I visited this facility
When we walk into Jackson Square Skilled Nursing and Rehab Center, it smells old. It only has one elevator that breaks sometimes, and it takes them a while to fix it. The first floor is nice and clean. It's difficult to get a doctor there. We waited for almost two years before an eye doctor came in to check my mom-in-law's eyes because they were volunteer doctors. The caregivers are very attentive but have too many patients to look after. The nurses are inattentive and bathe my mom-in-law once a week, and I have to ask them to do that. The room is just like a hospital room. There's a cot on one wall for my father-in-law and then my mother-in-law is in a regular hospital bed. The odor on that floor is so bad, and it doesn't look clean.

I am a friend or relative of resident
This is the worst nursing home I have ever been to.. I have a relative living in this place, and wears depends, and every single time I visit which is daily, or every other day, I literally have to request for my family member to be changed. After having surgery on the rear end, the hospital discharge recommendation was to have the bottom dry at all times, and also have a wound care nurse change the wound at least 3X's daily, or more if needed due to bowel movements.. unfortunately, the wound only gets changed twice daily, and most time I have to send a request for that. The food that the residents eat for breakfast lunch, and dinner is unhealthy. I never ever saw them once give them any fresh fruit, or any 100% orange/apple juice. Instead, they give the residents some type of lemon cool-aid with sugar. They use to give out 2% milk, but they stopped giving that out months ago.. I requested INSURE for my family member, but was told by the nursing home doctor that he could get only two cans total*' I'm not sure exactly what that problem is.. but After talking to some of the staff members, It seems that their work loads are very heavy due to a shortage of CNAs. I can see that being true, 'cause most time I have to wait 15-30 minutes for a nurse assistance to aid (change) my family member if CNS is assisting another resident. PLEASE PLEASE PLEASE, if you care about your mom, dad, aunt, uncle, grandmother, or grandfather, or LOVE them then DO NOT SEND THEM TO THIS PLACE.. location is great, it's close, but it's really not worth it. It's best to send them somewhere further away, so that they can get the care, treatment, health, and nutrients that they need, especially if you are not going to be visiting them regularly making sure that Jackson Square staff do their jobs and take care of your love ones as they are suppose to.. at least give them that much. The facility is clean, secure and really provide excellent care to their residents.

I am a friend or relative of a resident
I had a great-uncle who was in Jackson Square Nursing Home last in 2011 and I visited him there multiple times. Overall the facility was clean and the staff were tolerant, but they seem to be very understaffed. Asking for anything out of the ordinary means you'll have to wait. My uncle was there temporarily--it was for about 3 months--after hip surgery. In terms of his medical needs, the staff was on top of things and my family really appreciated that. The facility also had activities on a daily basis and I found that this really helped my uncle to keep his spirits up during rehab for his hip. The physical therapist who worked with him was excellent!The only critical thing that I can say, which isn't even the fault of the facility, is that it is in a rather crime-riddled area. My sister's car was broken into during a visit. They do have a parking lot though, and that was the only incident like that which occurred during the three months we frequented Jackson Square.Overall, I found that Jackson Square provided adequate nursing services to my uncle.
