Awful place. They put clients in diapers even though they are not incontinent because staff is too lazy to help them get to the toilet. They don’t listen when relative is on a restricted diet due to their health condition, literally served food that could kill them and that was the end result. Avoid this facility.

My mother was in CareOne at Somerset Valley - SNF for rehab. Our experience was very good. They just showed very good care in each department. They had good physical therapy and OT. The social worker was good. Everybody was very attentive. They had Covid go through. They separated everybody nicely and timely, and it all worked out. It was very clean. The rooms were small. There were two people in a room, and they're tiny. They're older, but it's what you get around here, I think. I didn't try the food, but she liked everything. We didn't pay anything for the rehab.

This facility was like being in a nightmare. My father was here for only a week before we transferred him out. The rooms are filthy; smell like urine. There is a clear lack of aides and nurses are almost impossible to find. This was before Covid (Jan 2020). My father who had dementia was left unattended in the dining area (wearing his hospital gown) with a tray of food in front of him. He was unable to feed himself and was left alone to figure it out. RWJ Somerset forced this rehab on us saying there was no availability anywhere else (which I later found not true). Once we transferred him, he was able to rehab back on his feet and home a month later.

I was not able to sit up or walk when I came home from there. After 5 weeks at care one I was sent home with a putrid bedsore. They denied that the sore developed at their place and lied about it to me and in my records. IF YOU CARE ABOUT YOURSELF OR FAMILY DO NOT GO TO THIS PLACE!

I see a lot of old reviews and some for the assisted living this is for the Skilled rehab here. My grandfather was here for 3 weeks. When he was admitted he could barley walk with his walker. The PT department was absolutely amazing with getting him involved and giving him therapy 7days every week. The nursing director were very involved not just sitting in their office. Any special request was heard and if they couldn't accommodate they would let us bring everything in. I would highly recommend this facility to anyone looking for rehab. Couldn't thank them enough for getting grandpa home and walking again!

I will say we had a difficult time making a decision to pick a rehab facility during covid times. Reading some of the reviews didn’t help but I do take into consideration that people tend to only review negative experiences but I would like to tell you about our experience. From the start our first chats with Bianca was so detailed and assuring we decided to go with CareOne. We were assigned another social worker also named Bianca Z. She was so caring and kind. Made sure we were able to contact her at anytime. My dad went in for a leg infection after staying at the hospital for 21 days. He needed to regain his strength. The director of physical therapy, Elaine was beyond amazing! I can tell her goal was to get my dad better fast and safely. She asked key questions to how his living situation was to make sure he would go home and be able to function well in his own home. During my father’s hospital stay he ended up having to be on dialysis. They arranged transport to and from the facility and all went seamlessly. Art the director of food was amazing making sure his dietary needs were met. My dad actually said he liked a lot of the foods. The nurses were very diligent with his care. His wound was well taken care of and they did a great job with standard hygiene procedures. He said he always felt clean and clothes were changed regularly. Because of covid he was quarantined for his first 14 days. The facility visitation team, Enite and Will were absolutely amazing! They would schedule window visits for us and made sure we could see my dad daily. Knowing we could only see him though a window was tough but being able to see him when we wanted was amazing. Covid times make for difficult situation but we couldn’t have felt more comfortable that he was in great hands. Communication from all his care team was top notch. We were very happy with our experience and my dad he said he was going to miss the staff. I really felt so much understanding from the team, Bianca, Elaine, Art, Will and Enite really took an interest in our family and treated my dad like family! A very scary and hard decision ended up being really positive and amazing. We won’t forget the people that helped get him better and we are grateful for them. I hope this helps someone make their decision.

While the immediate caregivers are good, the administrators of this facility are especially awful. From the nurse supervisor to the social worker to the billing office, they are uncommunicative, unhelpful, ineffective, and, at times, downright hostile.

