I am a friend or relative of a current/past resident

My dad lived in the Residence at Elim Park. The whole campus is beautiful and everyone who works there is lovely. The healthcare center has dining rooms for meals but also has a central area with restaurants, a market, a large theatre (Nelson Hall!!) and a wellness center with a pool and hot tub. There are a different levels of care from nursing home to independent living apartments. My dad lived in the Residential Care section which is like assisted living/senior housing that accepts Medicaid. He loved all the amenities and got to maintain more independence than he had living alone at home unable to drive. When he got sick he had priority to go to the skilled nursing and short term rehab units which are really highly rated and offered great care.

Sent home barely able to walk, stand, or get up from seated position on own. Has good insurance and Medicare but told 2week limit so made to leave. Diabetes out of control but had no dietician or nutritionist advice, sent home with no guidance. While there told out of oatmeal at least once. Told only heavy weight blankets available, had to get blankets from home since unable to tolerate weight on legs. Some meals not what was requested, at least one not edible.

I am a friend or relative of a current/past resident

ABSOLUTELY HORRIBLE PLACE. DO NOT LET YOUR LOVED ONE REHAB THERE!!!!! I discharged him home after only one day there. They take better care of their pet ducks and chickens than the patients in inpatient Rehabilitation. I will be filing a complaint with the Health Department. My father is a paraplegic confined to a wheelchair. We told the intake nurse that he could use a bathroom and/or bedpan and that he needed support stockings due to poor circulation. He also had a fungal infection in his groin. We visited him the next day at 1 p.m.. He was sitting in a diaper (which would make his fungal infection WORSE as it needs to air out), his legs were BLUE because of no compression stockings on and he wasn't shaved. My dad was able to tell us that he had buzzed for the nurse and when no one came, he defecated all over himself. The way they fixed it? Throw a diaper on him because it's easier to put a diaper on a partially paralyzed man than to take him to the bathroom. My children had to put on his support stockings and transfer him to the recliner in his room to elevate his legs. They also had to shave him. There were no CNA's to be found. I addressed my concerns with the head charge nurse that evening and she said that they would remove the diaper and that she would get to the bottom of what happened. The next day when we went to take him home, he was again sitting in a diaper. I spoke with the head of the Elim Rehabilitation and he apologized and admitted that they dropped the ball.
