day, Friday. I told them that it needs to be done that day because they needed to cremate her tomorrow, Friday, and on Saturday was the service, which I have people coming from all over the world, and they couldn’t guarantee it. It was very stressful for both daughters who were taking care of their mother and very insensitive of the doctor to say that she couldn’t go because she was at an appointment. She had to do it over the Internet, which was so impersonal, but she took care of it. The other thing is that the doctors they have on staff, unfortunately, are not flexible. They don’t have flexible hours to speak to the family. When we left notes with a phone number, they wouldn’t call. We would leave specific instructions for the doctors to call, and they wouldn’t call. We had to continuously call, and they would say, “Oh no, she was here, but she left/we don’t know when she comes tomorrow.” The administration and the staff, the food, and the ones that took care of her and the nurses were very nice.

My father is in skilled nursing at Hollymead since the middle of September, and it's wonderful. Everybody has been very nice to him. It's very clean and well taken care of. They have outside gardens, a beauty/barber shop, and a little gift shop. They don't have a lot of activities, but they just hired a new activities director. Services include medications, labs, and wound care. The staff has been very nice. It's newer and a lot prettier, and the rooms were a lot nicer.

We already got my mother in at Hollymead. I've been very happy with them. It is a very good facility. We chose it because they're very nicely decorated and very clean, the staff was very friendly, and it just gave me a good feeling that it was more like my mom's home. Hollymead is more expensive, but it is well worth it. The facility is well kept and clean. The staff is really good to the people there. It is just a really nice place. It is almost like a spa instead of an old person's home.

I visited this facility

Hollymead was a new facility. It just opened in September. The facility was great with all the new and fancy paint and carpets. If someone needs good rehab, it is the place to go. The rehab people were excellent. Their price was extremely high. It was very expensive, but it was a great facility. Their food was good. The rooms and apartments were as good as you can expect.
