I am a friend or relative of a current/past resident

The staff at Green Valley went above and beyond to care for my father & his medical needs. His only complaint is the food. I appreciate the time & energy the busy staff give to my father, especially with health care facing staffing shortages on all fronts. My father really enjoys the group physical activity each day.

I am giving 3 stars because the staff are very nice. The facility is clean and has no oder. I am taking away two stars because the food is terrible. I can’t imagine that prison food wouldn’t be better. My husband is frail and can’t get by with throwing away his dinner each night. He needs the nutrients. The other issue is that their response time is very slow. I have had to go and find someone to help. The red button sometimes does not get answered. I’m sure that some of the issues are due to understaffing, hence the 3 star rating.

My dad was at this rehab place for 2 1/2 weeks. We live in Salem and had to call every day about some kind of issue. The front desk phone would ring and ring, not even going to voicemail. This was an almost daily thing when calling. When asked to talk with the nurse was told they would call us back, which never happened. We had to call back. Dad said that the meals was always late and not very tasty. He would push the call light and it would take the nurse forever to see what his needs were. He was cold and asked for some long sleeve shirts and a belt, so we bought those and took to him, dropping them at the front desk. When we called dad several hours later to see if they had taken the shirts to him, they had not. So, we called the front desk and they said they had to take them to their laundry to mark them with his name. They wouldn’t take him extra blankets to help him get warm. When we got him home to Salem, they did not send all his clothes with him. He is missing a pair of jeans and brand-new pants with elastic from Costco and elastic belt. ***NEVER SEND YOUR LOVED ONES OR YOURSELF TO THIS PLACE***

I had a great experience post surgery at Green Valley. All of my needs were met. The food was good. The nurses & CNA"s worked hard to provide comfort & medical care to me. Great activity program & great physical therapists. I highly reccomend this place.

O gave 5 stars for the post surgical rehabilitation side of Green Valley. This is a top notch facility. The Occupational therapy and Physical therapy departments wee to notch. The medical and office staff were all genuine and friendly. Everyone was professional. The side I was on was very well maintained and smelled good. The only department I would rate lower would be the dining. l know it is hard to develop and serve meals for every palate out there so I won't be too harsh. They do allow some other options for picky people like me. When you add it all together they get 5 stars from me. I plan on returning after hip surgery this fall!.

Do not put anyone you care about into Green Valley Rehab Center. The care provided is poor. Scheduled medicine is missed routinely. Rehab sessions cut short or missed. Waiting for meals in dinning area up to an hour. Food on the plate not food ordered and no regard for special diets. Waiting for help in room anywhere from 15 to 60 minuets. Spoke with Administrator, Nurses and onsite Doctor. Seems there is a problem with communication at this Facility. "We were not aware of that" was standard answer. Pretty evident this facility is all about profit.

STAY AWAY!!!!!!!!!!!!!!! I chose this facility in which to receive rehabilitation following an orthopedic surgery in 2017. This is a terrible place from top to bottom. Starting with the very worst incident, when I discharged myself, the facility sent another patient's medication home with me. Had a caregiver in my home not noticed, there could have been dire, perhaps fatal, results. The "med aides" were individuals who passed out medication were, for the most part, incompetent. When I asked one what pills she was giving me, she pointed to her own stomach. The pills were for another part of my anatomy and were never identified. Another "aide" starting crying when I asked her the same question. There was no discernible treatment plan, no notes on the instruction board above my bed, standard rehab protocols were not followed, and never once in eight days were my sheets changed. The CNA staff appeared to be trained in delivering food trays, and toileting with not many skills in between. The food itself was inedible and I survived on cottage cheese, yogurt, and bananas for eight days Don't pay attention to their high scores, they do not represent the reality of the situation..

The social worker suggested our mother go to Green Valley for rehab for a broken pelvis – this is the start of the problem. She was supposed to be picked up at 4:00 pm Thursday, June 1. The driver was over an hour late, when we got to Green Valley, they did not have a room for her and did not seem to know she was coming – one of the workers said “that happens”. This is when we should have run from Green Valley. They finally got a room and checked her in. Should not be surprising, but the children work, so came after work. Mom was in quite a bit of pain, and the worker said she did not ask for pain meds – for crying out loud, she is not very vocal with her dementia, and a real skilled nurse would have been able to determine her level of pain despite dementia. When her daughter talked to the nurse, she though it was Lois’ shoulder that was the problem – if she would have taken the time to read her chart, she would have seen that mom was there for a fractured pelvis! Then come to find out she did not eat lunch, and they said if she doesn’t eat they would just wheel her out to the dining room. If she cannot feed herself in the room, how in the heck is she supposed to feed herself in the dining room? If Green Valley is not capable of providing skilled nursing to a patient with dementia and broken pelvis, then someone needs to let peace the State of Oregon and family of patients looking at Green Valley for help in their time of need. So either mom’s inept care is an outlier, or it is a trend. We were told Green Valley was the jewel of the Avalon system. If this is the jewel, I wonder how bad the care is at the other Avalon facilities.

I visited this facility

Food was mediocre and they only keep sugar free items in community refrigerator, when they should have regular food for those who are not diabetic and do not want to ingest the poisonous sugar free sweeteners. Calls lights are not answer on a timely basis - could have more staff on each shift and especially during meals when residents still need toileting. Even though they have a call light system, they come in and turn off your call light and tell you that they are in the middle of something and will come back and a lot of times do not come back and the call light system logs it as being answered in a timely manner. They should not be able to turn off those call lights until they actually help the resident with whatever reason they initially called for. Have a hard time with staff not listening to family concerns. Sends residents home with UTI's.

I visited this facility

Green Valley looked very clean, and their director of nursing seemed very much on top of things. The rooms were large, clean, and the facility was fairly new. I liked what the director had to say with the information she gave me; it sounded good. I also liked the setup of the rooms. They were large, clean, and very handicapped accessible for wheelchair use. They also had lifts for getting in, and out of the bed in the rooms.

I was a patient at Green Valley following my release from a hospital's Intensive Care Unit,. When I was admitted, I was unable to stand, walk or even speak coherently. Within 24 hours, I was up with assistance and beginning to feel stronger and more confident. The physical and occupational staff are outstanding - and effective. I had a single room with a lovely view of the landscape. Nursing staff were generally attentive, and some staff were excellent. The Center has a system to ensure a timely staff responses to patient calls and it seems to work. Fresh fruit and cereal are available 24 hours. Three meals are served with choices for each meal on a weekly menu. I did not present as a willing, optimistic patient. The hospitalist had informed me that I am terminally ill and hospitals are for people who are expected to recover. She relegated me to a place where I could expect to die. Green Valley's administrative dealt with the complexity of this very emotional issue admirably.

I visited this facility

I would say that compared to all the other ones, Green Valley has the nicest feel about it. So I mean if I had to give somebody a five, I would probably give them a five and everybody else a four just to give them something better. Everybody was great, but I want them to be a little better in terms of being aesthetically pleasing.
