My family member had the best care from the staff and administrative staff. The place feelings like we were at home. No better place. Recommend to all my family.

The facilities have the feel of a 50’s bunker. The staff members slide by with doing as little as possible and the management does not resolve issues. They give assurances, and seem to be sympathetic, only for nothing to be done. Medical necessities have been being ignored and certain issues are being exasperated because of this. Patients are left sitting in wheelchairs in the hallways. Their room alarms go off multiple times and some patients are even crying out for Help, only to go unanswered. The excuse of the nurses, that they can’t hear the alarms down the hallways from the nurses station.

I had mom at Signature HealthCARE of Clarksville. She had just recently got out from rehab. They were pretty good, except for not being able to see her. They let me know when she fell. She fell twice there. The first time, they left a message. The second time, I had visited and they told me that she fell that day just before we got there. She didn't really have any complaints about the care that she received. I told her that if she called for help, they're not going to come immediately because they could be in another room. There's only so many of them. I think they went through staffing issues because of Covid. They were short staffed, so she probably sat more than she should have. She didn't get walked, but she also can refuse; she has that right, so that could've been an issue too. I can't fault them on that. You can't force them to do something that they don't want to do. I know she needed assistance getting to the bathroom, and there wasn't anybody there. I was there, so I assisted her to the bathroom. They do come pretty quick if that emergency call button gets pushed, so that's good. She pulled it when she was in the bathroom twice, and they came running. It tells me that they are paying attention to that kind of stuff. I would be concerned if that happened and nobody came, but that wasn't the case. They all didn't want to see her go. They wanted her to stay. They had bingo, and they had a musical group come in. My mother mentioned they had some pretty good stuff going on, so that was nice for her. She pretty much has a good appetite, and she ate everything.

I don't often leave theses types of reviews, but I was compelled to come share my experience with others. SHC of Clarksville was so great for my husband . He has had a plethora of injuries that you wouldn't believe. The therapy team was so wonderful at helping her recover from all of his injuries sustained in the accident. When she was struck by lighting we thought there was no chance he'd get back to normal. He has never been in a facility before and we were worried about him not being comfortable during his rehab, but the staff treated him so well and was very kind through the whole process. Brandi the administartor was great at taking care of all the concerns I had and really eased our minds. The food at the facility was very great and we always could get something that we wanted, so much better than hospital food! If you live in clarksville or anywhere in TN I would recommend staying here if you want to rehab from an injury as the rehab department was the best part. We'd give it 10 stars if possible.

My mom was at Signature HealthCare of Clarksville for rehab. That's the one we were closest to, so it was nice being able to just go over there in five minutes as opposed to going across town. The staff was super nice. Everybody seemed to be right on, as far as getting to her when she needed something. There was one time she passed out and they were right on top of it. They were on top of what she needed most of the time. They were very responsive. The place was a little on the shabby side but it seemed to be very clean.

I visited this facility

Signature HealthCARE of Clarksville is a good community. Their staff seemed to be well-trained. Some of their amenities are a dining hall and physical therapy. It looks like a nice community, it's clean and well-maintained. The main reason that drew us to the other place instead of this one was that the other one was brand new, and the cost.

My father is still in rehab at Signature Healthcare Of Clarksville. I'm very pleased with the upkeep of the facility. It's nice, nicely decorated, and not rundown. The rehab staff has been incredible in working with my father. I cannot say that I've had one bad experience in talking with the rehab people or even working with the rehab people with my father. I would highly recommend it. I personally feel it's the best we have in Clarksville. I've had such positive experience in dealing with them. The food leaves a bit to be desired, but it's not as bad as prison food. It's based on the American Diabetes Association diet. I've seen what he eats, and it's not bad at all. He can only get out of the wheelchair and lay on the bed. They have a room where people have rehab and a room where they can go watch TV, do bingo, and eat popcorn. They have postings of what they're going to do for the day. I have seen other people participate and they seem to enjoy it. The food is prepared in the cafeteria. It's nicely presented, tasty, and nutritional.

I am a friend or relative of a resident

I had my grandmother at Montgomery Care and Rehab. It was a nice facility. I thought that the nurses and people cared. My grandmother was in a semi-private room, and the room was kept nice and clean. I would say their cafeteria needed to be enlarged because they had quite a few people in there, which was sometimes tight. It was pretty tight when they were serving meals. They brought patients out to sit in the hall, which was good because they got to sit with each other, but it really was just a pass in the hall. The people who were taking care of her did a good job. The people who did work there seemed to care about what they were doing. It was clean every time I went. They didn't go out very much, even in the backyard. They do have a backyard, but they didn't use that much. I guess they don't have time to take people out in the back, but I thought it was a nice place to sit. They did have like a little garden area in the back, and I believe it was fenced in, so if you had people who wanted to escape, I don't think they could. I liked the facility. It was nice and clean, and the people took care of each other. My grandmother was there about a long time, maybe ten years. I really could not complain about the way they took care of her.
