I am a friend or relative of a current/past resident

Thanks to the team at Deerfield that gave my aunt great care. The rehab was excellent. She goes there for rehab every time that she needs rehab. This is the only place that keep her for long term care and help with her Medicaid until she's ready to go back to her ALF. I appreciate everyone there as they are really special people that takes care of the sick.

Mom is at Deerfield Beach Health and Rehabilitation Center and our experience has been OK. It's certainly not the worst. It's certainly not the best. Their communication skills are a little slow. For the most part, I would give the staff a good review. Like anywhere, some are not so great, but most of them have been very friendly and helpful. Her room is fine. It's an older building, but it's well-kept and always clean. She doesn't complain about the food. It seems pretty good.

WELL NO ONE ANSWERS PHONES, BUT IF WE DIDNT MOVE MY SISTER OUT OF THAT PLACE SHE WOULD HAVE DIED.. WE WERE QUICK TO THINKING WHEN WE CALLED HER AND HER SPEACH WAS SLIRRED, NOT MAKING SENSE, CALLED REB MY NIECE CALLED NUMEROUS TIMES TO CHECK HER MOM AND SEE WHAT WAS GOING ON, WE DIDNT LIKE THE RESPONSE, WE WERE GETING FROM REAB.. AND CALLED A AMBULANCE TO TAKE HER TO HOSPITAL SHE IS NOW RECOVERING FROM PNUEMONIA SHE WILL BEVER GO BACK THERE EVEN THE DRS AT THE HOSPITAL WHEN WE ASKED THEM ON THIS PLACE,, THE CARE THERE.. WE GOT NO RESPONSE FROM, SO THAT TELLS YOU A STORY THEY KNOW BUT COULDNT SAY ANYTHING

My mother was in this facility for five days, the worst experience ever in my life. I would write this facility 00.This facility is the original one flew of the cuckoo‘s nest. No one to help my mother, feces on the door frame of the bathroom, just a very filthy facility. Please if you care about your loved one, never ever step foot in this dirty nasty place. Someone should call the state to have them shut down!!!

If I could give this place a -5 in stars I would. This place doesn't care about our loved ones at all. The Nurses all they are there for is a pay check. They don't come in when the patients are pushing the call button most of the time they don't even come in. I am very unhappy with this place. The person that I had to speak with that answered the phone a few times was very rude. They would see i was calling and would answer the phone and hang up. Complained about in and the same person kept doing it. If you don't like doing your job then go get a new one. The customer service was -100. In the almost 3 weeks my mom was in this place was more stressful then her being home. The only people that i spoke to that was not rude was the carrie the lady that gets you ready to leave and the lady that sets you up to come in the place. From what my mom said she only had a few people that would help her when she needed.

The place is awful. My father was placed there by the hospital. Within the first couple of hours my father fell on the floor after he was discovered hours later they put a blanket on him and was left there till PT could come help get him back into bed. That same afternoon, my daughter and I went in for a visit and my fathers legs and feet were extremely swollen, I said something to the nurse and she tried to put the bed up which did not work because the bed was broken. to make a log story short - they put him in a new bed which only collapsed twice and hurt his back. They don't give out medication properly. the place has bugs and is dirty. My father has mercer and they spilled his urine on the floor and just wiped it with a paper towel and then a dried it up with a regular white towel. They never answer there phones, the doctor took over a week to get back to me just so I could try to get him some pain medication. Now I am waiting for her to call me again with his back xrays since the bed collapse last week. I have never seen a place this bad and the nurses just have the CNA's do their jobs. I filed a complaint with the state of Florida and I am filing a complaint against 2 of the nurses with the department of health. My father got stuck here and I am trying to get him out before they seriously injur him or kill him from the falls/collapsing beds he is on which could rupture his 5.6cm aortic aneurysm at any time. DO NOT PUT YOUR FAMILY or FRIENDS HERE! If you do visit your own Dr first for Blood Pressure Pills - you will need them!!!

