Very kind nurses on the 4th floor. only one thing that there's not enough of nurses there to attend to all the residents. but there is a young lady there is so great. Ms. Hutchinson is a great asset to your organization. She exudes professionalism, she is compassionate. I really do thank God for her every day. I just want to thank you for having such a wonderful soul on your team. When God bring a soul to a team or to a person there's no mistake. Thank you, Freda Olds

THIS PLACE SHOULD BE AVOIDE LIKE THE PLAGUE; THEY TURN A DEAF EAR TO THE CONCERNS OF THE PATIENT AND ARE VER STANDOFFISH WITH FAMILY MEMBERS >REMOVED MY SISTER AS SOON AS ANOTHER FACILITY COULD BE FOUND;REFUSED TO PROVIDE TRANSPORTATION FOR THE MOVE AND WAS ALWAYS TARDY WITH MY SISTER RECEIVING HER MEDICATION AND GENERAL CARE

My father Mario arrived on July 4th '19 after spending 10 days in Bklyn Hospital Center for back injuries sustained during a series of falls. Upon entrance to The Phoenix we thought he would be in a wheelchair for at least 6 months since he had a hairline fracture of his L1 vertebrae along with other spinal issues. He was a patient at Phoenix for 5 weeks. It is a very well run, clean, organized facility. All of the staff we encountered was very patient focused, diligent, with my father- and he at times was a very difficult patient. People like Andrea (Supervising Nurse), Gabriel (Physical Therapist), Mr. Mohammed (Physical Therapist Supervisor), Ms. Foster (Nurse), Mrs. Singer (Social Worker), Ms. Shanona (Social Worker), all formed a team that ultimately did a great job . There was also a young lady (Speech Pathologist) whose name escapes me who administered to my father's hearing aid issues who was excellent. Also, a young lady whose name escapes me (Vocational Therapist) was wonderful to deal with. As it all turned out, Mario is doing much better than expected regarding his mobility issues, and is getting around the house on his own power quite well! All of this due to the strong team that served him at The Phoenix. In summation, we found we were in very good hands in this facility with lots of competent people around us. Thanks to you all!

I feel sorry for people who try to talk to anyone in this pathetic nursing home. After the phone is answered, the operator hangs up on you, pretending he does not hear you. I tried this from 3 different phones and the same game was played. No wander the rating for this rehab is "POOR". Run away from this nursing home.

The Phoenix Rehabilitation & Nursing Center was functional and had good physical therapy. They had a nice mix of people and had everything that was required. The food was fair. People seemed to be pretty active playing cards and watching movies, so they seemed to have enough activities. The rooms were small but functional. The staff was great.

5His place is Horrible!! Idon't know why the government hasn't shut this place. My dad went there for physical therapy. Now my dad makes on himself and has forgotten everything. He went there due to pain on his back and he has fallen from his bed since day one. Yes they say he is watching TV and what he was watching was the wall and a lady on a wheel chair who had machines on her and incoherent. The staff is there to eat laugh with each other, talk about their day and gossip besides have the staff from kitchen bring them tons of food such as milk oranges and putting and who knows what else to take home and feed their family cause I am sure one person can't eat all that. If you care for the patient. Please do not take them there. They will not go home healed. They will go directly to a grave. Someone in the government needs to have most of the staff members arrested/ put in jail for mistreating the patient and have this place shutdown. I pray for the patients there. For god to heal them and protect them from these Demons. Amen

My grandmother was just in this facility. It is horrible. The staff really does not care about the patients. She went into this place because she was losing the ability to walk and needed physical therapy each day which could not be provided at home. She ended up being confined to her bed here. Got bedsores and blisters. Which she never had EVER, before she went into the facility. They put her cellphone out of reach so she could not call us at night when we were not visiting, and tell us about how she was being treated. They kept her confined to that bed literally for about 21 hours a day. 1 hr off therapy and 2hrs in the "rec room" on her floor which is a small room they put a bunch of patients in, turn on a tv wayyyy in the corner and then face them towards the wall where they cannot even see the tv. If you really care about the physical AND mental health of your loved one, do not take them here. Mentally my grandmother is now messed up. They left her alone and confined to her bed so long that it was like solitary confinement . She was upbeat and coherent before she got there and now she is depressed and talks about her time there while crying.