My mom spent the last year of her life at Care One Somerset Valley and I always felt she was very well care for and cared about. She was under hospice care for a good portion of her remaining months and I can't say enough about nice they all were to her. I was there a LOT and I got to know the aides, the physical therapists and the nurses there. I felt very good having her there. The food wasn't bad (I tried it myself) and they try to keep everyone engaged with activities. She was on the sub-acute side for 99% of her stay and it's a little clinical but that's to be expected but she was always treated well.

My dad was over medicated n undereducated all at once..not monitored enough w being unstable n diabetic. Therapy is lack there of with mostly having him eat and sleep often. Hard to get nurse and staff to call back and doctor hardly calls me to see concerns. Overcharged me as well..headache and hope things get solved asap. They should have online portal for logs of meds ..food..etc. 😟 rooms not spacious unless a private room. Staff nice but understaffed. They don't have many activities and they don't encourage him. Amenities there aren't much..should offer way more.

CareOne at Somerset Valley was great. The room wasn't pretty or ugly but was sufficient for my mom's needs. My mom was in their assisted living for 11 months. The staff was outstanding and cared for Mom. They were concerned and tried to address all of her needs. The food was good for my taste, but it needed more seasoning. They had a lot of things going on. My mom had physical therapy and light exercise in the common room, bingo, games, and various artists coming in like musicians and magicians. They did a very good job in keeping the place.

My dad is living at CareOne at Somerset Valley. We liked that it's close to where we live, and it's clean. They take care of him there, and the nurses are great, informative, take time to talk to you and tell you what they know, and the doctors are good. It's a good place. My dad gets therapy, and three meals and a snack. They offer the medical care that my dad needs, and they watch him.

If I could give them zero stars I would. I had to wait until I moved my dad oUT before writing a review. Where should I start...the management is a bunch of fabricators of the truth. They sell you many promises to get you moved in. My dad was examined by their doctor who recommended that he reside in the Memory Community. However, he didn't belong there, they just wanted the additional money charged for the Memory Community. They promised my dad would get out of bed and dressed everyday and would take part in numerous activities and off site trips. My 3 siblings and I would visit my dad at various times and days. Dad would be in the bed at 10-11am, he was never taken off site and was only offered bingo and coloring as activities. We noticed my active dad declining and becoming withdrawn. I could go on about the untrained, underpaid staff that is expected to work miracles with incompetent management, but I'll stop here. My dad has been in another facility for about 3 months now and there is such a noticable change. One last thing, they kept insisting that we put my dad on medication. If you visit the Memory Community, just take a look at the number of residents that appear to be over medicated. If you care for your lover one, don't move them into Care One!

I visited this facility

Food was terrible. Never had what I requested and always served Luke warm to cold.. Some nurses not capable... Gave me wrong meds, if I was not aware of it, that is a scary thing, especially due to most patients not capable of keeping track. Took 10 days to get a box of tissues !!!!! They were out of them!!!! Would not recommend the therapy , occupational or physical. Not organized at all . I felt I was kept there just to collect my insurance money... WOULD NOT recommend this facility whatsoever ...

I visited this facility

Care One At Somerset Valley is OK. The process was fine and good. My mom is on the memory floor since she has dementia. The staff has been very good, pleasant, and informative, so I'm pleased with all that. It's a new place, so it's very clean and very nicely decorated. It's current and not dated. It's very nice, and the people are very nice. The nurses and the support staff are very cooperative. They have activities; my mom has met some friends, and they keep them busy.

I visited this facility

Care One At Somerset Valley was very nice and good, and the rooms are superb. The staff is very professional, and the price was reasonable. The dining room was very nice also. Although I think it's reasonable, I thought it would be great if they could bring the price down a little bit. I liked the accommodations and the staff.

My mother was there for a month, recovering from car accident.. Other than close to my house, I can not make a good comment. Food is terrible. Caring staff does not exist at night. CNA is rude and incompetent. My mother drop to floor at night and had to rush to emergency room. This place is just plain terrible.

My dad was at CareOne. It was awful. They did have adequate staff, but they did not have any activity director. The one nurse and two assistants on at night were constantly sleeping. They basically run it like a hotel and not like an assisted living or another facility. You need to be there 10 to 12 hours a day to make sure that your loved ones are being cared for well.