I visited this facility

This SNF/ rehab offered substandard care. I honestly don’t know how they are still open and i am a health care worker. I walked in on a Sunday to visit my mom and saw 4 patients lined up against the wall next to the nurses station. 2 in geri chais that wete in bed position and 2 in wheelchairs. 1 patient had a sheet wrapped around his body with only feet exposed. I asked the nurse what was this 3 times thinking it was a dead body in the hallway with no answer then i saw the toes move. The second man was strapped down by the waist and he was trying to get out of the geri chair. His torso was off the chair with one hand on the floor and the staff just kept walking by. The other 2 were screaming at the top of their lungs help. My moms bathroom stunk of stale urine. We asked for housekeeping to clean it. The lady finally came in muttering to herself “i cant stand this place”. My mom was there for iv antibiotics and physical therapy and the pumps for the iv have been sitting on the nightstand for the last 2 days. Seems to me if youre going to give iv antibiotics the pump should at least be attached to the pole. The next thing is my mom is on eliquis and she needed to go to the bathroom. Her roommate yelled help me o the nurse as my weakened mom tried to go to the bathroom on her own. The nurse looked in and said that the aide was in another room helping another patient and walked out. It took 10 minutes for her to get back there. Dr dazil took 24 hours before calling me back because i had concerns. I had to call her supervisor at team health before she responded. Her first words to me were, imvery busy doing work. Patients have their tv and music up full blast throughout the night and there is no quiet time ther for rest. The curtains were visibly soiled with i dont know what as well as the floors. I would expect this in a third wold country but not i America. Substandard living and health conditions. I wouldnt take my dog there. I took my mom out. The only positive was the speech therapist, physical therapist on monday and a couple of the nurses and aides. Thats why i gave this place a 1 star. My sister said it should 0. Visit this place first before you allow you loved ones to be taken there. That was my mistake. All SNF’s / rehab not created equal

This place is a joke. It's very dirty.My friend did not feel safe,woke up to nurse sleeping in her chair.She needed soft foods which barley came and were always cold. She claims it was worse care than in the hospital. We wanted to bring her home and they said No.Theycalled the social worker for 3days and no response so we went to take her home and they locked the doors on them. Finally having to get nasty thry gavr us papers to sign.Wouldnt let my dog rehab there.

My father spent one week at this facility and was treated HORRIBLY! He was yelled at by a staff member (Rose) that he was not to call her to his room again, that was she would not come. This was a witnessed event. He fell from his bed that evening. Its is unknown how long he laid on the floor unattended. He was not taken to the hospital asap nor was the doctor promptly notified, per protocol. While at phys therapy, his mattress was changed to a lessor grade and his cloths thrown all over the room along with food and paper products. When asked about this witnessed event, the night manager advised me, he knew nothing of it. The staff failed to bring my father regular meals and his required medications. I spoke to the staff at least 6 times to place a phone in my fathers room as there was none. The staff played loud music late at night causing patients much disruption and did not answer the patient call button, nor did they answer phone calls promptly. There were many, many times, no one answered my calls. May father was left sitting in a wheel chair for apprx 2 hours unattended. One staff member was witnessed supplying medication to a non-patient. It is my opinion, the staff at this facility is completely untrained and uncaring! Do not place your loved one in this facility!

Stay away! My father was almost septic after 4 days. The care is horrific. The nurse told me I was over reacting when I said my dad was sick and needed medical attention. The halls stink of urine and bowel movements. No one cares about the patients. It’s a disgrace.

My brother was placed here .dealing with the business office is horrible. They are very unprofessional . Grossley under staffed. They do not answer the phone. Nor do they return calks from messages left. My brother had items stolen from his room. I'm looking for another facility.

My sister was transferred here yesterday from the hospital. The phone system is deplorable. There are no phones in the room. I try to call the wing and it rings and rings and then says "no one is answering, hang up and call back later." I got a human one time, asked for the room and got cut off. It's frustrating when you can't communicate with a patient before a visit, to see if they need or want anything. I can't rate anything else yet, since I don't know what's going on and can't get through on the phone.

I read the horrible reviews and was quite worried because I had to send my mother here for rehab (and didn't have many other convenient options) after being hospitalized. I actually found the facility to be fairly clean. There were no bugs and nothing disgusting as others have mentioned in their reviews. There is some construction going on as they are renovating the rooms and so there is the smell of paint. The nurses were very caring and took good care of my mother. At times they are busy and we'd have to wait a little longer than we hoped, but they were apologetic about the wait and helped out as soon as they were available. The therapists were great too. It isn't the Ritz Carlton... but I was satisfied and don't regret my decision one bit.

