Rooms were dirty. Hardly ever cleaned. Place stinks. Patients are not routinely checked on. Very poor on all counts. Could go on and on with more.

The facility is old and not well kept. Often seen are broken items. The place smells like urine. There’s no one at the entrance and they just tell you the security code as a family member so you can get in. I’m sure that never gets changed. The rooms are tiny! Like they should be one person rooms but instead shove two people in. There’s no space to visit a loved one. They’re grossly understaffed. On the skilled nursing facility side it’s one nurse and one aide in the evenings. They don’t inform you when your loved ones fall. Only once of we’re guessing at least four times he fell were we contacted. While the staff is friendly, your loved one will likely sit in their own waste for a while because of lack of staffing. There’s four to a bathroom (it’s shared between two tiny rooms). No human deserves this. He didn’t get bathed. He left in someone else’s dirty clothing with more bruises and injuries than he arrived. Do not send your loved ones here.

I am/was a resident of this facility

I'm living in Meadowcrest Nursing Center because I broke my leg. It's assisted living and rehab. I've been here for 3 weeks now for rehab. The place is older so it could use some help with it. The staff is very nice, very friendly, and they do whatever you ask them to do. There are activities every day. There's one at 10:30 and one at 2:00. Last week, we planted flowers in pots and then we decorated the pot. We also played dominoes and I had ice cream, donuts, and a Turkey dinner. You could eat in your room because of COVID. They haven't opened up the dining room yet. You could go to the dining room if you want to but mostly, everybody stays in their rooms. The food is disgusting, it's very bad. Yesterday, we had pulled pork sandwiches. First of all, they put it on a hot dog bun because they didn't have buns. Secondly, they mixed baked beans with the barbecue sauce for the pulled pork and it was pretty bad. The room is small, with two beds and a dresser, and a cupboard in the bathroom. They could get a better cook. The biggest thing to improve is the food here, it's really bad.