Unfortunately my family wasn't given much time in choosing a facility to place our father for short term rehab after being hospitalized. Sadly, we ended up here. In retrospect, I should have just taken him home. This place is pathetic. No work ethic, let alone caring for the patients. It took hours for anyone to answer a question. The place was filthy with bugs crawling everywhere. Doctors would not call back when paged. I couldn't get my father out of there fast enough! Do yourself a favor and steer clear of this place. Don't be fooled by its 4 star rating. That's a joke.

I visited this facility

Very disappointed in this place. Filthy environment, patients bathroom not fit for humans. Services were herrendous, waited over two hours for a glass of water, left a patient nude with door and curtain open. Nurses always to busy to talk to you, no one new what the other was doing. Very little English spoken. I would highly suggest you go elsewhere. Most depressing, filthy, place I have been in.

Bad Bad Bad The place is not for humans or animals its that bad, came to visit my relative the bed was with cocoraches , food a scoop of salad with crackers, no equipment to be fond to measure blood pleasure, doctor asked for twisers to removed stiches didnt have one, call the ambulance to take him to the hospital nobody at the front door which is look and have to have a code to open and they couldnt open it, i have the code from the night before and open the door for the ambulance to take the patience to the hospital, I am very angry.

My husband was here to receive physical therapy after knee replacement surgery. When he arrived, the room had not been cleaned; personal hygiene items of the previous client were left in the room, the floor had not been washed, the bathroom was filthy with feces left in the toilet bowl and the sink filled with scum. The bed had a sheet and pillow no blanket. When we asked for a blanket we were told there were none and he was given another sheet. There were no towels available either. The food tray was brought in with only the main plate covered. The metal utensils were uncovered as was the beverage. His pain medication and blood thinner medication were not there upon arrival. We were told they would be there by 11:00 that night and he would be given them at that time. That never happened. This next morning he called for a nurse at 8:00 ; no one came until 9:20. At that time he was given a Tylenol, still no blood thinner. I picked him up from that place at 10:30 that morning. Still no medications, no towel to wash with, no bathroom cleaned, no help to dress. He left with me at 11:00. I would never leave anyone here. It is an awful place!

Deerfield Beach Health and Rehabilitation Center was nicer looking. It was less dreary and less old. However, when I went to see my aunt, she was lying in the fetal position. I understand she is very stubborn. She does have dementia, so she is not always cooperative. All I told these people: If you're a little firm with her, you can get her to work with you. I never saw any of it. The place was nicer kept; it was more up-to-date. It was clean, and the people were very nice. Those were important, but that's not what my aunt was there for.

My mother was there and while the rooms were spacious the attentiveness to providing meds and treatments such as asthma were subpar including cleaning patient or helping restore ROM they just let your loved ones lay in bed you have to beg to get help in getting your loved one up where's the physical therapy? if they cannot on their own. Some nurses care and offer attention but not all possibly due to ratio of patient to staff not safe allow anyone in no ID required saw man sleeping in lobby with his bike outside I was so scared that he could harm those vulnerable older patients not just my mother praying for all there

My father and mother both were here at different times, both horrible experiences. You will eat 3 times a day but don't get your hopes up for much more than that.Cell phones vanished. Physical therapy gets rushed and shortened or skipped. Patients can only lay in bed or sit in a wheelchair, no other seats available, no medically approved recliners. The nurse didn't recognize my dad's confusion as a seizure coming on and didn't get sent to hospital until hours later, he never recovered and passed away soon after. They are content to leave patients in diapers rather than help them to the toilet or offer a bedpan. They don't use absorbent pads for the diapers so patients wet themselves and the bed. They do not give referrals when you are discharged to any one other than their own connections for physical therapy even if you are in a facility with a physical therapy department!

I found Deerfield Beach Health and Rehab to be a clean facility. The rooms were adequate. The people were very attentive to my mother, but I think they need to improve on the ratio of their employee to patients. Their food was not good and they need to improve on that as well.

I visited this facility

When I visited North Broward, they hardly explained anything. They just showed me the dining room and they didn't show me their rooms. They just took me where they want to take me. Also there was nobody in their dining room. It felt very cold. Some places were very friendly, but this one seemed like a cold place for me. They rushed us through the facility. The visit was like 10 minutes and that was it. The facility wasn't clean and it seemed like the people that even took me around weren't knowledgeable as well.
